Cardiovascular risk (CVR) in patients with calcium pyrophosphate crystal deposition disease (CPPD) has not been studied, and the optimal method for assessing it has not been established yet. Objective : Evaluation of CVR and comparison of results using Adult Treatment Panel III (ATP III) and Reynolds Risk Score (RRS) scales in patients with CPPD, gout, rheumatoid arthritis (RA) and in the control group. Materials and methods : Cross-sectional, single-center study performed by case-control method. There are 42 patients with CPPD in main group, 42 patients with gout and RA in the comparison groups are, 42 healthy volunteers in the control group. The survey included measurements of anthropometric measures, blood pressure (BP), serum glucose, creatinine, cholesterol (TC), high density lipoproteins (HDL), low density lipoproteins (LDL), C-reactive protein (CRP). CVR was assessed on ATP III and RRS scales, comparison of its evaluation results was carried out between groups and between scales within groups. Results and discussion : Most of the parameters in the compared groups did not differ. However, HDL CS levels were significantly higher in patients with CPPD and in the control group than in RA and gout ( p 0.05). The CRP > 5 mg/L in CPPD and in the control group was greater than in RA ( p <0.05) and gout ( p <0.05), but CRP≥5 mg/L was determined in 18 patients (43%) with CPPD and only in 3 (7%) people in the control group ( p <0.05). A high and very high risk of cardiovascular disease (CVD) on the ATP III scale in CPPD was noted in 5 (12%) in gout – in 7 (17%), in RA – in 9 (21%) and in the control group – in 8 (19%) cases. Its frequency in all groups was comparable. A high and very high risk of CVD for RRS was identified in 9 (21%), 14 (33%), 12 (29%) and 7 (17%) cases, respectively. Conclusions : CVR under CPPD, RA and gout is comparable and quite high. The RRS scale may be a more objective method of assessing CVD risk in patients with CPPD, gout and RA.

Colchicine is an alkaloid isolated from plants of the Colchicum genus. Colchicine has been used for thousands of years and remains one of the few drugs whose use remains relevant today. The therapeutic use of colchicine is widely known for gout, familial Mediterranean fever, Behcet’s disease, cardiovascular diseases (pericarditis, coronary heart disease, pericarditis, after coronary artery bypass grafting, etc.) and other diseases and is due to anti-inflammatory effects, which are based, including , inhibition of NLRP3 inflammasome and interleukin (IL)-1β production. The mechanisms of anti-inflammatory action of colchicine are diverse. It is primarily an inhibitor of mitosis and microtubule assembly. Colchicine destroys the structure of microtubules and reduces the elasticity and relaxation of neutrophils, thereby preventing the extravasation of neutrophils from blood vessels to the site of inflammation. Systemic inflammation is also a hallmark of coronavirus disease (COVID-19), which develops immunological disorders accompanied by the  production of  a large number of  pro-inflammatory cytokines, including interleukin-1. Coronavirus disease 2019 (COVID-19), which has become the most urgent medical problem in the world in the last 2 years, given the mechanisms of inflammation similar to rheumatic diseases, is also considered a disease with the potential effectiveness of colchicine treatment. SARS-CoV-2 vaccines also have a trigger factor for the development of an exacerbation of arthritis. On average, colchicine use was associated with a 47% reduction in post-vaccination arthritis. This article provides an update on the mechanisms of action and current experience with colchicine in COVID-19, including in patients with microcrystalline arthritis.

Colchicine for the treatment of COVID-19: short path from theory to practice

The data accumulated to date suggests that it is extremely rare for a gout patient to have only his or her main disease and no accompanying pathology. One of the frequent situations is a combination with another microcrystal arthritis, a disease of calcium pyrophosphate crystals deposition. In addition, diseases of the kidneys (including chronic renal failure), cardiovascular system, gastrointestinal tract, as well as metabolic disorders directly associated with gout or indirectly related to taking medications necessary to control the disease, not only affect the quality and longevity of life of the patient, but also create difficulties for its curation. The prescription of drug therapy, both symptomatic and pathogenetic, in such cases involves an assessment of all the associated risks, and the choice of drugs, in addition to efficiency, should be based on their safety profile in relation to comorbid pathology. This article analyzes the main principles and approaches to the treatment of gout and the disease of calcium pyrophosphate crystals deposition in the presence of concomitant diseases (arterial hypertension, chronic kidney disease, chronic heart failure, obesity, dyslipidemia, etc.) on the example of a 50-year-old patient. The possibilities of combined symptomatic therapy including colchicine, non-steroidal anti-inflammatory drugs and glucocorticoids are shown. The necessity and tactics of choice of preventive prophylactic therapy for arthritis attacks and use of phebuxostat in the presence of contraindications for prescription or ineffectiveness of allopurinol are also discussed.

Along with familial Mediterranean fever (FMF), it is now considered an autoinflammatory disease and gout. The commonality of the basic mechanisms of inflammation underlying the pathogenesis of FMF and gout predetermines the possibility of using similar therapies aimed at stopping and preventing seizures (colchicine and IL-1 inhibitors). A clinical case is presented describing the presence of a combination of FMF and gout in a patient. The patient was prescribed anakinra, which proved to be effective both as a treatment for FMF and gout. The appointment of an IL-1 inhibitor fully justified expectations: already after the first injection of anakinra, the intensity of swelling and pain in the joints decreased in the patient.

The reluctance of some doctors to follow current recommendations for the management of patients with gout is one of the factors for poor adherence of patients to therapy. The review considers modern approaches to the treatment of gout, which provide for long-term strategies for lowering of serum uric acid level.

Principles of urate-lowering therapy: eight steps to success

Objective: to compare the efficacy of methotrexate (MTX) and colchicine in patients with chronic arthritis in calcium pyrophosphate crystal deposition disease (CPPDD). Subjects and methods . Data from a controlled prospective cross-sectional study of 10 patients (8 women and 2 men) with chronic arthritis in CPPDD are presented. In the initial period of treatment, all the patients were given colchicine 1 mg/day for 3 months, followed by a wash-out period for 1 month and then subcutaneous MTX 20 mg/week for 3 months. The diagnosis of CPPDD was made if there were calcium pyrophosphate crystals in synovial fluid and signs of chondrocalcinosis, as evidenced by joint X-ray and/or ultrasonography. DAS44, the swollen joint count (SJC) and tender joint count (TJC), pain intensity on a visual analog scale (VAS), the Health Assessment Questionnaire (HAQ) index, and serum C-reactive protein (CRP) levels were determined in all the patients at baseline, 3 months after the beginning of treatment with colchicine, after a wash-out period, and 3 months after the beginning of MTX treatment. Results and discussion . At baseline, mean DAS44 value was 2.47±0.27; SJC and TJC were 2.0±0.6 and 2.4±1.1, respectively; pain intensity was 55.2±12.3 mm; serum CRP level – 3.89±3.82 mg/l; HAQ – 1.1±0.3. Three months after colchicine therapy initiation, mean DAS44 value decreased to 1.76±0.28 (p = 0.004), SJC – to 1.4±0.5 (p = 0.048), TJC – to 1.6±1.35 (p = 0.023), pain intensity – to 42.0±13.2 mm (p = 0.023), CRP level – to 3.13±2.85 mg/l (p = 0.75), HAQ – to 0.95±0.3 (p = 0.041). Good response was achieved in 7 patients after 3 months of colchicine therapy. After the wash-out period, the mean DAS44 value was 2.08±0.26; SJC and TJC – 1.6±0.5 and 1.7±1.4, respectively; pain intensity – 46.5±9.8 mm; CRP level – 3.38±1.74 mg/l; HAQ – 1.3±0.34. Following 3 months of MTX therapy, mean DAS44 value decreased to 1.39±0.45 (p = 0.027), SJC – to 0.7±0.5 (p = 0.023), TJC – to 0.6±0.5 (p = 0.007), pain intensity – to 26.0±18.97 mm (p = 0.045), CRP level – to 2.87±2.06 mg/l (p = 0.75), HAQ – to 0.8±0.6 (p = 0.045). Two of the 3 patients with an insufficient effect of colchicine achieved DAS44 remission after MTX treatment; two patients attained remission after therapy with colchicine and developed an exacerbation of the disease when this drug was replaced by MTX. Conclusion. MTX 20 mg/week is as effective as colchicine in most cases and can be the drug of choice in patients with chronic arthritis in CPPDD if colchicine therapy is ineffective.

Objective : to investigate the efficacy and safety of low-dose colchicine in patients with gout receiving urate-lowering therapy (ULT). Patients and methods . A single-center prospective study included 113 patients with gout. The main group consisted of 92 patients who were prescribed colchicine 0.5 mg/day combined with ULT (allopurinol or febuxostat), control group – 21 patients with contraindications to colchicine therapy who received only ULT. Patient data were entered into individual registration cards at the first visit, as well as on the 90th and 180th day after the start of drug intake. The presence of arthritis attacks was recorded by doctor or patient using a validated questionnaire. We compared the mean frequency of arthritis attacks in the groups, their duration and maximum pain intensity according to the visual analogue scale (VAS). Laboratory tests included: complete blood count test, general urinary test, uric acid (UA), blood glucose, alanine aminotransferase, aspartate aminotransferase, creatinine, serum creatine phosphokinase. Results and discussion . Patients who did not receive prophylactic anti-inflammatory therapy had significantly longer duration of the disease and higher number of affected joints. For 6 months of follow-up, there were no arthritis attacks in 54% of patients who were prescribed colchicine, and only in 19% of patients who did not use prophylactic anti-inflammatory therapy (p=0.004). The duration of arthritis attacks and the intensity of pain according to the VAS were also statistically significantly lower in the colchicine group (p<0.031 and p<0.01, respectively). Due to the development of adverse events, related to colchicine therapy, only 3 were excluded from the study. The mean serum UA level by the end of the study in colchicine group did not differ from that in the control group. Conclusion . Administration of colchicine, 0.5 mg/dai for 6 months after initiation of ULT is safe and can reduce the frequency and severity of arthritis attacks.

Colchicine is a drug that has been known for several millennia, since the days of Ancient Egypt, but has been successfully used to treat gout to this day. The action of colchicine is based on microtubule damage, mitosis suppression, as well as the ability to inhibit the activation of NLRP3 inflammasoma by monosodium urate crystals and block the release of interleu-kina (IL)-1p - key cytokine in the development of gout inflammation. However, the mechanism of action of colchicine is still not fully understood. Colchicine should be considered as the optimal drug not only for relieving an acute attack of arthritis in gout, but also as the best method for preventing attacks of gouty arthritis when choosing urate-lowering therapy. Recent studies have confirmed the good efficacy and safety profile of the drug when used correctly in patients with gout. Currently, it is recommended to use low doses of the drug (1-1.5 mg per day to relieve an acute attack of arthritis and 0.5-1.0 mg to prevent attacks when initiating uratelowering therapy). At the same time, according to the results of recent studies on the effectiveness, low doses are not inferior to high doses, but when low doses are used, the frequency of undesirable effects is much less. Unlike non-steroidal anti-inflammatory drugs and glucocorticoids, the drug does not have a negative effect on the cardiovascular system and can be used in patients with diabetes mellitus. This allows the drug to be used for a long time, which is especially important, since it is recommended to prevent arthritis attacks for at least 6 months from the start of taking urate-lowering drugs. This avoids the exacerbation of arthritis in most patients and significantly reduces their frequency. In addition, in the light of the accumulating data on the effect of the drug on the cardiovascular system of the drug, in conjunction with the high safety profile, other points of application should be considered where both anti-inflammatory and cardioprotective properties of colchicine can be used.
