Effects of Continuous Positive Airway Pressure Treatment and Withdrawal in Patients With Obstructive Sleep Apnea on Arterial Stiffness and Central BP

Snoring (SN) is an early sign of upper airway dysfunction, and it is strongly associated with obstructive sleep apnea. SN detection is important to monitor SN objectively and to improve the diagnostic sensitivity of sleep-disordered breathing. In this study, an automatic snore detection method using an electromechanical film transducer (Emfit) signal is presented. Representative polysomnographs of normal breathing and SN periods from 30 subjects were selected. Individual SN events were identified using source separation applying nonnegative matrix factorization deconvolution. The algorithm was evaluated using manual annotation of the polysomnographic recordings. According to our results, the sensitivity, and the positive predictive value of the developed method to reveal snoring from the Emfit signal were 82.81% and 86.29%, respectively. Compared to other approaches, our method adapts to the individual spectral snoring profile of the subject rather than matching a particular spectral profile, estimates the snoring intensity, and obtains the specific spectral profile of the snores in the epoch. Additionally, no training is necessary. This study suggests that it is possible to detect individual SN events with Emfit mattress, which can be used as a contactless alternative to more conventional methods such as piezo-snore sensors or microphones.

Background: Obstructive sleep apnea (OSA) is defined by recurrent apnea and hypopnea during sleep. The main treatment of OSA is continuous positive airway pressure (CPAP). Adherence to CPAP is challenging and depends on multiple factors. This study was designed to evaluate the compliance with CPAP in patients with OSA. Materials and Methods: This was a prospective observational study including 106 patients with confirmed OSA by a standard polysomnography. We recorded CPAP usage hours after 7 and 90 days by a smart card. We compared the adherence of the patients with respect to body mass index (BMI), gender, smoking status, living area, and education level. Drug-Induced Sleep Endoscopy Performed in the Endoscopy Suite: A Resource Utilization Analysis

Background This study aimed to examine the association between self-reported sleep disordered breathing (SDB) ("awaken short of breath or with a headache") and mortality in a large and ethnically diverse group of community-dwelling elderly people. Methods A total of 1288 participants, 65 years and older, were examined longitudinally. Sleep problems were estimated using the Medical Outcomes Study Sleep Scale examining sleep disturbance, snoring, awaken short of breath or with a headache, sleep adequacy, and sleep somnolence. Cox regression analysis was used to examine the association between sleep problems and mortality. Age, gender, education, ethnicity, and body mass index were included as covariates. In further analyses we included hypertension, diabetes, heart disease, and stroke as additional covariates. Results The participants were followed for up to 6 years (mean = 2.9, standard deviation = 1.1), and 239 (18.6%) participants died during the follow-up. In unadjusted models, SDB at the initial visit was associated with mortality (hazard ratio [HR] = 1.37; 95% confidence interval [CI] 1.21-1.55; P P Conclusions Our results suggest that SDB is a risk factor for mortality in a large and ethnically diverse group of older adults, independent of demographic and clinical factors. Further research is needed to examine the underlying mechanisms of this association.
