A spinal plate system and method for fixation of the human spine is provided. In an embodiment, the system includes a bone plate, a bone screw and a ring. The bone screw preferably connects the bone plate to a bone, and the ring preferably fixes the bone screw into a borehole of the bone plate such that the bone screw extends from the bone plate at a selected angle. The ring is preferably capable of swiveling within the borehole to allow the bone screw to be angulated at a plurality of angles oblique to the plate. The bone screw may have a head having a tapered, threaded surface for engaging the ring. The ring preferably has threading on its inner surface for mating with the threading on the head. The inner surface of the ring may be tapered. Movement of the head through the ring preferably expands the ring against the bone plate to fix the bone screw at a selected angle relative to the bone plate.

System and method for stabilizing the human spine with a bone plate

Methods and apparatus for forming one or more trans-sacral axial instrumentation/fusion (TASIF) axial bore through vertebral bodies in general alignment with a visualized, anterior or posterior axial instrumentation/fusion line (AAIFL or PAIFL) in a minimally invasive, low trauma, manner and providing a therapy to the spine employing the axial bore. Anterior or posterior starting positions aligned with the AAIFL or PAIFL are accessed through respective anterior and posterior tracts. Curved or relatively straight anterior and curved posterior TASIF axial bores are formed from the anterior and posterior starting positions. The therapies performed through the TASIF axial bores include discoscopy, full and partial discectomy, vertebroplasty, balloon-assisted vertebroplasty, drug delivery, electrical stimulation and various forms of spinal disc cavity augmentation, spinal disc replacement, fusion of spinal motion segments and implantation of radioactive seeds. Axial spinal implants and bone growth materials can be placed into single or multiple parallel or diverging TASIF axial bores to fuse two or more vertebrae, or distract or shock absorb two or more vertebrae.

Methods and apparatus for performing therapeutic procedures in the spine

Method and apparatus for promoting a spinal fusion between neighboring vertebrae. Apparatus may be located within the intervertebral disc space and preferably includes a pair of engaging plates for contacting the vertebrae. An alignment device may be used to alter the vertical distance between the engaging plates to customize the apparatus to fit a given patient. In one embodiment, the alignment device includes a pair of struts having a predetermined height and extending between the engaging plates from an anterior end to a posterior end of the apparatus. In another embodiment, the alignment device includes a rotatable connector and cam pins for adjusting the distance between the engaging plates. The alignment device is preferably adapted to vary the distance between the engaging plates such that the height of the apparatus proximate the anterior end is greater than that proximate the posterior end whereby the natural lordosis of the spine is maintained after the apparatus is installed. The apparatus may further include a load-sharing member to allow stress to be imparted to bone in the vicinity of the apparatus to promote bone growth in accordance with Wolff's law.

Adjustable height fusion device

An apparatus (10) promoting a spinal fusion between neighboring vertebrae (16, 18) is disclosed. Apparatus (10) may be located within the intervertebral disc space and preferably includes a pair of engaging plates (12, 14) for contacting the vertebrae (16, 18). An alignment device may be used to alter the vertical height (20) between the engaging plates (12, 14) to customize the apparatus (10) to fit a given patient. In one embodiment, the alignment device includes two turnbuckles (250, 270) and two pairs of cam blocks (260, 261; 280, 281) for adjusting the height between the engaging plates (202, 204). In another embodiment, the alignment device includes three turnbuckles (540, 550, 560) and three pairs of cam blocks (544, 545; 554, 555; 564, 565) for adjusting the height between the engaging plates (502, 504). In another embodiment, the alignment device includes three screws (640, 650, 660) and three cam blocks (644, 654, 664) for adjusting the height between the engaging plates (602, 604). In another embodiment, the alignment device includes four screws and four cam blocks for adjusting the height between the engaging plates. The alignment device may be adapted to vary the height (20) between the engaging plates (12, 14) such that the height of the apparatus (10) proximate the anterior end (22) is greater than that proximate the posterior end (24), whereby the natural lordosis of the spine is maintained after the apparatus (10) is installed, and such that the height (20) of the apparatus (10) proximate the first side edge (26) is substantially different from the height of the apparatus (10) proximate the second side edge (28), whereby the natural lateral alignment of the spine is maintained after the apparatus (10) is installed. In an alternate embodiment, the apparatus includes a single alignment device that may be used to alter the vertical distance between the engaging plates. The alignment device may include a turnbuckle and a single pair of cam blocks for adjusting the height between the engaging plates (12, 14).

Multi-variable-height fusion device

A surgical cable system and method for securing surgical cable around a portion of a human element (e.g., bone) is described. The surgical cable system may include a connector and a tensioner. The connector may be adapted to hold a pin, positionable within the connector, such that the pin may secure the cable within the connector. The pin may be repositioned, after securing the cable, to allow the cable to move freely through the connector. The cable may be oriented within the connector such that the ends of the cable are perpendicular or parallel with respect to each other. The tensioner is preferably adapted to vary the tension of the cable. The cable may be passed through the connector, around a portion of a human bone, and back through the connector. The cable may be tensioned by use of the tensioner and secured into position within the connector.

Surgical cable system and method

Spinal fixation devices are used in the thoracic and lumbosacral spine to stabilize the spine, reduce deformities and fractures, and replace abnormal vertebrae. A bone fusion is usually attempted along with placement of the instrumentation because in most cases the hardware would eventually fail if it were used alone. The thoracolumbar spine is inherently unstable, and early operative intervention improves mobilization and rehabilitation. In some cases of lumbar spinal pain, surgical intervention is necessary for the treatment of conditions such as herniated disks, spondylolysis with spondylolisthesis, and degenerative disease with scoliosis. Surgical procedures consist of posterior (posterior elements) and anterior (vertebral body) fixation. Radiologists face continual changes in both surgical technique and instrumentation and should be knowledgeable about the devices available and the biomechanical principles that direct their use. They need to work with their surgical colleagues to become familiar with the techniques used at their institutions.

Spinal fixation devices can be used to form a rigid construct with the spine to replace bone, restore alignment, maintain position, and prevent motion in the treatment of fractures, degenerative disease, neoplasm, and congenital deformities. Because most spinal constructs will eventually fail if bone fusion does not occur, bone graft material is often used along with the implant to promote fusion. Conventional radiographs, obtained in two projections, remain the mainstay of implant evaluation, demonstrating the position of the spinal elements, hardware, graft material, and evidence of complication. Possible complications connected with use of fixation devices include intraoperative soft-tissue injuries, postoperative hematomas, and infection. The components (through incorrect use, malpositioning at surgery, and later dislodgment or fracture) may also contribute to complications such as instability; failure of fusion; or pain, with possible resultant neurologic damage.
