Since the 2000s, sporadic outbreaks of whooping cough have been reported in advanced countries, where the acellular pertussis vaccination rate is relatively high, and in developing countries. Small-scale whooping cough has also continued in many countries, due in part to the waning of immune protection after childhood vaccination, necessitating the development of an improved pertussis vaccine and vaccination program. Currently, two different production platforms are being actively pursued in Korea; one is based on the aP (acellular pertussis) vaccine purified from B. pertussis containing pertussis toxoid (PT), filamentous hemagglutin (FHA) and pertactin (PRN), and the other is based on the recombinant aP (raP), containing genetically detoxified pertussis toxin ADP-ribosyltransferase subunit 1 (PtxS1), FHA, and PRN domain, expressed and purified from recombinant E. coli. aP components were further combined with diphtheria and tetanus vaccine components as a prototype DTaP vaccine by GC Pharma (GC DTaP vaccine). We evaluated and compared the immunogenicity and the protective efficacy of aP and raP vaccines in an experimental murine challenge model: humoral immunity in serum, IgA secretion in nasal lavage, bacterial clearance after challenge, PTx (pertussis toxin) CHO cell neutralization titer, cytokine secretion in spleen single cell, and tissue resident memory CD4+ T cell (CD4+ TRM cell) in lung tissues. In humoral immunogenicity, GC DTaP vaccines showed high titers for PT and PRN and showed similar patterns in nasal lavage and IL-5 cytokine secretions. The GC DTaP vaccine and the control vaccine showed equivalent results in bacterial clearance after challenge, PTx CHO cell neutralization assay, and CD4+ TRM cell. In contrast, the recombinant raP vaccine exhibited strong antibody responses for FHA and PRN, albeit with low antibody level of PT and low titer in PTx CHO neutralization assay, as compared to control and GC DTaP vaccines. The raP vaccine provided a sterile lung bacterial clearance comparable to a commercial control vaccine after the experimental challenge in murine model. Moreover, raP exhibited a strong cytokine response and CD4+ TRM cell in lung tissue, comparable or superior to the experimental and commercial DTaP vaccinated groups. Contingent on improving the biophysical stability and humoral response to PT, the raP vaccine warrants further development as an effective alternative to aP vaccines for the control of a pertussis outbreak.

Comparative Evaluation of Recombinant and Acellular Pertussis Vaccines in a Murine Model

Despite considerable progress in the understanding of clinical pertussis, the contemporary emergence of antimicrobial resistance for Bordetella pertussis and an evolution of concerns with acellular component vaccination have both sparked a renewed interest. Although simian models of infection best correlate with the observed attributes of human infection, several animal models have been used for decades and have positively contributed in many ways to the related science. Nevertheless, there is yet the lack of a reliable small animal model system that mimics the combination of infection genesis, variable upper and lower respiratory infection, systemic effects, infection resolution, and vaccine responses. This narrative review examines the history and attributes of non-primate animal models for pertussis and places context with the current use and needs. Emerging from the latter is the necessity for further such study to better create the optimal model of infection and vaccination with use of current molecular tools and a broader range of animal systems. KEY POINTS: • Currently used and past non-primate animal models of B. pertussis infection often have unique and focused applications. • A non-primate animal model that consistently mimics human pertussis for the majority of key infection characteristics is lacking. • There remains ample opportunity for an improved non-primate animal model of pertussis with the use of current molecular biology tools and with further exploration of species not previously considered.

Non-primate animal models for pertussis: back to the drawing board?

There is currently an increasing interest in the development of new-generation purified antigen-based vaccines with a higher safety profile compared to conventional inactivated vaccines. The main problem of subunit vaccines is their lower immunogenicity compared to whole-cell vaccines and inducing weaker and shorter-lasting immune responses. In this paper, the results of the assay of the potency of the tetanus component combined with the diphtheria component and whole-cell pertussis vaccine (DTwP), diphtheria and tetanus vaccine (DT), and in monovalent tetanus vaccine (T) are presented. In the mice model, an adjuvant impact of the whole-cell pertussis component on the immune response against tetanus was observed. It was noticed that the potency of tetanus component in the DTwP vaccine was significantly higher than tetanus potency in DT and T vaccines, despite the same bounding ability unit of the tetanus toxoid in the vaccine formulations. The levels of induction of tetanus antibodies by the tested vaccines were also examined. There were no differences in the induction of humoral responses against tetanus by tested vaccines. This publication discusses the possible mechanisms of impact of the whole-cell pertussis component on the other vaccine antigens and the positive and negative aspects of using the whole-cell pertussis component as an adjuvant.

Adjuvant Effect of Whole-Cell Pertussis Component on Tetanus Toxoid Potency in Murine Model

Bordetella pertussis population changes under whole-cell and acellular pertussis vaccines-induced immunity selection pressure in Poland

La necesidad de nuevas vacunas para la enfermedad respiratoria denominada tos convulsa, pertussis o coqueluche está bien establecida debido a la situación epidemiológica de resurgencia que se ha comenzado a detectar a partir de los años noventa luego de muchos años de haberla controlado gracias a la aplicación masiva de las vacunas comerciales actualmente disponibles 1. Por ello, en esta tesis se profundiza en la caracterización de la respuesta inmune desencadenada por un nuevo candidato vacunal contra pertussis. Esta enfermedad altamente contagiosa que afecta a todos los grupos etarios ha resurgido con un aumento preocupante de casos aún en países con altas coberturas de vacunación 2–6. Si bien pertussis afecta a personas de cualquier edad, los más sensibles a la enfermedad, con riesgo de hospitalización y muerte, son los niños y niñas menores de 6 meses de edad no vacunados o vacunados con esquemas incompletos para su edad. Esta situación de preocupación sanitaria provocó la revisión de las estrategias de prevención y control de esta enfermedad 1. Así, las medidas que se implementaron y se están implementando refieren al mejor empleo de las vacunas hoy existentes contra la enfermedad de forma de reducir en el corto plazo los casos severos y muertes en los más pequeños 7–10. Se incorporaron refuerzos a los esquemas de vacunación en los grupos poblacionales que presentan alta incidencia y que son principales fuentes de contagio para los lactantes, como los adolescentes y adultos, personal de salud, miembros familiares en contacto de un bebé y/o gestantes. En simultáneo a la aplicación de estas medidas sanitarias, la comunidad científica concuerda en la necesidad de mejoras y desarrollo de nuevas vacunas que superen las debilidades de las actuales vacunas en uso. Las vacunas comerciales basadas en el agente causal (la bacteria Bordetella pertussis inactivada, vacunas celulares o wP) presentan alta reactogenicidad, y desencadenan un perfil de respuesta inmunológico mixto Th1/Th2/Th17 mientras que las constituidas por inmunógenos proteicos altamente purificados derivados de B. pertussis (vacunas de componentes o acelulares aP), si bien son menos reactogénicas, confieren una inmunidad de corta duración con un perfil predominante del tipo Th2 11. Afortunadamente en la actualidad se avanzó en la consolidación de varios candidatos vacunales 11. Uno de ellos, que se encuentra en fase clínica en humanos, corresponde a una formulación a base de bacteria viva atenuada de administración intranasal, BPZE1, que induce una respuesta inmunitaria robusta tanto a nivel mucosal como sistémica 12–21. Otro candidato es el desarrollado a partir de B. pertussis enteras inactivadas en la que se ha reducido la cantidad de endotoxina con el objetivo de reducir la reactogenicidad, pero manteniendo la diversidad de inmunógenos y epitopes 22,23. En lo que refiere a los desarrollos de vacunas acelulares de nueva generación algunos investigadores están trabajando en formulaciones que contienen los mismos inmunógenos que las vacunas actuales pero modificados genéticamente de forma de inducir una respuesta inmunológica más robusta y con un perfil de respuesta Th1 24; otros agregando nuevos antígenos y/o adyuvantes a las formulaciones acelulares en uso 25–29. Una estrategia diferente para el desarrollo de una nueva vacuna acelular fue el abordado por nuestro grupo de trabajo al diseñar una vacuna a base de vesículas de la membrana externa derivadas de B. pertussis (BpOMVs por su abreviatura en inglés) (Patente internacional concedida en USA y en trámite en otros países, Application Number: PCT/IB2014/060143) 30,31. Este candidato ha resultado ser tanto inmunogénico como también protector y bioseguro en el modelo murino de protección, con características superadoras en comparación a los dos tipos de vacunas comerciales existentes 31,32. Nuestro candidato vacunal contiene una diversidad de inmunógenos similar a la de la vacuna celular y genera un perfil de respuesta inmunológica mixta Th1/Th2/Th17, pero siendo menos reactogénica 32. Esta nueva formulación induce una respuesta humoral robusta, un perfil protector Th1/Th17 y una población linfocitaria de memoria local en pulmón que la vacuna acelular no es capaz de montar, aunque ambas son seguras en similar medida 30,32,33. Por ello, en este trabajo de tesis seguimos avanzado en la caracterización de nuestro candidato vacunal basado en las BpOMVs, profundizando en el conocimiento de las poblaciones efectoras y los mecanismos de la respuesta inmune que colaboran en la protección desencadenada por el mismo. En particular, ahondamos en la identificación y caracterización de poblaciones linfocitarias de importancia en el contexto de la inmunización contra pertussis en el modelo murino de infección: linfocitos T CD4, linfocitos T CD8, linfocitos B220. En ese contexto, pusimos en discusión el impacto que tiene la compartimentalización de dichas poblaciones para el establecimiento de los mecanismos protectores en el individuo. Por otro lado, estudiamos las vías y mecanismos de señalización de la inmunidad innata que desencadenan las BpOMVs, en particular los asociados a la vía del inflamosoma, en miras de detallar la articulación entre la activación de la respuesta inmune innata y la respuesta adaptativa protectora que presenta nuestro candidato vacunal. Por último, generamos hibridomas productores de anticuerpos monoclonales contra BpOMVs, los cuales tienen potencial aplicación en diversos estudios de la biología, bioquímica, producción y estandarización, entre otros aspectos, de las BpOMVs y de la bacteria B. pertussis. Estas herramientas que caracterizamos bioquímica y funcionalmenteresultan de valor en el trabajo de seguir caracterizando y posicionando nuestro candidato vacunal para ensayos clínicos futuros. La experiencia y herramientas generadas durante el trabajo en pertussis, me permitieron colaborar en proyectos enfocados a la enfermedad emergente COVID-19 causada por el virus SARS-CoV-2. Esta enfermedad no sólo ha conducido a una gran crisis sanitaria y socioeconómica a nivel global, sino también humanitaria en la cual aún estamos inmersos. El sistema científico, al igual que muchos otros sectores de nuestra sociedad, focalizaron sus actividades en pos de responder a esta situación de crisis. Nuestro grupo se ha sumado a actividades con impacto en las capacidades sanitarias estatales para el manejo de esta pandemia. En particular, participé en dos proyectos, uno enfocado en la generación de hibridomas productores de anticuerpos monoclonales contra distintas proteínas del virus, con potenciales usos en diagnóstico, seguimiento, vigilancia y terapia; y el otro, enfocado en la evaluación de la capacidad inmunomodulatoria de una droga ampliamente usada en la clínica y segura (lidocaína) para posible reposicionamiento de la misma para tratar pacientes COVID-19 severos. 

Evaluación de mecanismos inmunológicos involucrados en el control de enfermedades infecciosas resurgentes (pertussis) y emergentes (COVID-19)

The aim of this study was to compare the elimination of Bordetella pertussis clinical isolates, representing different genotypes in relation to alleles encoding virulence factors (MLST—multi-locus antigen sequence typing), MLVA type (multi-locus variable-number tandem repeat analysis) and PFGE group (pulsed-field gel electrophoresis) from the lungs of naive mice or mice were immunised with the commercial whole-cell pertussis vaccine, the acellular pertussis vaccine and the experimental whole-cell pertussis vaccine. Molecular data indicate that the resurgence of pertussis in populations with high vaccine coverage is associated with genomic adaptation of B. pertussis, to vaccine selection pressure. Pertactin-negative B. pertussis isolates were suspected to contribute to the reduced vaccine effectiveness. It was shown that one of the isolates used is PRN deficient. The mice were intranasally challenged with bacterial suspension containing approximately 5 × 10 7 CFU/ml B. pertussis. The immunogenicity of the tested vaccines against PT (pertussis toxin), PRN (pertactin), FHA (filamentous haemagglutinin) and FIM (fimbriae types 2 and 3) was examined. The commercial whole-cell and acellular pertussis vaccines induced an immunity effective at eliminating the genetically different B. pertussis isolates from the lungs. However, the elimination of the PRN-deficient isolate from the lungs of mice vaccinated with commercial vaccines was delayed as compared to the PRN ( +) isolate, suggesting phenotypic differences with the circulating isolates and vaccine strains. The most effective vaccine was the experimental vaccine with the composition identical to that of the strains used for infection.
