BACKGROUND
Red blood cell distribution width (RDW), a component of an electronic complete blood count, is a measure of heterogeneity in the size of circulating erythrocytes. In patients with symptomatic cardiovascular disease (CVD), RDW is associated with mortality. However, it has not been demonstrated that RDW is a predictor of mortality independent of nutritional deficiencies or in the general population.


METHODS
Red blood cell distribution width was measured in a national sample of 8175 community-dwelling adults 45 years or older who participated in the 1988-1994 National Health and Nutrition Examination Survey; mortality follow-up occurred through December 31, 2000. Deaths from all causes, CVD, cancer, and other causes were examined as a function of RDW.


RESULTS
Higher RDW values were strongly associated with an increased risk of death. Compared with the lowest quintile of RDW, the following were adjusted hazard ratios (HRs) for all-cause mortality (and 95% confidence intervals [CIs]): second quintile, HR, 1.1 (95% CI, 0.9-1.3); third quintile, HR, 1.2 (95% CI, 1.0-1.4); fourth quintile, HR, 1.4 (95% CI, 1.2-1.8); and fifth quintile, HR, 2.1 (95% CI, 1.7-2.6). For every 1% increment in RDW, all-cause mortality risk increased by 22% (HR, 1.22; 95% CI, 1.15-1.30; P < .001). Even when analyses were restricted to nonanemic participants or to those in the reference range of RDW (11%-15%) without iron, folate, or vitamin B(12) deficiency, RDW remained strongly associated with mortality. The prognostic effect of RDW was observed in both middle-aged and older adults for multiple causes of death.


CONCLUSION
Red blood cell distribution width is a widely available test that is a strong predictor of mortality in the general population of adults 45 years or older.

To develop and validate a noncontrast computed tomography (NCCT)–based clinical-radiomics nomogram to identify spontaneous intracerebral hemorrhage (sICH) patients with a poor 90-day prognosis on admission. In this double-center retrospective study, data from 435 patients with sICH (training cohort: n = 244; internal validation cohort: n = 104; external validation cohort: n = 87) were reviewed. The radiomics score (Rad-score) was calculated based on the coefficients of the selected radiomics features. A clinical-radiomics nomogram was developed by using independent predictors of poor outcome at 90 days through multivariate logistic regression analysis in the training cohort and was validated in the internal and external cohorts. At 90 days, 200 of 435 (46.0%) patients had a poor prognosis. The clinical-radiomics nomogram was developed by six independent predictors namely midline shift, NCCT time from sICH onset, Glasgow Coma Scale score, serum glucose, uric acid, and Rad-score. In identifying patients with poor prognosis, the clinical-radiomics nomogram showed an area under the receiver operating characteristic curve (AUC) of 0.81 in the training cohort, an AUC of 0.78 in the internal validation cohort, and an AUC of 0.73 in the external validation cohort. The calibration curve revealed that the clinical-radiomics nomogram showed satisfactory calibration in the training and internal validation cohorts (both p > 0.05), but slightly poor agreement in the external validation cohort (p < 0.05). The clinical-radiomics nomogram is a valid computer-aided tool that may provide personalized risk assessment of 90-day functional outcome for sICH patients. • The proposed Rad-score was significantly associated with 90-day poor functional outcome in patients with sICH.

Findings In this survey study including 14 767 individuals with post–COVID-19 condition surveyed in late 2022 to early 2023, 57% reported experiencing cognitive symptoms daily, compared with 27% with prior SARS-CoV-2 infection who did not develop post–COVID-19 condition. In those with post–COVID-19 condition, cognitive symptoms were associated with greater levels of depressive symptoms, greater reported functional impairment, and lesser likelihood of full-time employment. Meaning The findings of this study suggest that self-reported cognitive symptoms are prevalent in post–COVID-19 condition, often co-occur with depressive symptoms, and are associated with functional impairment.

Red cell distribution width (RDW) has been associated with mortality and outcome in a wide variety of non-neurological and neurological diseases, namely in myocardial infarction and acute ischemic stroke, and the reason for this is not completely understood. We aimed to investigate RDW as a potential prognostic marker in patients with intracerebral hemorrhage (ICH). This is a retrospective study of consecutive patients with acute non-traumatic ICH admitted to a single center during a 4-year period. We reviewed individual clinical records to collect demographic and baseline information, including RDW at admission, 3-month functional status, and incidence of death during follow-up. Baseline computed tomography imaging was reviewed to classify the location of ICH, and to measure ICH volume and perihematomal edema volume. Patients were divided according to quartile distribution of RDW (RDW-Q1-4). The final study population consisted of 358 patients, median age 71 years (interquartile range [IQR] 60–80), 55% were male, and median Glasgow Coma Scale was 14 (IQR 10–15), with a mean follow-up of 17.6 months. Patients with higher RDW values were older (p = 0.003), more frequently presented with an active malignancy (p = 0.005), atrial fibrillation (p < 0.001), intraventricular hemorrhage (p = 0.048), and were anticoagulated (p < 0.001). Three-month functional independence was similar throughout RDW quartiles. RDW-Q4 was independently associated with increased 30-day mortality (adjusted odds ratio = 3.36, 95%CI = 1.48–7.62, p = 0.004), but not independently associated with increased mortality after 30 days (adjusted hazards ratio = 0.71, 95%CI = 0.29–1.73, p = 0.448). RDW is a robust and independent predictor of 30-day mortality in non-traumatic ICH patients, and further studies to understand this association are warranted.

O presente projeto tem como objetivo determinar a relacao entre a imagem corporal (atraves da percecao que os adolescentes tem da sua aparencia fisica) e o efetivo indice de massa corporal (IMC), coadjuvado com o perimetro abdominal. A amostra sera constituida por cerca de 300 criancas e adolescentes de ambos os sexos e com idades compreendidas entre os 13 e os 18 anos de idade. A avaliacao da percecao da imagem corporal sera realizada atraves de uma escala de silhuetas, que consiste no calculo da insatisfacao com a imagem corporal, tendo por base a diferenca entre a imagem real e a ideal. Para estimar a prevalencia de obesidade sera calculado o IMC (atraves da medicao do peso e altura) e medido o perimetro abdominal. Este projeto permitira perceber a qualidade do peso corporal dos adolescentes, analisar e avaliar a discrepância entre a satisfacao corporal e o efetivo IMC. Este estudo torna-se relevante para a motivacao, no âmbito das aulas de Educacao Fisica, no que respeita ao empenhamento motor, proporcionando nao apenas o seu desenvolvimento motor, como tambem social, afetivo e cognitivo.

The Spectrum of Post-COVID Neurological Syndrome: A Prospective Cohort Study

Behçet's disease is a relapsing multisystemic inflammatory syndrome characterized by recurrent oral and/or genital ulcers, uveitis, arthritis, skin lesions, and gastrointestinal and neurological involvement. Neuro-Behçet corresponds to nervous system involvement and is one of the most severe complications of Behçet disease. It occurs in 3% to 30% of cases and is categorized into parenchymal (most common) or non-parenchymal disease. The most common manifestation of parenchymal neuro-Behçet is meningoencephalitis with involvement of the brainstem, where patients present with cranial neuropathies, encephalopathy, sensory-motor syndromes, epilepsy, or myelitis. The main non-parenchymal manifestation is cerebral venous thrombosis. Neuro-Behçet has a predominantly subacute course, with remission within weeks, or clinical progression in one third of the cases. The diagnosis is essentially clinical and diagnostic tests help to corroborate the suspicion, distinguish from differential diagnoses, and exclude complications. Brain magnetic resonance imaging allows the identification of acute lesions (hypointense or isointense on T2-weighted and hypointense on T1-weighted sequences) contrast-enhanced, and chronic lesions characterized by non-contrast enhanced small lesions and brainstem atrophy. If non-parenchymal involvement is suspected, cerebral veno-magnetic resonance imaging /computed tomography should be performed. Cerebrospinal fluid shows elevated proteinorachia and pleocytosis in parenchymal and no changes in non-parenchymal neuro-Behçet (except increased opening pressure). Outbursts of parenchymal disease should be treated with high dose intravenous corticosteroid therapy, with subsequent switch to oral corticoids, followed by biologic therapy, usually an anti-TNF. The treatment of cerebral venous thrombosis is controversial and may consist of a combination of corticosteroids and anticoagulation.
