Efficient with low-cost removal and adsorption mechanisms of norfloxacin, ciprofloxacin and ofloxacin on modified thermal kaolin: experimental and theoretical studies.

It is estimated that the growth of the population, the augmented expectancy of life, and the emergence of new pandemics will significantly increase the consumption of pharmaceutical drugs in the coming years. Due to its high efficiency, the group of fluoroquinolones, where the antibiotic ofloxacin hydrochloride (OFL) is found, is widely used to combat bacterial infections in humans and animals. The big problem is concentrated in the effluents generated by industries and hospitals. Additionally, most of the drug is not absorbed by the body and is released directly into domestic effluents. On the other hand, treatment stations have removal limitations for small concentrations. This review analyzed all adsorbents developed and used in OFL removal, listing the main parameters influencing the process. In the end, the other existing technologies in the literature and the gaps and future prospects were described. OFL adsorption in most studies occurs under basic conditions (pH between 6.5 and 8). The increase in concentration provides an increase in adsorption capacity. The adsorbents analyzed showed moderate kinetics, reaching equilibrium before 250 min for most studies. The pseudo-second-order model showed the best statistical fit. In most of the studies, the increase in temperature (313, 315, and 328 K) favored the adsorption of OFL. The Langmuir monolayer model represented most of the isothermal studies. The adsorption capacity varied from 3702 to 0.3986 mg g−1. In this aspect, factors such as OFL concentration and textural characteristics of the adsorbent exerted great influence. The thermodynamic parameters were compatible with the isothermal data, where the endothermic nature of the studies was confirmed. Physical interactions (π-π stacking, H bonding, hydrophobic and electrostatic interactions) governed the main adsorption mechanism. Although some studies stated that chemosorption occurred, thermodynamic parameters cannot validate the same. Coexisting ions in the solution can positively and negatively influence OFL adsorption. The listed studies are all applied to batch processes, where fixed bed studies should be better explored. From this review, it can be concluded that adsorption is a promising technique for OFL removal. However, it is extremely necessary to break the laboratory scale barrier and analyze possible conditions for applying these materials in treating real effluents together with combining technologies.

Associations of statin use with the onset and progression of open-angle glaucoma: A systematic review and meta-analysis

In the current issue of JAMA Ophthalmology, Sandhu et al1 report a retrospectivecohort studyofmedical claimsdata inorder todeterminetheassociationbetweenfluoroquinolonesand uveitis.Theauthorsshouldbe commended on their excellent analyses and interpretationof the results.Their study didnot findanassociationbetween fluoroquinolonesanduveitis. Their conclusions are both in keeping and in contrastwith the findings of recent similar studies.2,3 The differing conclusionsderivedfromthesestudieshighlightsometopics thatneed tobeconsideredwheninterpretingtheresultsofmedicalclaims database research: the importanceofusingproper controls, accounting for confoundingbias, replicationandvalidationof results, and the inherent limitations of this type of research. Uveitis is a heterogeneousdisease, and canbe causedby a variety of infectious andnoninfectious (inflammatory) etiologies. Patients canalsodevelopnoninfectiousuveitis following anunrelatedsystemic infection (eg,poststreptococcaluveitis). Furthermore,patientswithearly signsof a systemic inflammatory disease that is associated with uveitis may present with nonspecific systemic symptoms that canbemisdiagnosedas a systemic infectious process. Thus, in any study that attempts to examine the association between fluoroquinolones and uveitis, thepotential confoundingbiasof this systemicprocess needs tobeproperlyaccountedfor.Forooghianetal3previously published a large, retrospective case-control study examining theassociationbetweenfluoroquinolonesanduveitis.Thestudy usedβ-lactamandmacrolideantibioticsascontrolsandexcluded patients with 9 different systemic diseases known to be associatedwithuveitis. The investigators found that all 3 classes of antibiotics (fluoroquinolones,β-lactams,andmacrolides)were associatedwithuveitis. Theyconcluded that the systemicprocesses forwhich fluoroquinolones are prescribed, and not the fluoroquinolones themselves, are the likely inciting factors for uveitis.Followingthepublicationoftheseresults,anothergroup2 publisheda report in JAMAOphthalmology (usingadatabaseof older men) claiming an association between the fluoroquinolonesmoxifloxacin and ciprofloxacin and uveitis. The control used by Eadie et al in this report was finasteride, and they adjusted for only 3different systemicdiseases known tobe associated with uveitis. It is unclear why they did not use other antibiotics as controls or adjust for amore comprehensive list of systemic inflammatory diseases. The current report by Sandhu et al1 appropriately addresses the issue of potential confounding bias in their analyses. They used β-lactams as a control group and excluded patients with 18 different systemic diseases that are known to beassociatedwithuveitis. Furthermore, theyperformedasecondary analysis using incident diagnosis of a systemic disease known to be associated with uveitis as an outcome measure. When they examined fluoroquinolones as a group, Sandhu and colleagues1 found no increased hazard of uveitis within 30-, 60-, or 90-day treatment periods. Controversy Surrounding the Proposed Ocular Adverse Events of Fluoroquinolones.

OBJECTIVE To evaluate the risk of developing glaucoma in patients taking systemic fluoroquinolones. METHODS A case-control study was carried out among a cohort of subjects who had visited an ophthalmologist in the Province of British Columbia, Canada from 2000 to 2007. Cases were identified as those newly diagnosed with glaucoma (ICD-9 360). For each case, 5 controls were selected and matched to the cases by age and calendar time. Crude and adjusted rate ratios (RRs) for current, recent, past, and distant use of fluoroquinolones were calculated. RESULTS From the cohort of 989,591 subjects, 178,264 subjects were diagnosed with glaucoma and 891,320 were corresponding controls. The 2 groups had same average age of 65 and comparable systemic comorbidities including hypertension, coronary artery disease, and diabetes. There was no statistically significant association between the use of systemic fluoroquinolones and the development of glaucoma for current use [RR=1.01 (95% confidence interval (CI), 0.95-1.07)], recent use [RR=1.00 (95% CI, 0.92-1.08)], or past use [RR=0.94 (95% CI, 0.90-1.00)]. Distant use of systemic fluoroquinolones had a small statistically significant increased risk of developing glaucoma [RR=1.12 (95% CI, 1.09-1.14)]. CONCLUSIONS There was no detected increased association of the development of glaucoma with current, recent, or past use of systemic fluoroquinolone but a minimal statistically significant increased risk was associated with distant use. Future studies should further examine a potential delayed response with fluoroquinolones and glaucoma.
