In the healthcare domain, a transformative shift is envisioned towards Healthcare 5.0. It expands the operational boundaries of Healthcare 4.0 and leverages patient-centric digital wellness. Healthcare 5.0 focuses on real-time patient monitoring, ambient control and wellness, and privacy compliance through assisted technologies like artificial intelligence (AI), Internet-of-Things (IoT), big data, and assisted networking channels. However, healthcare operational procedures, verifiability of prediction models, resilience, and lack of ethical and regulatory frameworks are potential hindrances to the realization of Healthcare 5.0. Recently, explainable AI (EXAI) has been a disruptive trend in AI that focuses on the explainability of traditional AI models by leveraging the decision-making of the models and prediction outputs. The explainability factor opens new opportunities to the black-box models and brings confidence in healthcare stakeholders to interpret the machine learning (ML) and deep learning (DL) models. EXAI is focused on improving clinical health practices and brings transparency to the predictive analysis, which is crucial in the healthcare domain. Recent surveys on EXAI in healthcare have not significantly focused on the data analysis and interpretation of models, which lowers its practical deployment opportunities. Owing to the gap, the proposed survey explicitly details the requirements of EXAI in Healthcare 5.0, the operational and data collection process. Based on the review method and presented research questions, systematically, the article unfolds a proposed architecture that presents an EXAI ensemble on the computerized tomography (CT) image classification and segmentation process. A solution taxonomy of EXAI in Healthcare 5.0 is proposed, and operational challenges are presented. A supported case study on electrocardiogram (ECG) monitoring is presented that preserves the privacy of local models via federated learning (FL) and EXAI for metric validation. The case-study is supported through experimental validation. The analysis proves the efficacy of EXAI in health setups that envisions real-life model deployments in a wide range of clinical applications.

Explainable AI for Healthcare 5.0: Opportunities and Challenges

Pneumonia is a life-threatening lung infection resulting from several different viral infections. Identifying and treating pneumonia on chest X-ray images can be difficult due to its similarity to other pulmonary diseases. Thus, the existing methods for predicting pneumonia cannot attain substantial levels of accuracy. This paper presents a computer-aided classification of pneumonia, coined Ensemble Learning (EL), to simplify the diagnosis process on chest X-ray images. Our proposal is based on Convolutional Neural Network (CNN) models , which are pretrained CNN models that have been recently employed to enhance the performance of many medical tasks instead of training CNN models from scratch. We propose to use three well-known CNNs (DenseNet169, MobileNetV2, and Vision Transformer) pretrained using the ImageNet database.

On the Analyses of Medical Images Using Traditional Machine Learning Techniques and Convolutional Neural Networks

Deep learning (DL) has been introduced in automatic heart-abnormality classification using ECG signals, while its application in practical medical procedures is limited. A systematic review is performed from perspectives of the ECG database, preprocessing, DL methodology, evaluation paradigm, performance metric, and code availability to identify research trends, challenges, and opportunities for DL-based ECG arrhythmia classification. Specifically, 368 studies meeting the eligibility criteria are included. A total of 223 (61%) studies use MIT-BIH Arrhythmia Database to design DL models. A total of 138 (38%) studies considered removing noise or artifacts in ECG signals, and 102 (28%) studies performed data augmentation to extend the minority arrhythmia categories. Convolutional neural networks are the dominant models (58.7%, 216) used in the reviewed studies while growing studies have integrated multiple DL structures in recent years. A total of 319 (86.7%) and 38 (10.3%) studies explicitly mention their evaluation paradigms, i.e., intra- and inter-patient paradigms, respectively, where notable performance degradation is observed in the inter-patient paradigm. Compared to the overall accuracy, the average F1 score, sensitivity, and precision are significantly lower in the selected studies. To implement the DL-based ECG classification in real clinical scenarios, leveraging diverse ECG databases, designing advanced denoising and data augmentation techniques, integrating novel DL models, and deeper investigation in the inter-patient paradigm could be future research opportunities.

A Systematic Review of Computational Image Steganography Approaches

Pneumothorax is a thoracic disease leading to failure of the respiratory system, cardiac arrest, or in extreme cases, death. Chest X-ray (CXR) imaging is the primary diagnostic imaging technique for the diagnosis of pneumothorax. A computerized diagnosis system can detect pneumothorax in chest radiographic images, which provide substantial benefits in disease diagnosis. In the present work, a deep learning neural network model is proposed to detect the regions of pneumothoraces in the chest X-ray images. The model incorporates a Mask Regional Convolutional Neural Network (Mask RCNN) framework and transfer learning with ResNet101 as a backbone feature pyramid network (FPN). The proposed model was trained on a pneumothorax dataset prepared by the Society for Imaging Informatics in Medicine in association with American college of Radiology (SIIM-ACR). The present work compares the operation of the proposed MRCNN model based on ResNet101 as an FPN with the conventional model based on ResNet50 as an FPN. The proposed model had lower class loss, bounding box loss, and mask loss as compared to the conventional model based on ResNet50 as an FPN. Both models were simulated with a learning rate of 0.0004 and 0.0006 with 10 and 12 epochs, respectively.

A Systematic Review on Acute Leukemia Detection Using Deep Learning Techniques

Colorectal Cancer (CRC) is the third most dangerous cancer in the world and also increasing day by day. So, timely and accurate diagnosis is required to save the life of patients. Cancer grows from polyps which can be either cancerous or noncancerous. So, if the cancerous polyps are detected accurately and removed on time, then the dangerous consequences of cancer can be reduced to a large extent. The colonoscopy is used to detect the presence of colorectal polyps. However, manual examinations performed by experts are prone to various errors. Therefore, some researchers have utilized machine and deep learning-based models to automate the diagnosis process. However, existing models suffer from overfitting and gradient vanishing problems. To overcome these problems, a convolutional neural network- (CNN-) based deep learning model is proposed. Initially, guided image filter and dynamic histogram equalization approaches are used to filter and enhance the colonoscopy images. Thereafter, Single Shot MultiBox Detector (SSD) is used to efficiently detect and classify colorectal polyps from colonoscopy images. Finally, fully connected layers with dropouts are used to classify the polyp classes. Extensive experimental results on benchmark dataset show that the proposed model achieves significantly better results than the competitive models. The proposed model can detect and classify colorectal polyps from the colonoscopy images with 92% accuracy.
