We describe the clinicopathologic features of a small low-grade invasive mammary carcinoma with cytomorphological columnar cell features arising in a complex fibroadenoma that in addition to sclerosing adenosis, apocrine metaplasia, and usual ductal hyperplasia also displayed columnar cell change with flat epithelial atypia and low-grade ductal carcinoma in situ merging with the invasive carcinoma. There were strong cytomorphological similarities between the invasive carcinoma and the low-grade ductal carcinoma in situ, which also showed significant overlap in the immunohistochemical findings.

Zachorowania na nowotwory złośliwe piersi w wieku poniżej 25 lat są rzadkością, ale mimo sporadycznego występowania stanowią poważny problem, ponieważ charakteryzują się większą biologiczną agresywnością i obarczone są poważ nym rokowaniem. Problematyka do tycząca schorzeń piersi u młodzieży i młodych dorosłych jest obecnie mało znana, a postępowanie diagnostyczno-terapeutyczne niejednokrotnie nastrę cza wiele trudności ze względu na brak jednolitych standardów. Autorzy przedstawiają aktualne poglądy na temat diagnostyki zmian w obrębie piersi u pacjentów w wieku rozwojowym i propozycję algorytmu postępowania diagnostycznego.

Diagnostic and therapeutic procedures in adolescents and young adults with changes of the breast

: Mucinous carcinoma arising in a background of fibroadenoma is rare. We report a case of a 34 year old woman who presented in our hospital with a mobile mass in the right breast. Physical examination, breast ultrasound and fine needle aspiration cytology (FNAC) were all concordant for diagnosis of fibroadenoma. However microscopic examination of excisional biopsy revealed mucinous carcinoma within a background of fibroadenoma. The patient underwent right mastectomy with dissection of the right axillary lymph nodes. Nodal metastasis was not seen. She was also treated with chemotherapy and radiotherapy and has shown no sign of recurrence for more than one year since after surgery. This report highlights one of the pitfalls of FNAC, particularly in the diagnosis of mixed lesions of the breast.

Invasive Mucinous Carcinoma Arising in a Background of Breast Fibroadenoma

Fibroadenomas are the most common benign tumors of the breast. Carcinomas occuring within fibroadenoma are very rare. Fibroadenomas present in approximately 10% of women at autopsy, also occurs 20% of all the benign breast mass and usually identified as circumscribed masses at physical examination or mammography. The most common histologic type is lobular carcinoma in situ, followed by ductal carcinoma in situ, invasive ductal carcinoma and invasive lobular carcinoma. Positive family history, complex fibroadenomas, proliferative disease adjacent fibroadenomas and BRCA mutation are the risk factors for malign transformation within fibroadenomas. Histopathological examination should be performed routinely to rule out malignancy because of no definitive clinical or radiological criteria for diagnosing carcinoma occuring within fibroadenoma. We presented a case of invasive breast carcinoma occuring within fibroadenoma.

Invasive Breast Carcinoma Occuring Within Fibroadenoma: Case Report

Malignant neoplasms arising in the epithelial component of breast fibroadenomas are rare. The most frequent types are lobular and ductal intra-epithelial carcinomas, with a minority of infiltrating carcinoma. We report a case of 36-year-old patient with invasive mucinous carcinoma (30 x 30 mm) arising in a complex breast fibroadenoma (130 x 60 x 30 mm). The patient underwent mastectomy with dissection of the axillary lymph nodes, which were free of tumour. The patient is alive without disease five years later. To the best of our knowledge, this is the first report of an invasive mucinous carcinoma arising within breast fibroadenoma. Our case provides information about the clinicopathologic characteristic of this unusual tumour.
