Case Report Hydatidosis is an antropozoonosis caused by the larval form of the cestode of the genus echinococcus granulosus. Despite the endemic state of the disease in some countries of the Mediterranean basin, splenic hydatidosis is rare. It comes in 3rd position after the liver and the lung localization. The treatment is mainly surgical. We report the case of a 21-year-old patient living in a rural area around Marrakech, with no particular medical history, who presented with pain in the left hypochondrium that had developed for seven months revealing a splenic hydatid cyst on ultrasound with positive hydatid serology. The abdominal CT scan showed a hydatid splenic cyst with a membrane detachment occupying almost the entire splenic parenchyma. The patient underwent a resection of the protruding dome, the evolution of which was simple.

A rare case: Spontaneous gastric fistula from a hydatid cyst of the liver

Introduction: Isolated cystic echinococcosis of the spleen (Splenic hydatid cyst) is extremely rare, ranking third after liver and lung involvement. It represents approximately 5% of all hydatid cyst localizations. Aim: To report a new case of splenic hydatid cyst and conduct a literature review on its pathophysiology, diagnostic circumstances, and therapeutic options. Observation: A 21-year-old male, professionally a student, residing in an urban area with no medical history, presented with left hypochondrial pain of a dragging nature evolving over several days. Abdominal examination revealed a visible and palpable bulge in the left hypochondrium. Ultrasonography, abdominopelvic CT scan, and hydatid serology confirmed the diagnosis of splenic hydatid cyst. Total splenectomy via laparotomy was performed in our patient. The postoperative course was uneventful. Conclusion: Isolated splenic hydatid cyst is rare. Several routes of contamination are possible for the spleen. Diagnosis is often incidental. Treatment remains primarily surgical.

Le kyste hydatique de la rate est une affection rare en Afrique sub-saharienne. Il arrive en 3 eme position apres celui du foie et du poumon. Le traitement est essentiellement chirurgical en dehors d'un traitement medical ayant prouve son efficacite. Nous rapportons le 1 er cas pris en charge au Service de Chirurgie Generale du CHU Le Dantec a Dakar. Il s'agit d'un patient de 33 ans, originaire de Mauritanie, sans antecedent particulier qui nous a ete adresse pour prise en charge d'un kyste splenique de decouverte fortuite a l'echographie lors d'un bilan de routine avec serologie hydatique positive a Eccinochoccus granulosa. L'examen clinique etait sans particularite. La biologie retrouvait une thrombopenie a 88000 plaquettes/mm 3 . Le scanner montrait des kystes spleniques avec calcifications occupant plus de la moitie du parenchyme splenique. Il a beneficie d'une splenectomie totale dont les suites ont ete simples. Aucune particularite n'etait notee apres un suivi d'un an. Le kyste hydatique splenique est rare. Le traitement est chirurgical qu'il soit radical ou conservateur avec de bons resultats.

The authors report a case of a primary sternal tumor and the problem posed during wall reconstruction in a context of limited resources.
