This is the proceedings of an international symposium held to evaluate moire fringe topography in patients with spinal and other musculoskeletal deformities. It consists of 34 papers presented by 58 persons, both physicians and engineers, from seven countries. There is an excellent index and a very comprehensive bibliography of 254 entries. In addition to the individual research papers, the proceedings of workshops on instrument design and clinical standardization of data are presented. Several panel discussions provide an excellent interchange between the biomedical engineer and the physician, and cover three topics: (1) standardization of techniques, (2) state of the art of screening for scoliosis, and (3) setting goals for future research. The respiratory inductance plethysmograph (RIP) was used to record resting breathing for 45 min in 12 patients with stable chronic obstructive pulmonary disease (COPD) and 8 age- and sex-matched control subjects. The COPD group had mean FEV1.0 of 31% predicted (range: 15 to 49%), mean PaO2 of 70 mmHg (56 to 83 mmHg), and mean PaCO2 of 37 mmHg (31 to 47 mmHg). All subjects were studied in the sitting position, and the rib cage (RC) and abdomen (ABD) RIP signals were simultaneously recorded on a polygraph and sampled at 20 Hz by a microcomputer. The summed RC and ABD signals were processed to create a spirogram from which the timing components of the respiratory cycle were subsequently analyzed. The mean number of breaths analyzed per subject was 702 +/- 213 (SD). Mean tidal volumes were identical in both groups. Inspiratory and expiratory times were significantly less (p less than 0.01), and mean inspiratory flow was significantly greater (p less than 0.01) in the COPD group. Frequency and minute ventilation also were significantly greater in the COPD group (p less than 0.005). Variability of breathing pattern, assessed in terms of coefficients of variation, was significantly less for TI, TI/Ttot, and VI in COPD patients than in normal subjects, even after sighs had been excluded from the analysis. We suggest that alterations in breathing pattern and its variability reflect changes in neural control of breathing consequent to disease.

Breathing Patterns in Patients with Chronic Obstructive Pulmonary Disease

The most characteristic clinical signs of stroke are motor and/or sensory involvement of one side of the body. Respiratory involvement has also been described, which could be related to diaphragmatic dysfunction contralateral to the brain injury. Our objective is to establish the incidence of diaphragmatic dysfunction in ischaemic stroke and analyse the relationship between this and the main prognostic markers. A prospective study of 60 patients with supratentorial ischaemic stroke in the first 48 h. Demographic and clinical factors were recorded. A diaphragmatic ultrasound was performed for the diagnosis of diaphragmatic dysfunction by means of the thickening fraction, during normal breathing and after forced inspiration. Diaphragmatic dysfunction was considered as a thickening fraction lower than 20%. The appearance of respiratory symptoms, clinical outcomes and mortality were recorded for 6 months. A bivariate and multivariate statistical analysis was designed to relate the incidence of respiratory involvement with the diagnosis of diaphragmatic dysfunction and with the main clinical determinants. An incidence of diaphragmatic dysfunction of 51.7% was observed. 70% (23 cases) of these patients developed symptoms of severe respiratory compromise during follow-up. Independent predictors were diaphragmatic dysfunction in basal respiration (p = 0.026), hemiparesis (p = 0.002) and female sex (p = 0.002). The cut-off point of the thickening fraction with greater sensitivity (75.75%) and specificity (62.9%) was 24% (p = 0.003). There is a high incidence of diaphragmatic dysfunction in patients with supratentorial ischaemic stroke which can be studied by calculating the thickening fraction on ultrasound. Among these patients we have detected a higher incidence of severe respiratory involvement.
