Fine Needle Aspiration Cytology

Chest wall disorders, especially in a submammary location, occasionally mimic breast tumors. We report a case of a venous angioma between the pectoralis major and minor muscles. It presented as a palpable mass in the left breast. The mass mimicked a breast tumor both clinically and on mammography.

This study aimed to investigate the performance of the S-detect method in breast ultrasonography and to determine how to reduce unnecessary biopsy by comparing the results of the S-detect method and biopsy. Thirty patients who had undergone breast ultrasonography between August and October 2018 and were scheduled to undergo biopsy because of the presence of breast nodules were retrospectively analyzed. The McNemar test was performed to determine whether detection of a malignant breast mass significantly differed between the S-detect method and biopsy. The following results were obtained from the analysis of the S-detect method: sensitivity, 90.9%; specificity, 84.21%; validity, 86.66%; positive predictive value, 76.92%; and negative predictive value, 94.11%. Analysis of the degree of agreement between the S-detect method and biopsy revealed a kappa value as high as 0.724 (p < 0.05), exhibiting good agreement between the two methods. The S-detect method in breast ultrasonography is diagnostically valuable in terms of distinguishing between malignant and benign breast masses, and if used properly before breast biopsy, unnecessary biopsy can be reduced.

Natural Course of Cytologically Diagnosed Benign Thyroid Nodules

“화성에서 온 임상의사, 금성에서 온 병리의사”, 이는 Powsner 등이 발표한 논문의 제목이다 [1]. 이 제목만으로도 병리의사가 의미하는 판독 결과와 그것을 보는 임상의사 사이 에 괴리가 얼마나 큰지 짐작할 수 있다. 왜 병리 의사와 임상 의 사 사이에 소통의 문제가 있는 것일까? 세침흡입생검 (fine needle aspiration biopsy)은 갑상선 결 절의 양성과 악성을 구별하는데 정확한 검사이며, 이 검사가 널리 쓰인 후 불필요한 수술을 많이 줄일 수 있었다 [2-7]. 세 침흡입생검이 갑상선 결절의 진단과 치료에 핵심 역할을 하나, 생검의 결과를 보고하는 양식은 매우 다양하였다. 1997년, Papanicolaou 등은 세침흡입생검 결과를 양성 (benign), 비정 형세포의 존재 (atypical cell present), 악성 의심 (suspicious for malignancy), 악성 (malignant), 불충분검체 (inadequate or unsatisfactory)의 5가지로 분류, 보고하는 지침서를 발표 하였다 [8]. 이들은 비정형세포의 존재는 독립적인 진단명으 로 사용할 수 없으며, 반드시 임상소견과 비교하여 다른 권고 가 있어야 한다고 명시 하였다 [8]. Baloch와 LiVolsi 등은 양 성, 임상의미를 정확히 알 수 없슴 (indeterminate), 악성 의심, 악성, 불충분검체의 5가지로 분류하는 권고안을 발표 하였다 [9]. Baloch 등은 악성 의심은 소포변형유두암 (follicular variant of papillary thyroid carcinoma)에 사용해야 한다고 명 시 하였다 [9]. 이처럼 저자들마다 세침흡입생검의 결과를 보 고하는 용어가 다르고, 같은 용어를 사용하더라고 다른 정의를 내리고 있다. 같은 기관에 근무하는 병리의사들조차도 서로 각 기 다른 권고안을 사용하며, 심지어는 결론이 없이 소견만을 기술한 결과 보고도 있다 [10, 11]. 양성, 악성의 결과는 논란 의 여지가 적으나, 비정형세포의 존재, 악성 의심, 임상의미를 정확히 알 수 없슴 등의 결과는 임상의사들을 혼란스럽게 하 며, 때로는 병리의사가 말하고자 하는 바를 임상 의사가 정확 히 인지하지 못하는 경우도 있다 [10, 11]. 불충분검체와 임상 의미를 알 수 없슴을 혼돈하는 경우도 있다 [10, 11]. 병리의사 들은 임상의사가 자신이 보고한 결과를 잘 이해하고 있을 것이 라고 믿지만 [1, 11], Powsner 등의 연구에 의하면 약 30%의

54세 여자 환자가 좌측 유방 상외측에 만져지는 병변으로 유 방촬영술을 시행하였고 내부에 석회화를 동반한 종괴가 발견 되어 본원으로 전원 되었다. 본원에서 시행한 유방촬영술에서 좌측 유방 상외측 액와부 에 다수의 석회화를 포함한 약 3.3 cm 크기의 비교적 경계가 좋은 고농도의 엽상 모양 종괴가 보였다 (Fig. 1A). 유방확대 촬영술에서 엽상 모양의 고농도 종괴 내에 석회화가 있었고, 내부의 석회화 모양은 원형이며, 일부 석회화는 내부가 비어 있었다 (Fig. 1B). 유방촬영술에서 보였던 병변은 초음파 검사 에서 경계가 좋은 비균질한 저에코의 종괴로 대흉근과 소흉근 의 외측 경계 사이에 있으며 내부에 후방음영감소를 동반한 고 음영의 석회화가 있었다 (Fig. 1C). 도플러 초음파에서 색 혈류 신호는 종괴의 가장자리에 미약하게 보였다 (Fig. 1D). 흉부 전산화 단층촬영에서 종괴는 대흉근과 소흉근의 외측 경계 사 이에 위치하고 내부에 다수의 석회화를 포함하며 비교적 경계 가 좋은 계란형의 종괴였고, 근육과 비슷한 정도의 조영증강이 있었다 (Fig. 1E). 이 병변에 대하여 초음파 유도 하 14 게이지 핵생검을 실시 하였고 병리 결과상 비특이적인 지방섬유조직만 확인되었다. 따라서 절제생검을 시행하기로 하였다.

59세 여자 환자가 갑상선에 결절이 있다는 이야기를 듣고 내 원하여 시행한 갑상선 초음파상 왼쪽 갑상선 후면에 10×14 mm의 경계가 좋은 저에코성 결절 (Fig. 1)이 있었다. 결절의 많은 부분이 갑상선 조직과 인접해 있지만 후면에 위치하면서 피막으로 추정되는 고에코의 경계면이 관찰되어 부갑상선 결 절의 감별을 위해 미세침흡인세포검사와 함께 미세침세척액에 서 부갑상선 호르몬의 농도를 측정하였다. 미세침흡인세포검 사는 초음파유도 하에 23 게이지 바늘이 장착된 20 cc 주사기 를 이용해 시행하였다. 생검된 흡인액을 슬라이드에 뿌려 세포 검사를 위한 검사물을 얻은 후 그 미세침으로 1 cc의 식염수를 흡인하여 미세침을 세척한 후 다시 시험관에 담아 부갑상선호 르몬 검사를 위한 미세침세척액을 얻었다. 부갑상선 호르몬 농 도는 면역방사선법(Radioimmunoassay)을 이용하여 측정하 였다. 세포검사 상 원주 상피세포들이 밀집해있었고 악성 세포 는 보이지 않아“악성세포가 없음”으로 보고되었다. 미세침세 척액에서의 부갑상선 호르몬은 2456 pg/mL 이상으로 측정되 어 부갑상선 기원일 가능성이 높았다. 같은 날 시행하였던 혈
