Background. Diabetes mellitus is a metabolic disorder of multiple etiologic factors characterized by chronic hyperglycemia with disturbance of carbohydrate metabolism. It can play the vital role in the cause of morbidity and mortality through continued clinical consequence. Therefore, good knowledge, attitude, and practices of glycemic control are necessary in promoting care, in enhancing better therapeutic outcomes, and in the prevention and management of diabetes complications. Methods. A cross-sectional study design was conducted to determine knowledge, attitude, and practice towards glycemic control and its associated factors. Diabetic patients who were attending the University of Gondar Hospital from March to May 2018 were included in the study. The data was collected using questionnaires, and individuals that can fulfill our inclusion criteria were selected by a simple random sampling technique. SPSS version 20 was used for descriptive and logistic regression analysis, and finally, the variables were summarized and presented using tables and graphs. Results. Of the total 403 participants, 216 (53.6%) were males and 176 (43.7%) were illiterate. Of the total, 250 (62%) had good knowledge, 271 (67.2%) had a good attitude, and 300 (74.4%) had good practice towards glycemic control. In multivariate logistic regression, occupational status and marital status were significantly associated with the knowledge of participants towards glycemic control. Occupational status, educational status, and marital status were significantly associated with attitude and practice towards glycemic control. Conclusion. More than half of the participants had good knowledge, attitude, and practice towards glycemic control. Occupational status and marital status were significantly associated with knowledge, attitude, and practice towards glycemic control.

Knowledge, Attitude, and Practice towards Glycemic Control and Its Associated Factors among Diabetes Mellitus Patients.

Instruments Six questionnaires were used to collect the necessary information about factors that might affect the glycemic control. The first questionnaire sought information about socio-demographic and relevant characteristics including age, gender, and onset and duration of type 2 diabetes mellitus. Information related to patient’s self-care behaviors and other variables that may influence an individual’s ability and willingness to provide diabetes self-care were collected using different questionnaires including: Diabetes Knowledge Test questionnaire [14], Attitude Toward Diabetes Scale (Positive Attitude Scale and Negative Attitude Scale) [15], Family and Friends’ Support Scale [15], Long-Term Care Benefits Scale [15], and Self-Care Adherence Scale of the Diabetes Care Profile (DCP) [15]. In this study a symmetrical back-translation method was used to translate all study questionnaires. Higher scores on the Diabetes Knowledge Test questionnaire indicate higher levels of patient’s knowledge about diabetes and its care. A higher score on the Positive Attitude Scale indicate that the patient would have positive coping in dealing with type 2 diabetes mellitus on a daily basis. High scores on the Negative Attitude Scale indicate perception of more maladaptation of the patient toward the diabetes and its treatment. Higher scores on the Family and Friends’ Support Scale indicate the patients’ perception of more family and friends’ support about diabetes and its management. A higher score on the Long- Term Care Benefits Scale indicates that the patient’s health beliefs were directed toward a positive outlook on recommended health regimens of type 2 diabetes mellitus in preventing or delay. A higher score of Self – Care Adherence Scale indicates the patients were compliant with recommended health regimens most of the time (see the appendix for some of the questions on the questionnaires). Glycemic Control and Its Determinants among Patients with type 2 Diabetes Mellitus Attending a Teaching Hospital

Background: The rising burden of type 2 diabetes mellitus (T2DM) with its attendant's complication can be successively steamed in the face of appropriate self-care management. The latter is positively imparted by the level of knowledge of the disease itself, its impact on quality of life and available basic technique of its control. Aims: The study is, therefore, aimed to assess the level of glycemic control and its determinants among type 2 subjects attending a secondary hospital in Niger Delta. Subjects and Methods: Two hundred consenting adult type 2 diabetes patients of age more than 40 years and attended diabetes outpatient clinics at the Central Hospital Warri between March and August 2014 were used for this cross-sectional study. Two different questionnaires were administered to all the participants to collect the necessary information on diabetes knowledge as well as factors that might affect their glycemic control. Blood samples were collected for fasting blood glucose (FBG) and glycosylated hemoglobin (HbA1c) for all the respondents. Weight and height were also measured to the nearest 0.5 kilogram and centimeter using standardized equipment. Body mass index was then calculated as the ratio of weight in kilogram and height in meters square (kg/m 2 ). Statistical Analysis Used: Statistical Package for Social Science Version 16 was used to compute the data generated. Results: The mean age and diabetic duration of all participants were 54.8 ± 11.9 years and 8.5 ± 3.2 years, respectively. The overall mean knowledge score of the subjects was 6.90 ± 1.8 (69.0 ± 18.2%) The mean FBG level and HbA1c of respondents were 7.89 ± 3.6 mmol/L (range 4-20 mmol/L) and 8.2%, respectively, with 55% of the population having poor glycemic control and 45% good glycemic control. The diabetic knowledge scoring of those with poor glycemic control was significantly lower than those with good glycemic control. In addition, diabetics' with poor glycemic control HbA1c >7.0 had longer diabetic duration (1-19 years; 8.06 ± 4.30) when compared with those with good glycemic control, HbA1c

Tumoral calcinosis of the foot: An unusual differential diagnosis of calcaneal mass

Eur J Pediatr Surg 2010; 20: 421–423 Introduction ▼ Tumoral calcinosis (TC) is a rare disease characterized by unior multifocal calcifi cation of soft tissues [1] . The lesions commonly develop around large joints, in particular the hip joints, shoulders and elbows, but the hand and wrist may also be aff ected. A single or multiple palpable tumoral masses increasing in size represent the typical clinical presentation. Restricted joint mobility and pain herald disease progression [2] . Histologically, the lesions are composed of tumor-forming lobules of amorphous calcium salt deposits within a fi brous stroma. A mixed infl ammatory infi ltrate rich in macrophages and occasional giant cells of the foreign-body type are commonly present at the borders of the calcifi cations [3] . The onset of disease is in the fi rst and second decade of life in the majority of cases ( ≈ 80 % ). Although relatively rare, presentation in patients older than 50 years is well documented [2] . According to a pathogenesis-based classifi cation of TC proposed by Smack et al. [4] , three types of TC are recognized: 1) primary normophosphatemic TC; 2) primary hyperphosphatemic TC; and 3) secondary TC. Secondary TC is associated with systemic diseases known to predispose to soft tissue calcifi cations, in particular renal insuffi ciency, hyperparathyroidism, hypervitaminosis D, vitamin D defi ciency and collagen vascular disease [4 – 6] . Recent investigations showed loss-offunction mutations in GALNT3 as a cause of primary hyperphosphatemic familial tumoral calcinosis [7] . A history of antecedent trauma was documented in some cases and possibly contributes to the development of tumoral calcinosis in predisposed individuals [8] .

Cardiovascular disease remains the leading cause of mortality in patients with diabetes. Control of multiple cardiovascular risk factors in patients with type 2 diabetes mellitus often leads to substantial reduction in adverse cardiovascular events. Newer antihyperglycemic agents at several randomized controlled trials have demonstrated their cardiovascular safety along with reduction in cardiovascular outcomes in type 2 diabetes mellitus with or without heart failure. Sodium-glucose cotransporter 2 inhibitors and dipeptidyl peptidase-4 inhibitors reported to have positive CV outcomes; especially, combination therapy with dapagliflozin and sitagliptin seem to be suitable therapeutic option. However, data upon the fixed-dose combination of these agents is limited. This review discusses the rationale and clinical utility of combination therapy with dapagliflozin and sitagliptin in improving glycemic control and reducing cardiovascular events in patients with type 2 diabetes mellitus with multiple cardiovascular risk factors.

A 50-year-old woman presented with recurrent calcified mass in the left gluteal region. The clinical, radiological, and biochemical profile confirmed the diagnosis of tumoral calcinosis. She also had associated vitamin D deficiency. The patient underwent surgical removal of the mass to relieve the sciatic nerve compression and was managed with acetazolamide, calcium carbonate, and aluminium hydroxide gel with which she showed significant improvement. The management implications and effect of vitamin D deficiency on phosphate metabolism in the setting of tumoral calcinosis is discussed.

Background: Tight glycemic control in the critically ill is known to reduce both the morbidity and the mortality. It is essential that intensivists and endocrinologists involved in the care of these patients have a good understanding of the concepts related to this condition. Objectives: To assess the knowledge, attitudes and practices about achieving tight glycemic control in the critically ill among the endocrinologists and intensivists practicing in the city of Chennai. Materials and Methods: Questionnaires containing ten questions pertaining to clinical outcomes, drawbacks, target levels of glycemic control and insulin regimen in achieving tight glycemia in the critically ill were sent to a total of six endocrinologists and 52 intensivists practicing in Chennai. Results: All those who were administered the questionnaires responded. Majority of the responders (88%) believed in tight glycemic control in the critically ill because of better outcomes from hospitalization. A minority did not for fear of hypoglycemia. Fifty percent agreed on the cut off value of 110 mg/dL as followed in the Van den Berghe study. Seventy percent used glucometer for monitoring sugar levels. Most preferred using regular insulin as infusion. Conclusions: There seems to be a good understanding and standard practices among the endocrinologists and intensivists in achieving strict glycemic control in the critically ill. Setting up of standard intensive care unit glycemic control protocols will settle all the methodological differences and make the practices more uniform.

Background: Adrenal insufficiency is a common occurrence in the critically ill and it is essential that intensivists and endocrinologists involved in the care of these patients have a good understanding of the concepts related to this condition. Objectives: To assess the knowledge, attitudes and practices about adrenal insufficiency in the critically ill among the endocrinologists and intensivists practicing in the city of Chennai. Materials and Methods: Questionnaires containing ten questions pertaining to adrenal insufficiency in the critically ill were sent to a total of six endocrinologists and 52 intensivists practicing in Chennai. Results: About 77% of all the respondents agreed to the fact that adrenal insufficiency is a frequent occurrence in critical illness. But 57% of them felt that there is no need for routine evaluation of critically ill patients for adrenal insufficiency. Random serum cortisol was selected by 62% of the responders as the method for evaluating adrenal function in the critically ill. There is clearly no agreement among the endocrinologists or the intensivists on the various cut off levels for diagnosis. Neither is there a clear consensus on the method followed for treatment of patients with adrenal insufficiency in the critical care unit. Conclusion: There is no concordance in the knowledge, attitudes or practices on adrenal insufficiency in the critically ill among the endocrinologists and intensivists in Chennai. There is a need for developing standard diagnostic and treatment guidelines and making it available for all the practicing endocrinologists and intensivists.
