Background The outbreak of the coronavirus disease 2019 (COVID-19) has had a massive impact on the whole world. Computed tomography (CT) has been widely used in the diagnosis of this novel pneumonia. This study aims to understand the role of CT for the diagnosis and the main imaging manifestations of patients with COVID-19. Methods We conducted a rapid review and meta-analysis on studies about the use of chest CT for the diagnosis of COVID-19. We comprehensively searched databases and preprint servers on chest CT for patients with COVID-19 between 1 January 2020 and 31 March 2020. The primary outcome was the sensitivity of chest CT imaging. We also conducted subgroup analyses and evaluated the quality of evidence using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Results A total of 103 studies with 5,673 patients were included. Using reverse transcription polymerase chain reaction (RT-PCR) results as reference, a meta-analysis based on 64 studies estimated the sensitivity of chest CT imaging in COVID-19 was 99% (95% CI, 0.97-1.00). If case reports were excluded, the sensitivity in case series was 96% (95% CI, 0.93-0.99). The sensitivity of CT scan in confirmed patients under 18 years old was only 66% (95% CI, 0.15-1.00). The most common imaging manifestation was ground-glass opacities (GGO) which was found in 75% (95% CI, 0.68-0.82) of the patients. The pooled probability of bilateral involvement was 84% (95% CI, 0.81-0.88). The most commonly involved lobes were the right lower lobe (84%, 95% CI, 0.78-0.90) and left lower lobe (81%, 95% CI, 0.74-0.87). The quality of evidence was low across all outcomes. Conclusions In conclusion, this meta-analysis indicated that chest CT scan had a high sensitivity in diagnosis of patients with COVID-19. Therefore, CT can potentially be used to assist in the diagnosis of COVID-19.

Chest computed tomography for the diagnosis of patients with coronavirus disease 2019 (COVID-19): A rapid review and meta-analysis

Quick screening patients with COVID-19 is the most important way of controlling transmission by isolation and medical treatment. Chest computed tomography (CT) has been widely used during the initial screening process, including pneumonia diagnosis, severity assessment, and differential diagnosis of COVID-19. The course of COVID-19 changes rapidly. Serial CT imaging could observe the distribution, density, and range of lesions dynamically, monitor the changes, and then guide towards appropriate treatment. The aim of the review was to explore the chest CT findings and dynamic CT changes of COVID-19 using systematic evaluation methods, instructing the clinical imaging diagnosis. A systematic literature search was performed. The quality of included literature was evaluated with a quality assessment tool, followed by data extraction and meta-analysis. Homogeneity and publishing bias were analyzed. A total of 109 articles were included, involving 2908 adults with COVID-19. The lesions often occurred in bilateral lungs (74%) and were multifocal (77%) with subpleural distribution (81%). Lesions often showed ground-glass opacity (GGO) (68%), followed by GGO with consolidation (48%). The thickening of small vessels (70%) and thickening of intralobular septum (53%) were also common. The dynamic changes of chest CT manifestations showed that lesions were absorbed and improved gradually after reaching the peak (80%), had progressive deterioration (55%), were absorbed and improved gradually (46%), fluctuated (22%), or remained stable (26%). The review showed the common and key CT features and the dynamic imaging change patterns of COVID-19, helping with timely management during COVID-19 pandemic.

Chest Computed Tomography for the Diagnosis of Patients with Coronavirus Disease 2019 (COVID-19): A Rapid Review and Meta-Analysis

Abstract Background The protective effect of the inactivated vaccine against the omicron variant of COVID-19 is unclear. The purpose of this study was to investigate the protective effect of different vaccination status on omicron infection. Methods In this retrospective study, we analyzed patients over 14 years old, and were diagnosed with immune breakthrough Omicron-variant infection between December 2021 and February 2022 in Tianjin, China, as well as data from several previous study infected by other types of SARS-CoV-2. The data were subdivided into three groups: patients with fully, partially and unvaccine. Differences of clinical and imaging characteristics were compared based on the different vaccination status using Pearson Chi-square test, Fisher's accuracy test and non-parametric test. All of the data were also compared with other types of SARS-CoV-2. Logistic regression and mediation effect analysis were used to assess the association between vaccination status and pneumonia progression during hospitalization. Results Among the 314 cases of immune breakthrough Omicron-variant infected patients, 21(7%) patients were unvaccinated, 134 (43%) were partially vaccinated, and 159 (50%) were full vaccinated. Among fully vaccinated patients, the proportion of patients with positive CT findings (32%) and CT score 2 (6%) was significantly lower than that of partially vaccinated (46%, 14%) and unvaccinated patients (67%, 19%) (P &lt; 0.05). CT score by vaccination status are similar between Omicron and other types, only partially vaccinated group of Omicron infected patients show lower CT score than other types infected patients (P = 0.005). Increased age and lower IgG levels were associated with the risk of disease progression. IgG level had a complete mediating effect between vaccination status/ days after vaccination and disease progression. Conclusion The inactivated vaccine provided similar protection against Omicron infection of SARS-CoV-2, compared to the patients who received other types vaccines. Compared with partially vaccinated and unvaccinated patients, fully vaccinated patients had a higher CT negative rate and a lower rate of severe pneumonia. Vaccination status and days after vaccination affect disease progression through IgG levels.

BACKGROUND: The coronavirus disease 2019 (COVID-19) is an emerging pandemic of global public health concern We aimed to summarize the characteristics of COVID-19 patients in the early stage of the pandemic and explore the risk factors of disease progression METHODS: We conducted a systematic review with meta-analysis, searching three databases for studies published between January 1, 2020, and March 18, 2020 We used random-effects models to calculate the 95% confidence intervals of pooled estimated prevalence and the odds ratio between the severe and nonsevere cases RESULTS: Ninety studies involving 16,526 COVID-19 patients were included Hypertension (191%) and diabetes (95%) were the most common comorbidities The most prevalent clinical symptoms were fever (784%), cough (585%), and fatigue (264%) Increased serum ferritin (742%), high C-reactive protein (733%), and high erythrocyte sedimentation rate (ESR) (722%) were the most frequently reported laboratory abnormalities Most patients had bilateral lung involvement (822%) and showed peripheral (669%) and subpleural (621%) distribution, with multifocal involvement (731%) And the most common CT features were vascular enlargement (643%), ground-glass opacity (GGO) (607%), and thickened interlobular septa (551%) Respiratory failure was the most common complication (307%) and the overall case-fatality rate (CFR) was 42% Moreover, male, history of smoking, and comorbidities might influence the prognosis Most clinical symptoms such as fever, high fever, cough, sputum production, fatigue, shortness of breath, dyspnoea, and abdominal pain were linked to the severity of disease Some specific laboratory indicators implied the deterioration of disease, such as leucocytosis, lymphopenia, platelet, alanine aminotransferase (ALT), aspartate aminotransferase (AST), albumin, creatinine, creatine kinase (CK), lactic dehydrogenase (LDH), C-reactive protein, procalcitonin (PCT), and D-dimer Besides, the risk of bilateral pneumonia, consolidation, pleural effusion, and enlarged mediastinal nodes was higher in severe cases CONCLUSIONS: Most COVID-19 patients have fever and cough with lymphopenia and increased inflammatory indices, and the main CT feature is GGO involved bilateral lung Patients with comorbidities and worse clinical symptoms, laboratory characteristics, and CT findings tend to have poor disease progression

Clinical characteristics, laboratory abnormalities and CT findings of COVID-19 patients and risk factors of severe disease: a systematic review and meta-analysis.

Objective
To explore the imaging changes of lung lesions in patients with imported COVID-19 patients when reaching the discharge standard.


Method
The clinical and CT imaging data of 60 patients with imported COVID-19 cured and discharged from January to February 2020 in Anhui Province were retrospectively collected. The clinical characteristics of the patients and the characteristics of chest CT images at discharge were analyzed.


Results
Fever (57 cases) and cough (55 cases) were the main symptoms in 60 patients. At the initial diagnosis, 5 cases were mild, 53 were ordinary, and 2 were severe. In 5 light patients, 3 cases were negative in the whole course of CT examination, 2 cases were negative in the first time and abnormal in the second time. . The first CT imaging features of 55 patients (53 common type and 2 severe type)were mainly bilateral lung involvement (51 cases), multiple lesions (33 cases), more common under the pleura (40 cases), and ground glass opacities were the most common. (55 cases). The clinical features of chest CT in clinical outcomes are that the ground glass shadow in the lung gradually fades and was completely absorbed (19 cases); the scope of ground glass shadow in the lung expanded and progressed to crazy-paving, consolidation shadow, and the lesion gradually absorbs again followed by Fibrous cord shadows (27 cases); ground-glass opacities in the lungs quickly progressed to a consolidation and then slowly absorbed . Most of the lesions were accompanied by more residual fibrous cord shadows (4 cases). In 2 severe patients, the lesions in the lungs were larger ground glass, and a big amount of fibrous foci remained after slowly absorption.


Conclusion
Chest CT plays an important role in the diagnosis and treatment of imported COVID-19, and the degree of lung involvement seen on CT images is in good agreement with clinical outcome.
