The incidence of mental defect, visual and hearing disability, major neurological handicap, and such minor neurological handicap as can be detected on examination at 2--9 years without formal intelligence testing, is presented among survivors of neonatal mechanical ventilation at Hammersmith Hospital between the years 1966--1973 inclusive. 77(21%) of 367 children survived, over three-quarters of them being born elsewhere. 3 died before the age of 6 months, 2 suddenly and unexpectedly at home, the third accidentally. 1 child was lost to follow up. 11 (15%) of the remaining 73 children had neurological sequelae as defined. In two-thirds this was moderate to severe. Spastic diplegia may no longer be the commonest form of cerebral palsy among those of low birthweight, particularly those surviving severe neonatal illness.

In 1962, Arshad Warley and Douglas Gairdner wrote 'Management of the condition must at present rest on (1) recognition of the physiological disturbances present in the individual case; and (2) correction of these disturbances, so far as this may be possible, in order that the baby can be given optimal conditions for survival during... a self-limited illness'.1 17 years later this statement still summarises perfectly the neonatologist's role in producing neurologically intact survivors from hyaline membrane disease (HMD). The great advances made in recent years in understanding the role and effect of surfacant deficiency in HMD show how both these maxims still apply in the management of the condition. The avoidance of acidaemia, hypothermia, prematurity, and overventilation of the lung, each of which diminishes lung surfacant, is of prime importance in the management of such babies today. The management ofHMD can be considered under three headings. (1) The control, and if possible the normalisation, of the blood-gases. (2) The control, and if possible the normalisation, of the biochemical and physiological abnormalities generally seen in any group of seriously ill patients in intensive carefor example, hypotension, hypothermia, electrolyte imbalances, oliguria, paralytic ileus. (3) The prevention if possible, but otherwise the control of the side effects both of the treatment and the disease-for example, patent ductus arteriosus (PDA) and bronchopulmonary dysplasia.

OBJECTIVE To evaluate initial arterial blood gas, pulmonary pressures, pulmonary mechanics (compliance and resistance), pulmonary volumes, oxygenation indices and serum carotenoid levels as predictors of fatality in mechanically ventilated neonates DESIGN Cross Sectional SETTING Referral neonatal unit of a teaching hospital SUBJECTS 83 mechanically ventilated outborn neonates METHODS 83 neonates consecutively put on mechanical ventilator from March to December 2001 were enrolled in the study The mechanical ventilator used was pressure limited time cycled ventilator with facility for online measurement of volumes and pulmonary mechanics Arterial blood gas after half an hour of initiation of mechanical ventilation and initial pulmonary pressures, pulmonary compliance, resistance and duration of mechanical ventilation were recorded in a pre structured proforma Initial serum carotenoid levels were also measured using spectrophotometric method The neonates were regularly followed up for outcome Multiple logistic regression analysis was done to find out the predictors of fatality for those variables that were significantly associated with outcome on univariate analysis RESULTS On univariate analysis weight ( 60%, oxygenation index >10, AaDO2 >250 and serum carotenoid levels 60%, gestational age <34 weeks and initial serum carotenoid levels < 100 microg/dL were independent predictors of fatality in neonatal mechanical ventilation Even in a setting with high fatality rates, high risk of mortality in mechanically ventilated neonates can be identified

A controlled study of mechanical ventilation has been performed in infants with respiratory distress syndrome. 168 infants in respiratory failure were ventilated and 53 similar infants were not. Artificial mechanical ventilation improved survival in infants weighing more than 2000 g. from 15% to 43 o (4/27 vs. 29/67, p < 0 025). Infants who weighed more than 1500 g. and developed respiratory failure at less than 38 hours of age had an improved survival (16/31) on ventilatory treatment, as compared with infants more than 1500 g. ventilated at more than 38 hours of age (24/78) (p < 0 05).
