Establishing the basic knowledge, methodology, and technology for a framework for the continuous decoding of hand/arm movement intention was the aim of the ERC-funded project “Feel Your Reach”. In this work, we review the studies and methods we performed and implemented in the last 6 years, which build the basis for enabling severely paralyzed people to non-invasively control a robotic arm in real-time from electroencephalogram (EEG). In detail, we investigated goal-directed movement detection, decoding of executed and attempted movement trajectories, grasping correlates, error processing, and kinesthetic feedback. Although we have tested some of our approaches already with the target populations, we still need to transfer the “Feel Your Reach” framework to people with cervical spinal cord injury and evaluate the decoders’ performance while participants attempt to perform upper-limb movements. While on the one hand, we made major progress towards this ambitious goal, we also critically discuss current limitations.

Feel Your Reach: An EEG-Based Framework to Continuously Detect Goal-Directed Movements and Error Processing to Gate Kinesthetic Feedback Informed Artificial Arm Control

Transcatheter aortic valve implantation (TAVI) performed using femoral arterial access is now a guideline recommended treatment for severe calcific aortic stenosis (AS) in elderly patients. Technological advancements and procedural refinements have focused on making TAVI simpler, safer, more effective and durable. Myval (Meril Lifesciences) is a new generation balloon-expandable transcatheter heart valve (THV) developed in India that possesses novel features to improve deliverability and aid precise deployment. Following the first-in-human study, Myval was approved in India for commercial implantation in October 2018 and was subsequently given a CE mark in April 2019. This article reviews the science, technology and up-to-date clinical evidence for the Myval THV.

Myval: A Novel Transcatheter Heart Valve for the Treatment of Severe Aortic Stenosis

<title>Abstract</title> <bold>Objective:</bold> To explore the clinical and pathological characteristics of localized giant cell tumor of tendon sheath (L-GCTS) and fibroma of tendon sheath (FTS), and to compare and analyze them. <bold>Methods:</bold> 241 cases of L-GCTS and 41 cases of FTS who underwent surgical resection and diagnosis at Wuxi Ninth People's Hospital from 2019 to 2023 were collected. Clinical and pathological data of all cases were organized for retrospective analysis. Chi-square test was used to analyze differences between groups. <bold>Results:</bold> Comparing our two groups of cases, we found that L-GCTS is more common, while FTS is less common. However, the two were very similar in clinical features, disease course, age of onset, tumor size and overall appearance, as well as imaging manifestations; In our case, L-GCTS was more common in female patients, and there was no significant gender difference in FTS; L-GCTS was more common in the right hand, FTS was also more common in the hand, but the left incidence rate is slightly higher than the right, with no significant difference; In addition, a small number of L-GCTS were accompanied by damage to surrounding tissues, and 41 cases of FTS did not cause damage to surrounding tissues; L-GCTS and FTS are mostly single lesions, with a few being multiple lesions; Among 241 cases of L-GCTS, 235 were primary cases, 6 were recurrent cases, and 41 cases of FTS were all primary cases; The coincidence rate between preoperative imaging diagnosis and postoperative pathological diagnosis of L-GCTS was 89%, and the coincidence rate between preoperative imaging diagnosis and postoperative pathological diagnosis of FTS was 55.3%. The main reason was that it is easier to misdiagnose FTS as L-GCTS before surgery; There were certain differences in pathological and immunohistochemical manifestations between the two. <bold>Conclusion: </bold>L-GCTS and FTS are very similar in clinical features, disease course, predilection site, predilection age, tumor size, and tumor appearance. Imaging examination methods are difficult to distinguish between the two before surgery. The differentiation between the two mainly relies on histopathology and immunohistochemistry, and the two also exhibit different molecular genetics. Although both L-GCTS and FTS are benign tumors, their recurrence rates and other prognostic factors are different. Therefore, it is necessary to clearly distinguish between the two. 

Comparison of clinical and pathological characteristics between localized giant cell and fibroadenoma of tendon sheath

Musculoskeletal tumors and tumor-like lesions have an indeterminate appearance on magnetic resonance (MR) images and often display varying degrees and extent of T2 hyperintensity. However, a subset of neoplasms and tumor-like lesions may exhibit prominent areas of T2 hypointensity relative to skeletal muscle. The hypointensity observed on T2-weighted MRI can be caused by various substances, including hemoglobin degradation products, lesions containing calcium and calcification substances, mature collagen fibers, melanin, turbulent and rapid blood flow, and highly cellular lesions. Careful evaluation of the presence and pattern of T2 hypointensity in lesions and consideration of potential causes in their associated clinical contexts can help to narrow the differential diagnosis between neoplastic and nonneoplastic lesions. Appropriate interpretation of signal intensity and analyses of lesion location and clinical symptoms enable the correct choice of a further diagnostic algorithm or, in many cases, a final diagnosis determined exclusively by MRI examination. 

Musculoskeletal tumors and tumor-like lesions with “dark” signal intensity on T2-weighted MR images: A pictorial review

Objective: To evaluate the safety and efficacy of transcatheter aortic valve implantation (TAVI) with the novel Prizvalve® system in treating severe aortic stenosis. Methods: This is a single-center, prospective, single-arm, observational study. A total of 11 patients with severe aortic stenosis with high risk or inappropriate for conventional surgical aortic valve replacement (SAVR) were included, and TAVI was achieved with the Prizvalve® system between March 2021 and May 2021 in West China Hospital. Transthoracic echocardiography (TTE) was performed immediately after prosthesis implantation to evaluate mean transaortic gradient and maximal transaortic velocity. The device success rate was calculated, which was defined as (1) the device being delivered via the access, deployed, implanted and withdrawn, (2) mean transaortic gradient<20 mmHg (1 mmHg=0.133 kPa) or a maximal transaortic velocity<3 m/s post TAVI, and without severe aortic regurgitation or paravalvular leak post TAVI. TTE was performed at 30 days after the surgery, and all-cause mortality as well as the major cardiovascular adverse events (including acute myocardial infarction, disabling hemorrhagic or ischemic stroke) up to 30 days post TAVI were analyzed. Results: The age of 11 included patients were (78.1±6.3) years, with 8 males. A total of 10 patients were with NYHA functional class Ⅲ or Ⅳ. Devices were delivered via the access, deployed, implanted and withdrawn successfully in all patients. Post-implant mean transaortic gradient was (7.55±4.08) mmHg and maximal transaortic velocity was (1.78±0.44) m/s, and both decreased significantly as compared to baseline levels (both P<0.05). No severe aortic regurgitation or paravalvular leak was observed post TAVI. Device success was achieved in all the 11 patients. No patient died or experienced major cardiovascular adverse events up to 30 days post TAVI. Mean transaortic gradient was (9.45±5.07) mmHg and maximal transaortic velocity was (2.05±0.42) m/s at 30 days post TAVI, which were similar as the values measured immediately post TAVI (both P>0.05). Conclusions: TAVI with the Prizvalve® system is a feasible and relatively safe procedure for patients with severe aortic stenosis and at high risk or inappropriate for SAVR. Further clinical studies could be launched to obtain more clinical experience with Prizvalve® system.

[Transcatheter treatment of heart valve diseases in China: rapid progress and huge task ahead].

Objective: To investigate the clinicopathological and cytogenetic features of cryptic COL1A1-PDGFB fusion dermatofibrosarcoma protuberans (CC-DFSP). Methods: Three cases of CC-DFSP diagnosed in West China Hospital, Sichuan University, Chengdu, China from January 2021 to September 2021 were studied. Immunohistochemistry for CD34 and other markers, fluorescence in situ hybridization (FISH) for PDGFB, COL1A1-PDGFB and COL1A1, next-generation sequencing (NGS), reverse-transcriptase polymerase chain reaction (RT-PCR) and Sanger sequencing were performed. Results: There were three cases of CC-DFSP, including two females and one male. The patients were 29, 44 and 32 years old, respectively. The sites were abdominal wall, caruncle and scapula. Microscopically, they were poorly circumscribed. The spindle cells of the tumors infiltrated into the whole dermis or subcutaneous tissues, typically arranging in a storiform pattern. Immunohistochemically, the neoplastic cells exhibited diffuse CD34 expression, but were negative for S-100, SMA, and Myogenin. Loss of H3K27me3 was not observed in the tumor cells. The Ki-67 index was 10%-15%. The 3 cases were all negative for PDGFB rearrangement and COL1A1-PDGFB fusion, whereas showing unbalanced rearrangement for COL1A1. Case 1 showed a COL1A1 (exon 31)-PDGFB (exon 2) fusion using NGS, which was further validated through RT-PCR and Sanger sequencing. All patients underwent extended surgical resection. Except for case 3 with recurrence 2 years after surgical resection, the other 2 cases showed no recurrence or metastasis during the follow-up. Conclusions: FISH has shown its validity for detecting PDGFB rearrangement and COL1A1-PDGFB fusion and widely applied in clinical detection. However, for cases with negative routine FISH screening that were highly suspicious for DFSPs, supplementary NGS or at least COL1A1 break-apart FISH screening could be helpful to identify cryptic COL1A1-PDGFB fusions or other variant fusions.
