Background/Aims
There has been major progress in our understanding of the irritable bowel syndrome (IBS), and novel treatment classes have emerged. The Rome IV guidelines were published in 2016 and together with the growing body of Asian data on IBS, we felt it is timely to update the Asian IBS Consensus.
Methods
Key opinion leaders from Asian countries were organized into 4 teams to review 4 themes: symptoms and epidemiology, pathophysiology, diagnosis and investigations, and lifestyle modifications and treatments. The consensus development process was carried out by using a modified Delphi method.
Results
Thirty-seven statements were developed. Asian data substantiate the current global viewpoint that IBS is a disorder of gut-brain interaction. Socio-cultural and environmental factors in Asia appear to influence the greater overlap between IBS and upper gastrointestinal symptoms. New classes of treatments comprising low fermentable oligo-, di-, monosacharides, and polyols diet, probiotics, non-absorbable antibiotics, and secretagogues have good evidence base for their efficacy.
Conclusions
Our consensus is that all patients with functional gastrointestinal disorders should be evaluated comprehensively with a view to holistic management. Physicians should be encouraged to take a positive attitude to the treatment outcomes for IBS patients.

Second Asian Consensus on Irritable Bowel Syndrome

The kidney is the organ most vulnerable to nephrotoxic drugs such as gentamicin. Nephrotoxicity is a rapid deterioration of kidney function due to various factors. Gentamicin causes nephrotoxicity, which was manifested by an increase in serum kidney biomarkers. Asparagus africanus is one of the ethnomedicinal plants used as traditional medicine for treating various ailments, including kidney disease in Ethiopian society. Thus, the aim of this study is to evaluate the nephroprotective effect of A. africanus root extract on gentamicin-induced nephrotoxicity. Using maceration techniques, 100 g of dried plant powder was extracted in 1 L of ethanol. The physicochemical screening of plant extracts revealed the presence of flavonoids, phenols, tannins, saponins, and steroids. The nephroprotective activity of A. africanus crude extract was evaluated on male Swiss albino mice. The crude ethanolic extract at 200 and 400 mg/kg doses showed strong nephroprotective effects by restoring biomarkers such as creatinine, uric acid, and blood urea nitrogen, which were damaged by gentamicin (p < 0.05) in a dose-dependent manner. The mice treated with higher doses (400 mg/kg) had a comparable nephroprotective effect compared to the positive control group (200 mg/kg silymarin; p > 0.05). The histopathology of the control group showed normal glomeruli, normal parenchyma, distal convoluted, and no tubular damage. The toxicant-induced group showed damage to glomeruli and inflammatory infiltration. Therefore, A. africanus root extract has a nephroprotective activity by retarding the gentamicin toxicity in male Swiss albino mice.

Nephroprotective Effect of Asparagus africanus Lam. Root Extract against Gentamicin-Induced Nephrotoxicity in Swiss Albino Mice

Background: Intestinal disease is a common disease, which can cause serious digestion and absorption disorders, endanger the lives of patients and seriously affect the quality of life of people. Finding an effective treatment is a difficult problem at present, and stem cell therapy as a treatment has high application potential in intestinal-related diseases. Purpose: This paper mainly summarizes the mechanism, research progress and future development trend of stem cells in the treatment of intestinal diseases in the past decade, hoping to provide reference for future researchers in the research and application of stem cells and intestinal diseases. Method: Stem cells, inflammatory bowel diseases, Crohn's disease, radiation-induced intestinal injury, radiation enterocolitis, and extracellular vesicles ' were used as search terms. Relevant references in the past ten years were searched in CNKI journal full-text database, PubMed database, VIP network and Wanfang medical network, and 80 literature meeting the requirements were finally included for review. Result: This paper summarizes the research and application of stem cells in intestinal diseases from 2012 to 2021, and expounds the specific mechanism of stem cells in the treatment of intestinal diseases. It is found that stem cells can treat intestinal injury or inflammation through different ways. Future stem cells may also be used to reverse the natural aging of intestinal function, improve intestinal function and strengthen gastrointestinal function.
