Complex intervention trials should be able to answer both pragmatic and explanatory questions in order to test the theories motivating the intervention and help understand the underlying nature of the clinical problem being tested. Key to this is the estimation of direct effects of treatment and indirect effects acting through intermediate variables which are measured post-randomisation. Using psychological treatment trials as an example of complex interventions, we review statistical methods which crucially evaluate both direct and indirect effects in the presence of hidden confounding between mediator and outcome. We review the historical literature on mediation and moderation of treatment effects. We introduce two methods from within the existing causal inference literature, principal stratification and structural mean models, and demonstrate how these can be applied in a mediation context before discussing approaches and assumptions necessary for attaining identifiability of key parameters of the basic causal model. Assuming that there is modification by baseline covariates of the effect of treatment (i.e. randomisation) on the mediator (i.e. covariate by treatment interactions), but no direct effect on the outcome of these treatment by covariate interactions leads to the use of instrumental variable methods. We describe how moderation can occur through post-randomisation variables, and extend the principal stratification approach to multiple group methods with explanatory models nested within the principal strata. We illustrate the new methodology with motivating examples of randomised trials from the mental health literature.

Mediation and moderation of treatment effects in randomised controlled trials of complex interventions

We propose a framework for considering the role of pharmacokinetic/pharmacodynamic modeling in drug development and an appraisal of its current and potential impact on that activity. After some introduction, definitions, and background information on drug development, we discuss subject-matter models that underlie pharmacokinetic/pharmacodynamic modeling and show how they determine appropriate statistical models. We discuss the broad role modeling can play in drug development, enhancing primarily the "learning" steps, i.e. acquiring the information needed for the label and for planning efficient confirmatory clinical trials. Examples of past applications of modeling to drug development are presented in tabular form, followed by a discussion of some practical issues in application. Modeling will not reach its potential utility until it is manifest as a visible and separate work unit within a drug development program. We suggest that that work unit is the "in numero" study: a protocol-driven exercise designed to extract additional information, and/or answer a specific drug-development question, through an integrated model-based (meta-) analysis of existent raw data, often pooled across separate (clinical) studies.

Pharmacokinetic/Pharmacodynamic Modeling in Drug Development

Most classification approaches aim at achieving high prediction accuracy on a given dataset. However, in most practical cases, some action such as mailing an offer or treating a patient is to be taken on the classified objects, and we should model not the class probabilities themselves, but instead, the change in class probabilities caused by the action. The action should then be performed on those objects for which it will be most profitable. This problem is known as uplift modeling, differential response analysis, or true lift modeling, but has received very little attention in machine learning literature. An important modification of the problem involves several possible actions, when for each object, the model must also decide which action should be used in order to maximize profit. In this paper, we present tree-based classifiers designed for uplift modeling in both single and multiple treatment cases. To this end, we design new splitting criteria and pruning methods. The experiments confirm the usefulness of the proposed approaches and show significant improvement over previous uplift modeling techniques.

Summary. We estimate cause–effect relationships in empirical research where exposures are not completely controlled, as in observational studies or with patient non-compliance and self-selected treatment switches in randomized clinical trials. Additive and multiplicative structural mean models have proved useful for this but suffer from the classical limitations of linear and log-linear models when accommodating binary data. We propose the generalized structural mean model to overcome these limitations. This is a semiparametric two-stage model which extends the structural mean model to handle non-linear average exposure effects. The first-stage structural model describes the causal effect of received exposure by contrasting the means of observed and potential exposure-free outcomes in exposed subsets of the population. For identification of the structural parameters, a second stage ‘nuisance’ model is introduced. This takes the form of a classical association model for expected outcomes given observed exposure. Under the model, we derive estimating equations which yield consistent, asymptotically normal and efficient estimators of the structural effects. We examine their robustness to model misspecification and construct robust estimators in the absence of any exposure effect. The double-logistic structural mean model is developed in more detail to estimate the effect of observed exposure on the success of treatment in a randomized controlled blood pressure reduction trial with self-selected non-compliance.

Causal inference with generalized structural mean models

In database marketing, data mining has been used extensively to find the optimal customer targets so as to maximize return on investment. In particular, using marketing campaign data, models are typically developed to identify characteristics of customers who are most likely to respond. While these models are helpful in identifying the likely responders, they may be targeting customers who have decided to take the desirable action or not regardless of whether they receive the campaign contact (e.g. mail, call). Based on many years of business experience, we identify the appropriate business objective and its associated mathematical objective function. We point out that the current approach is not directly designed to solve the appropriate business objective. We then propose a new methodology to identify the customers whose decisions will be positively influenced by campaigns. The proposed methodology is easy to implement and can be used in conjunction with most commonly used supervised learning algorithms. An example using simulated data is used to illustrate the proposed methodology. This paper may provide the database marketing industry with a simple but significant methodological improvement and open a new area for further research and development.

The true lift model: a novel data mining approach to response modeling in database marketing

SUMMARY The causal effect of a treatment is estimated at different levels of treatment compliance, in a placebocontrolled trial on the reduction of blood pressure. The structural nested mean model makes no direct assumptions on selected treatment compliance levels and placebo prognosis but relies on the randomization assumption and a parametric form for causal effects. It can be seen as a regression model for unpaired data, where pre- and post-randomization covariables are treated differently. The causal parameters are found as solutions to estimating equations involving estimated placebo response and treatment compliance based on base-line covariates for all subjects. Our example considers a linear effect of the percentage of prescribed dose taken on achieved diastolic blood pressure reduction. We propose an exploration of structural model checks. In the example, this reveals an interaction between the causal effect of active dose taken and the base-line body weight of the patient.
