Pain disrupts sleep, and sleep deprivation or interference can alter pain perception in animals and humans, for example by increasing sensitivity to pain. However, the mechanism by which sleep affects neuropathic pain remains unclear. In this review, we discuss the available evidence from the epidemiologic, clinical, and human, as well as laboratory studies. In previous studies, we have found that sleep deprivation affects various injurious systems, including opioids, dopaminergic, immune, orexins, hypothalamic-pituitary-adrenal axis, and adenosine. At the same time, these systems play a crucial role in neuropathic pain regulation. In the complex interactions between these neurobiological systems, there may be potential regulatory pathways through which sleep deprivation amplifies neuropathic pain. Because of the impact sleep problems and neuropathic pain can have on the patients' quality of life, studying the link between sleep and neuropathic pain is important for neuropathic pain prevention and public health. 

The Underlying Mechanisms of Sleep Deprivation Exacerbating Neuropathic Pain

This study aims to investigate the temporal effects of two Kinesio Taping (KT) techniques on lateral gastrocnemius muscle activity, motor neuron excitability, and countermovement jump height in university athletes from hockey, football, basketball, and volleyball. Additionally, it investigates whether the athletes' playing positions-either attacker or defender-influence these outcomes following the KT application.Forty-eight subjects were randomly assigned to one of three groups: Group A (n = 16), Group B (n = 16), and Group C (n = 16). All groups were further subdivided into attackers and defenders. Adhesive Kinesio tape was applied to the lateral gastrocnemius using the Y-shaped technique for three days. Facilitatory KT was applied from the origin to the insertion of the lateral gastrocnemius, while inhibitory KT was applied from the insertion to the origin. Motor neuron excitability, electromyographic activity, and countermovement jump height were tested at baseline, as well as after KT application, to evaluate if the dependent variables had changed. One-way ANOVA was used for baseline comparison, and mixed ANOVA was applied to assess post-interventional effects on the outcome measures.Significant group effects for lateral gastrocnemius activation were found, measured using percentage of maximum voluntary isometric contraction (% MVIC) average root mean square (RMS). In motor neuron excitability, maximal M-wave (Mmax) was significantly improved in group comparison. Further, there was also a significant increase in countermovement jump height. There was no significant difference in outcome measures based on playing position (attacker and defender).Both KT techniques effectively influenced the lateral gastrocnemius muscle's activity, motor neuron excitability, and jump height when compared with the control group. Additionally, there is no effect of playing position, specifically attacker or defender, on the examined variables following KT application.

Abstract Objective : The aim of this study was to investigate the relationship between radiographic severity and BMI, pain, and physical function in elderly women with knee osteoarthritis (OA). Methods: A total of 80 elderly women with knee OA were enrolled in this study. Their age, course of disease and body mass index (BMI) were recorded. Radiographic severity was assessed with the Kellgren-Lawrence (K/L) scale, and pain, stiffness, and physical function were assessed with Western Ontario and McMaster University Osteoarthritis Index (WOMAC) scales, objective assessment of patients' functional performance using the Time Up and Go (TUG) test. Spearman correlation analysis was used to assess the relationship between radiographic K/L scores and BMI, pain, and physical function. Logistic regression analysis was used to determine the important factors contributing to knee OA severity. Result ：The mean age of the patients was 64.7 ± 6.77 years, and the mean course of disease was 5.02±2.12 years. There was no significant correlation between radiographic severity K/L and normal BMI ( p =0.087), but there was a moderate correlation between radiographic severity K/L and high BMI ( p =0.02, r=0.457). It was positively correlated with WOMAC pain, stiffness and physical function ( p =0.001, r=0.447; p =0.004, r=0.316; p =0.025, r=0.250). In addition, K/L grading was positively correlated with TUG ( p = 0.001, r=0.395). On the other hand, BMI, pain, and stiffness may be the important influencing factors of the severity of knee OA, while age, course of disease, and physical function have little influence on the severity of knee OA. Conclusion: The radiographic severity of knee OA was correlated with high BMI, pain, and physical function, and high BMI, knee pain and stiffness were important factors for the severity of knee OA.

OBJECTIVE
The objective of this study is to delineate disparities between patients with knee osteoarthritis (KOA) based on obesity status, investigate the interplay among body composition, physical activity and knee pain/function in patients with KOA and conduct subgroup analyses focusing on those with KOA and obesity.


DESIGN
Cross-sectional study.


SETTING
Residents of eight communities in Shijiazhuang, Hebei Province, China, were surveyed from March 2021 to November 2021.


PARTICIPANTS
178 patients with symptomatic KOA aged 40 years or older were included.


MAIN OUTCOMES AND MEASURES
The primary outcome measure was knee pain, assessed using the Western Ontario and McMaster Universities Osteoarthritis Index-pain (WOMAC-P) scale. Secondary outcome measures included function, evaluated through the WOMAC-function (WOMAC-F) scale and the Five-Time-Sit-to-Stand Test (FTSST). Data analysis involved t-tests, Wilcoxon rank-sum tests, χ2 tests, linear and logistical regression analysis.


RESULTS
Participants (n=178) were 41-80 years of age (median: 65, P25-P75: 58-70), and 82% were female. Obese patients (n=103) had worse knee pain and self-reported function (p<0.05). In general patients with KOA, body fat mass was positively associated with bilateral knee pain (β=1.21 (95% CI 0.03 to 0.15)), WOMAC-P scores (β=0.25 (95% CI 0.23 to 1.22)), WOMAC-F scores (β=0.28 (95% CI 0.35 to 1.29)) and FTSST (β=0.19 (95% CI 0.03 to 0.42)), moderate-intensity to low-intensity physical activity was negatively associated with bilateral knee pain (β=-0.80 (95% CI -0.10 to -0.01)) and Skeletal Muscle Index (SMI) was negatively associated with WOMAC-F scores (β=-0.16 (95% CI -0.66 to -0.03)). In patients with KOA and obesity, SMI was negatively associated with FTSST (β=-0.30 (95% CI -3.94 to -0.00)).


CONCLUSION
Patients with KOA and obesity had worse knee pain and self-reported function compared with non-obese patients. Greater fat mass, lower muscle mass and lower moderate-intensity to low-intensity physical activity were associated with increased knee pain and poor self-reported function. More skeletal muscle mass was associated with the improvement of objective function.

Abstract Previous studies have reported adverse effects of short and long sleep duration on cardiovascular health. However, how sleep time and sleep efficiency affect angina have not been studied in hypertensive individuals. This study aimed to assess the relationship of sleep with angina. Using a cross-sectional design, data from 1563 hypertensive individuals were collected from the parent Sleep Heart Health Study (SHHS). Age, alcohol use, average diastolic blood pressure (ADBP), average systolic blood pressure (ASBP), cigarette use, sleep time, sleep efficiency, percent time in stage N3 of sleep, and body mass index (BMI) were used as covariates. Multiple linear regression, the Chi-Square test, and Pearson’s correlation coefficient were used for data analysis. Unadjusted sleep efficiency, sleep time, ADBP, and age were significant (p &lt; 0.05) predictors of the number of angina episodes (Angina n ). When the covariates were adjusted, only ADBP and ASBP were significant (p &lt; 0.05) predictors of Angina n . Sleep efficiency, BMI, ADBP, sleep time, and age had a significant (p &lt; 0.05) correlation with Angina n . In hypertensive individuals, sleep time and sleep efficiency did not affect Angina n when adjusted for covariates. ADBP and ASBP were found to be significant predictors of Angina n when the covariates were adjusted.
