Detection of rupture-prone atherosclerotic plaques by time-resolved laser-induced fluorescence spectroscopy

Background—The individual makeup of atherosclerotic plaque has been identified as a dominant prognostic factor. With the use of an intravascular magnetic resonance (MR) catheter coil, we evaluated the effectiveness of high-resolution MR in the study of the development of atherosclerotic lesions in heritable hyperlipidemic rabbits. Methods and Results—Sixteen hyperlipidemic rabbits were investigated at the ages of 6, 12, 24, and 36 months. The aorta was studied with digital subtraction angiography and high-resolution MR with the use of a surface coil and an intravascular coil that consisted of a single-loop copper wire integrated in a 5F balloon catheter. Images were correlated with histological sections regarding wall thickness, plaque area, and plaque components. Digital subtraction angiography revealed no abnormalities in the 6- and 12-month-old rabbits and only mild stenoses in the 24- and 36-month-old rabbits. High-resolution intravascular magnetic resonance imaging: monitoring of plaque formation in heritable hyperlipidemic rabbits.

The advent of ultra-fast imaging techniques has extended the utility of magnetic resonance imaging (MRI) from a static and purely diagnostic status to an imaging modality ideally suited for a number of therapeutic applications. These advances--along with the recent development and refinement of miniature intravascular imaging catheters and MRI-compatible guidewires, balloon catheters, and radiofrequency ablation catheters--have created an exciting forum of novel approaches for detecting and treating both acquired and congenital cardiovascular disease. This review covers the current state of the art in fast cardiovascular MRI, catheter-tracking techniques for MR fluoroscopy, and currently available interventional MRI systems. Early diagnostic and therapeutic applications, such as high-resolution intravascular and intracardiac imaging, balloon angioplasty, stent placement, and radiofrequency ablation techniques, are discussed and extended to several potential approaches specific to pediatric cardiac therapeutic catheterization. Lastly, safety aspects of MR-guided interventional procedures are presented.

Real-Time Magnetic Resonance Imaging: Diagnostic and Interventional Applications

PURPOSE: To develop a technique for intravascular magnetic resonance (MR)–guided balloon angioplasty with use of an MR imaging guide wire. MATERIALS AND METHODS: An MR imaging guide wire (0.6-mm loopless antenna) that could be placed within a balloon catheter was manufactured. The guide wire was expected to function as either an MR receiver probe in real-time MR imaging or a guide wire for use with interventional devices. Laparotomy was performed in eight rabbits, and a dilatable stenosis was created at the upper abdominal aorta. Balloon angioplasty, validated at pre- and postoperative MR aortography with renal contrast enhancement was performed by using a 1.5-T MR unit with a fast spoiled gradient-echo pulse sequence, short repetition and echo times, and a rate of three frames per second. Intravascular MR imaging- guided balloon angioplasty with an MR imaging guide wire : Feasibility study in rabbits

We review fluorescence lifetime techniques including time-resolved laser-induced fluorescence spectroscopy (TR-LIFS) and fluorescence lifetime imaging microscopy (FLIM) instrumentation and associated methodologies that allow for characterization and diagnosis of atherosclerotic plaques. Emphasis is placed on the translational research potential of TR-LIFS and FLIM and on determining whether intrinsic fluorescence signals can be used to provide useful contrast for the diagnosis of high-risk atherosclerotic plaque. Our results demonstrate that these techniques allow for the discrimination of important biochemical features involved in atherosclerotic plaque instability and rupture and show their potential for future intravascular applications.

Fluorescence lifetime in cardiovascular diagnostics

PURPOSE: To assess the potential role of intravascular magnetic resonance (MR) imaging with receiver coils mounted to an inflatable balloon in characterizing atherosclerotic plaque. MATERIALS AND METHODS: Twelve human harvested segmental femoral arteries with atherosclerotic changes were studied with a 5-F imaging balloon catheter equipped with a single-loop wire receiver coil. Imaging was performed with an open-configuration 0.5-T (n = 6) or a 1.5-T (n = 6) MR system, with T1- and T2-weighted sequences. Histologic analysis was the reference standard, and MR images were analyzed with regard to vessel wall thickness, plaque area, and components. RESULTS: Images acquired at 1.5 T were characterized by better in-plane resolution (117 x 104 microm). Resolution at 0.5 T (234 x 178 microm) was sufficient to discriminate the wall layers.
