Chromosomal Changes in Human Tumors and Leukemias

A family is described in which four out of five siblings have chromosomal aberrations both in the myeloid and lymphoid cells. One of these died in the blastic phase of chronic myelogenous leukemia, another had myelofibrosis and carcinoma of the breast, a third had squamous cell carcinoma of the lip, and the other two siblings are normal. The mother had carcinoma or the breast. Two of the siblings have had occupational exposure to organic solvents. The relationship of genetic pre-disposition to malignancy, chromosomal instability, and environmental stresses is discussed.

Bone marrow chromosomes were studied in 2 patients with polycythaemia vera (PV) at the time of diagnosis and several times subsequently during therapy with chlorambucil. Both patients were normal in the initial study, but later developed pathological diploid cell lines with structural chromosome abnormalities. Case 1 presented after 25 months of myelosuppressive therapy a deletion of chromosome no. 12, del (12) (p11), in 60% of the diploid cells. 10 months later all studied cells in the marrow contained this deletion. In addition, the deletion of chromosome no. 20, described as characteristic for PV, was found in 3 cells. Case 2 demonstrated after 40 months on chlorambucil the presence of a balanced translocation, t (1;5) (p36;q31) in 90% of the cells. In none of the patients were any definite clinical, laboratory or morphological signs of an approaching leukaemic transformation of the polycythaemic disorder present.

The effect of contents page format on the efficiency and ease of scanning by 54 medical scientists, was tested using 14 different format patterns. Significant differences were found between formats with respect to speed of scanning, rate of missed key words, and subjective appeal to the reader. These three criteria were not necessarily related, but some formats were better than others in all three. The results seem to indicate that when the material directly related to the immediate purpose of the scanner is separated from unrelated material, such as in a separate column, and when the unrelated material is more concentrated spatially, efficiency of scanning is increased. Study of newly devised formats may lead to an optimal one with considerable gain in total scanning time and rate of key words located, as well as reduction in fatigue.
