Die optimale Therapie der Erstluxation hat groste Bedeutung um eine chronische Instabilitat mit rezidivierenden Luxationen zu vermeiden, die Alltags- aber auch Sportfunktion wiederherzustellen und schlussendlich die Entstehung einer Instabilitatsarthropathie zu verhindern. Die konsequente klinische und bildgebende Erstabklarung muss als Ziel haben, relevante strukturell-morphologische Verletzungen auszuschliesen und konstitutionelle Risikofaktoren fur wiederholte Luxationen zu erfassen. Eine prazise Diagnostik im Hinblick auf knocherne Defekte am ventralen Glenoidrand und posterosuperioren Humeruskopf steht beim jugendlichen Patienten im Zentrum. Die chirurgische Stabilisierung nach traumatischer Erstluxation zeigt bei diesem Patientenkollektiv bezuglich Reluxationshaufigkeit bessere Resultate als die konservative Therapie. In der zweiten Lebenshalfte fokussiert die Abklarung von Erstluxationen zusatzlich auf Verletzungen der Rotatorenmanschette. Langzeitstudien zeigen, dass selbst einmalige Schulterluxationen mit einem erhohten Risiko fur Langzeitschaden im betroffenen Gelenk einhergehen.
