Periapical lesions are caused by bacterial infections. The fundamental objective of endodontic treatment is to eliminate bacteria present in the root canal system because they play an important role in the development and maintenance of periapical lesions. Therefore, confirming the absence of bacteria before filling root canals is important. Anaerobic culture tests have been used in many endodontic cases, and they have brought about good treatment outcomes. These tests, however, require specific apparatuses and bacteriological techniques. Here, we report a chairside anaerobic culture test that does not require any specialized apparatuses or techniques. We also report two endodontic cases in which this simple test was used. Both patients were diagnosed with chronic purulent periapical lesions. After confirming the absence of bacteria in the root canals, they were filled with gutta-percha points. At followup, the radiolucencies showed recovery, although longterm observation is under way. From these results, the authors conclude that this simple chairside anaerobic culture test is effective for evaluating periapical lesion treatment procedures.

卒後に適切な歯周治療を行うためには卒前から十分な知識とできる限りの臨床能力を身につける必要があるが, 一方で歯学生の診療技術や治療の流れについての理解の低さも問題視されている。歯周治療全体の流れを理解させるためには診療参加型臨床実習を行うことが有効だと考えられている。今回, 診療参加型臨床実習で第5学年の学生に60歳女性の歯周炎患者を同意を得た後に配当した。本患者は抗がん剤治療直後で体力の低下を認めた他, 歯科治療に対して恐怖心が強く診療に注意が必要であったが, 十分な説明と慎重な診療により特に問題なく治療を進めることができた。歯肉の炎症, 臼歯部を中心とした垂直性骨吸収, 咬合上の問題等解決すべき点があったが, 学生が診査・診断, 治療計画立案, 歯周基本治療, 歯周外科処置, 口腔機能回復治療に参加し, 現在はsupportive periodontal therapy(SPT)を行っている。学生による診療について患者からフィードバックを得た。学生の説明や緊張の緩和等に関しては不十分な点も指摘されたが, 痛みに対する配慮や基本的なマナー等に対する評価は高く, 治療全体に対しても満足しているという結果が得られた。今回の診療参加型臨床実習は歯周治療の一連の流れを理解させるのに有効であったと思われる。また, 診療参加型臨床実習での治療内容を報告することは, 学生のモチベーション向上等高い教育効果が得られると期待される。日本歯周病学会会誌(日歯周誌)52(4) : 419-428, 2010

The Tooth Cleaning Habits of Rural India: Traditional Vs Contemporary

There are many causes for oral malodour, and tongue coating, periodontitis, deep caries and unfitted restorations are main causes of bad breath odour. It is not difficult to reduce oral malodour by careful examination and proper treatment. However, we sometimes encounter oral malodour associated with other problem. Here, we report a case of persistent oral malodor associated with periodontitis caused by tooth perforation. The patient (a 41 year old, female) had visited our breath clinic complaining of a strong breath odor. Oral malodor was detected by several tests including organoleptic test, portable sulfide monitor and gas chromatography. She had consulted two dentists to reduce her breath odor. The dentists performed periodontal treatments including scaling, and root planing. However, her oral malodour did not decrease, and she had been worrying about her breath odor for 15 years. The patient’s oral hygiene was not bad at the first visit. Periodontal condition was not severe except tooth 24, and there were deep pockets in the proximal of the tooth. We performed initial periodontal treatment, but the condition of 24 did not change. We performed a surgical inspection and perforation of the tooth 24 was considered to be the main cause of periodontitis. Large number of periodontopathic bacteria was detected from the saliva and examination revealed the strong oral malodor. After extraction of the causative tooth, periodontal condition improved, and bacterial number was decreased. The patient’s breath odor was reduced to less than the threshold level and she was free from long-lasting worries on oral malodor.

Halitosis is a condition that has health and social implications. The origin of breath malodour problems are related to both systemic and oral conditions. In some cases malodor is not physically detected, but the patient still feels that they have bad breath. Halitosis is one of common reason for social problems between couples, In this case report, we describe a family case of non oral pathological halitosis caused by sjörgen sydrome, and pseudo halitosis.

Here, we report a case of severe tooth wear associated with a patient's inappropriate efforts to reduce oral malodor. A 72-year-old male patient visited our breath clinic complaining of strong breath odor. Former dentists had performed periodontal treatments including scaling and root planing, but his oral malodor did not decrease. His own subsequent breath odor-reducing efforts included daily use of lemons and vinegar to reduce or mask the odor, eating and chewing hard foods to clean his teeth, and extensive tooth brushing with a hard-bristled toothbrush. Oral malodor was detected in our breath clinic by several tests, including an organoleptic test, portable sulphide monitor, and gas chromatography. Although patient's oral hygiene and periodontal condition were not poor on presentation, his teeth showed heavy wear and hypersensitiving with an unfitted restoration on tooth 16. Radiographic examination of the tooth did not reveal endodontic lesion, but when the metal crown was removed, severe pus discharge and strong malodor were observed. When this was treated, his breath odor was improved. After dental treatment and oral hygiene instruction, no further tooth wear was observed; he was not concerned about breath odor thereafter.

本症例は原因が口腔内にある真性口臭症であったが, 本人には自覚がなく歯科治療に対する意欲も少なかった。強い口臭を指摘すれば歯科治療を積極的に受ける可能性があるが, 一方で必要以上に口臭を気にし始め社会生活が困難になる危険もある。今回, われわれは歯科医師と歯科衛生士が協力し, 口臭に対する不安を生じさせずに治療に対するモチベーションを高めるように努力した。患者は56歳, 女性で初診の約2年前から家族から口臭を指摘されるようになったとのこと。本人には口臭の自覚がまったくないが, 夫のすすめで本院口臭クリニックを受診。初診時の口臭検査は全ての項目について「強い口臭」と判定された。口腔清掃は不良で舌苔付着も多かった。全顎的に歯周炎に罹患しており, 歯根破折や齲蝕も認められた。口臭の原因を説明し, 放置していると口臭が強くなることを説明したところ, 治療に積極的に参加するようになった。治療開始後の患者は協力的で順調に治療が進行した。抜歯を含む歯周治療を行ったが, 歯科衛生士はモチベーションの維持, 口腔衛生指導, 口臭に対する不安の解消等に努めた。治療後の歯周組織の状態は安定しており, 口臭も全ての検査で陰性であった。モチベーションが成功したため, 家庭での清掃が継続され長期間にわたって安定した歯周環境と低い口臭を維持できたと考えられる。このように, 真性口臭症の治療では歯科衛生士の役割は重要であると考えられる。日本歯周病学会会誌(日歯周誌)50 : 50-57, 2008
