Background: Case reports have indicated that psychologic treatments may have a beneficial effect on psoriasis activity. Objective: Our purpose was to further investigate the hypothesis that psychologic intervention has a beneficial effect on psoriasis activity in a blinded, controlled manner. Methods: Fifty-one patients with psoriasis vulgaris were randomly assigned to a treatment or a control group. Patients in the treatment group participated in seven individual psychotherapy sessions in 12 weeks. Intervention techniques included stress management, guided imagery, and relaxation. The Psoriasis Area Severity Index (PASI), Total Sign Score (TSS), and Laser Doppler Skin Blood Flow (LDBF) of a selected reference plaque was measured in a blinded fashion at baseline (week 0), week 4, week 8, and after treatment (week 12). Results: Slight, but significant, changes in TSS and LDBF were found in the treatment group but not in the control group. When analyses were performed for both groups separately, the treatment group displayed significant reductions for all three psoriasis activity measures, whereas no changes were seen in the control group. Conclusion: Our preliminary results suggest that psychologic intervention may have a moderate beneficial effect on psoriasis activity.

Psoriasis and psychiatry: an update.

Psychological and psychophysiological aspects of psoriasis.

The client was a 31-yr.-old white male evidencing multiple psoriatic plaques. The condition was considered severe and had been resistant to previous dermatological treatments. Following a baseline period of 3 wk., 30 treatment sessions were conducted over a 7-mo. period. Each treatment session consisted of 20 min. of skin temperature training at the target plaque site and 15 min. of supportive psychotherapy focused on personal adjustment problems. Photographs of the plaque area and skin temperature recordings were taken during each session. Ratings of the photographs indicated marked improvements of dermatological signs. Despite efforts to reduce skin temperature, small but consistent increases were obtained. The present study was designed to determine the effect of skin-temperature-biofeedback training on cellular proliferation in three psoriasis patients. It was hypothesized that (a) psoriasis patients would be able to consciously decrease skin temperature of psoriatic tissue, and (b) there would be a positive correlation between rate of cellular proliferation and temperature change.
Results obtained indicated biofeedback training to be effective in decreasing the surface temperature of psoriatic tissue. A 2 X 7 analysis of variance for two repeated measures indicated the change in skin temperatures as a function of sample period to be significant, F (6,26) = 3.29, p < .02. Generalization of temperature-training effects from the biofeedback to the no-feedback condition were observed. Rate of proliferation decreased from pretraining to posttraining biopsies.

Biofeedback and Control of Skin Cell Proliferation in Psoriasis

Evidence linking the development of chronic alcoholism with endogenous opioid peptides is reviewed. Particular emphasis is placed on enkephalin metabolism with respect to its involvement in the development of addiction and stress-related psychophysiological changes. The study was concerned with enkephalin hydrolysing activity (EHA) in chronic alcoholism as well as the mood changes that reportedly accompany alcoholism. Also of interest was the relationship of enkephalin degradation to voluntary relaxation.
