Xanthogranulomatous inflammation (XGI) is histopathologically characterized by a marked proliferative fibrosis, parenchymal destruction, and infiltration of foamy histiocytes intermixed with other inflammatory cells. Herein, we report a case of a 73-year-old man without symptoms who was initially diagnosed with a pancreatic cystic tumor but later with XGI in the peripancreatic region. Although XGI has been reported to occur in various organs or tissues, such as the gallbladder, kidney, bone, stomach, colon, appendix, lymph nodes, and soft tissues, XGI involving the pancreas or its surrounding tissues is extremely rare. When a pancreatic cystic lesion does not have typical clinicoradiological features of common pancreatic cystic neoplasms, this pathologic condition should be considered in the differential diagnosis.

Xanthogranulomatous Inflammation of the Peripancreatic Region Mimicking Pancreatic Cystic Neoplasm

A case of xanthogranulomatous inflammation of the small bowel in a 12-year-old male American Staffordshire Terrier is described. Disseminated yellow-white nodules 2 to 3 mm in diameter bulging on the serosal surface of the small bowel, as well as on mesenteric tissue, were detected. Histopathologic examination revealed a nodular collection of foamy cells, mainly involving serosal and muscular layers, associated with necrotic areas, hemorrhages, neovascularization, variable numbers of reactive spindle cells, neutrophils, lymphocytes, plasma cells, and rare multinucleated giant cells. Transmural lymphangectasia and mucosal lymphoplasmacytic inflammation were also observed. Both Oil Red O stain and ultrastructural study revealed lipid droplets in the cytoplasm of foamy cells. Lysozyme immunoreactivity was detected in single as well as in clustered foamy cells, while smooth muscle actin was positive in spindle cells and scattered foamy elements. Lymphangectasia associated with lymphoplasmacytic enteritis suggests a component of lymphatic fluid stasis in the pathogenesis of such lesions.

Xanthogranulomatous inflammation of the small bowel in a dog.

Xanthogranulomatous gastritis (XGG) is a rarely encountered condition, and its causative mechanism is still unclear. Given that some types of xanthogranulomatous inflammation (XGI) are associated with pathogens, infection should be considered as a possible cause of XGG. Herein, we report a case of an 86-year-old woman presenting with a large, bleeding lesion resembling a submucosal tumor. Distal gastrectomy was performed, and the surgically resected specimen revealed a mass measuring 6 × 4.5 × 3 cm and appearing yellowish on the cut surface. Histopathological examination revealed a few Actinomyces “sulfur granules” and cellular composition characteristic of XGI, supporting a diagnosis of XGG associated with actinomycosis. Gastric actinomycosis is a rare condition and has not previously been reported in association with XGG, although rare cases of XGI associated with actinomycosis have been documented in other organs.

Subcutaneous tumor in a 17-year-old female cockatiel is described. The yellowish, elliptic tumor measuring 4×3 cm was noticed by the owner of this bird, on the right side of the cloaca. The skin was damaged and the wound was bleeding. After the clinical examination, surgical removal of the tumor was suggested. Histopathological examination revealed the presence of a xanthoma. In a period of one year, neither the reoccurrence of the skin xanthoma nor any health problems were observed.

Xanthogranulomatous inflammation of terminal ileum: report of a case with small bowel involvement.

Reported herein is a rare case of xanthogranulomatous inflammation of the gastric wall occurring in a 77-year-old man. Two submucosal lesions presented as rapidly enlarging nodules, and biopsy showed interweaving bundles of spindle cells with numerous atypical cells with marked nuclear pleomorphism. The differential diagnosis from mesenchymal malignancies, particularly from a malignant gastrointestinal stromal tumor, was difficult and immunohistochemical investigations could not improve the diagnostic accuracy of HE histology alone. Thus, an erroneous diagnosis of malignancy was made and a partial gastrectomy was performed. On macroscopic examination of the resected material, spontaneous regression of the lesions was observed and microscopic examination showed characteristic features of xanthogranulomatous inflammation; large numbers of foamy histiocytes including multinucleated giant cells were admixed with chronic inflammatory cells and fibrous reaction. Although the precise pathogenesis could not be elucidated, recognition of this unusual morphological appearance is of importance to avoid an overdiagnosis of malignancy.

Laparoscopic Cholecystectomy in a Patient with Xanthogranuromatous Cholecystitis and Situs Inversus Totalis

A 20-year-old woman was hospitalized repeatedly because of intermittent bouts of intestinal obstruction and the symptoms usually improved with conservative treatments. One year after the first admission the patient was hospitalized in emergency and a laparotomy revealed a circular stricture with a pinhole perforation in the ileum. Histological sections of the stricture showed the characteristic features of microscopic polyangiitis varying from active to resolving stages, which were localized in the ileum. Fibrinoid necrosis, fibroblastic and fibrous proliferation of the intima and fibrous replacement of the media with a variable pan- and perivascular inflammatory cell infiltrate were characteristic in the muscular arteries and arterioles. Vascular occlusion by pale eosinophilic, fibrillar-like materials resembling livedo racemosa of the skin, was noticed in the small arterioles and capillaries. Under no prophylaxis, the postoperative course was uneventful with no recurrence of the illness at an 18-month follow up. The pathological alterations were distributed focally, occasionally segmentally, and haphazardly, and required detailed examination by stepwise sections for the histological diagnosis.
