Objectives: Using urinary 1-hydroxypyrene (1-OHP) as a measure of total absorbed dose, the primary objective of this study was to evaluate the total effect of inhalation and dermal PAH exposures while considering other factors such as age, body mass index and smoking that may also have a significant effect on urinary 1-OHP. Methods: The study population included two groups of highway construction workers: 20 paving workers and 6 milling workers. During multiple consecutive workshifts, personal air and dermal samples were collected from each worker and analyzed for pyrene. During the same work week, urine samples were collected pre-shift, post-shift and at bedtime each day and analyzed for 1-OHP. Distributed lag models were used to evaluate the independent effect of inhalation and dermal exposures that occurred at each of several preceding exposure periods and were used to identify the relevant period of influence for each pathway. Results: The paving workers had inhalation (mean 0.3 mg/m 3 ) and dermal (5.7 ng/cm 2 ) exposures to pyrene that were significantly higher than the milling workers. At pre-shift on Monday morning, following a weekend away from work, the pavers and millers had the same mean baseline urinary 1-OHP level of 0.4 mg/g creatinine. The mean urinary 1-OHP levels among pavers increased significantly from pre-shift to post-shift during each work day, while the mean urinary 1-OHP levels among millers varied little and remained near the baseline level throughout the study period. Among pavers there was a clear increase in the pre-shift data duringthe work week, such that theaverage pre-shift level onday 4(1.4mg/gcreatinine) was3.5 times higher than the average pre-shift results on day 1 (0.4 mg/g creatinine). The results of the distributed lag model indicated that the impact of dermal exposure was approximately eight times the impact of inhalation exposure. Furthermore, dermal exposure that occurred during the preceding 32 h had a statistically significant effect on urinary 1-OHP, while the effect of inhalation exposure was not significant. Conclusions: We found that distributed lag models are a valuable tool for analyzing longitudinal biomarker data and our results indicate that dermal contact is the primary route of exposure to PAHs among asphalt paving workers. An exposure assessment of PAHs that does not consider dermal exposure may considerably underestimate cumulative exposure and control strategies aimed at reducing occupational exposure to asphalt-related PAHs should include an effort to reduce dermal exposure.

Urinary 1-Hydroxypyrene and Polycyclic Aromatic Hydrocarbon Exposure Among Asphalt Paving Workers

BACKGROUND: An increased risk of lung cancers among asphalt workers has been suggested in epidemiological studies based on large scale statistical analyses. METHODS: In a multi-country study of 29,820 male workers employed in road paving, asphalt mixing and roofing, 32,245 ground and building construction workers and 17,757 other workers from Denmark, Finland, France, Germany, Israel, the Netherlands, Norway, and Sweden, with mortality that was documented from 1953-2000. Exposures to bitumen fume, coal tar, 4-6 ring polycyclic aromatic hydrocarbons, organic vapor, diesel exhaust, asbestos, and silica dust were assessed via a job-exposure matrix. Standardized mortality ratios (SMRs) and 95% confidence intervals (CIs) based on national mortality rates, as well as relative risks (RRs) based on Poisson regression models were calculated. RESULTS: The SMR of lung cancer among workers exposed to bitumen fume (1.08, 95% CI 0.99-1.18) was comparable to that of non-exposed workers (SMR 1.05, 95% CI 0.92-1.19). In a sub-cohort of bitumen-exposed workers without exposure to coal tar, the SMR of lung cancer was 1.23 (95% CI 1.02-1.48). The analysis based on the semi-quantitative, matrix-based exposures in the whole cohort did not suggest an increased lung cancer risk following exposure to bitumen fume. However, in an analysis restricted to road pavers, based on quantitative estimate of bitumen fume exposure, a dose-response was suggested for average level of exposure, applying a 15-year lag, which was marginally reduced after adjustment for co-exposure to coal tar. The results for cancer of the head and neck were similar to those of lung cancer, although they were based on a smaller number of deaths. There was no clear suggestion of an association with bitumen fume for any other neoplasm. CONCLUSIONS: The results of the analysis by bitumen fume exposure do not allow us to conclude on the presence or absence of a causal link between exposure to bitumen fume and risk of cancer of the lung and the head and neck.

Methods

We conducted a study of 29,820 male workers exposed to bitumen in road paving, asphalt mixing and roofing, 32,245 ground and building construction workers unexposed to bitumen, and 17,757 workers not classifiable as bitumen workers, from Denmark, Finland, France, Germany, Israel, the Netherlands, Norway, and Sweden, with mortality follow-up during 1953–2000. We calculated standardized mortality ratios (SMRs) and 95% confidence intervals (CIs) based on national mortality rates. Poisson regression analyses compared mortality of bitumen workers to that of building or ground construction workers.



Results

The overall mortality was below expectation in the total cohort (SMR 0.92, 95% CI 0.90–0.94) and in each group of workers. The SMR of lung cancer was higher among bitumen workers (1.17, 95% CI 1.04–1.30) than among workers in ground and building construction (SMR 1.01, 95% CI 0.89–1.15). In the internal comparison, the relative risk (RR) of lung cancer mortality among bitumen workers was 1.09 (95% CI 0.89–1.34). The results of cancer of the head and neck were similar to those of lung cancer, based on a smaller number of deaths. There was no suggestion of an association between employment in bitumen jobs and other cancers.



Conclusions

European workers employed in road paving, asphalt mixing and other jobs entailing exposure to bitumen fume might have experienced a small increase in lung cancer mortality risk, compared to workers in ground and building construction. However, exposure assessment was limited and confounding from exposure to carcinogens in other industries, tobacco smoking, and other lifestyle factors cannot be ruled out. Am. J. Ind. Bitumen fumes: review of work on the potential risk to workers and the present knowledge on its origin

Workers in the road construction industry include asphalt plant, ground construction, and road paving workers. These individuals can be exposed to a wide range of potentially hazardous substances. A summary of levels of exposure to different substances measured during road construction is presented. In modern road paving, workers typically are exposed to 0.1 to 2 mg/m3 of bitumen fume, which includes 10 to 200 ng/m3 of benzo(a)pyrene. Sampling strategies and analytical methods employed in each reviewed survey are described briefly. The published reports provide some insight into the identity of factors that influence exposure to bitumen among road construction workers: type of work performed, meteorological conditions, temperature of paved asphalt. Literature Review of Levels and Determinants of Exposure to Potential Carcinogens and Other Agents in the Road Construction Industry

Objectives The feasibility of a European epidemiologic study of cancer risk among asphalt workers was examined in Western Europe. The study was motivated by occupational and public health concern about possible health risk from exposure to bitumen fumes. Methods Information on the accessibility and quality of epidemiologic resources, retrospective worker records, mortality and cancer incidence records, and exposures was requested from research institutes and road paving and asphalt mixing companies in 15 European countries. Results Research institutes and asphalt companies in 12 countries responded. It was found that at least 44 companies in seven countries can be included in a retrospective mortality study of a minimum of 32 000 employees with 356 000 person-years (over 100 lung cancers). Coal tar will be an important confounder for these data. The power of a cohort study of workers who have never worked with tar-containing materials remains insufficient. Even in an ambispective study extending to the year 2005, the expected lung cancer deaths in a tar-free cohort would be only four. Conclusions The results suggest that a case-referent study of lung cancer, nested in an international cohort of asphalt workers, represents the design of choice, conditionally on the possibility of assessing relevant individual life-time exposures. A cross-sectional determination of relevant biomarkers of exposure such as adducts in lymphocytes or the presence of metabolites of polycyclic aromatic compounds in urine in a group of workers exposed to bitumen fumes will provide further relevant information.
