In humans, one highly prevalent reproductive disorder is polycystic ovary syndrome (PCOS); it is a condition characterized by anovulation, polycystic ovaries, high androgen levels, hyperinsulinemia, and predisposition for type 2 diabetes. PCOS is the most common endocrine disorder among women of fertile age. Although the pathogenesis of PCOS remains elusive, the syndrome is clearly associated with metabolic dysfunction in which hyperinsulinemia and peripheral insulin resistance are central features, quite often on a backdrop of obesity. Collectively, the metabolic perturbations in women with PCOS can increase the prevalence of cardiovascular disease including cardiomyocyte dysfunction, coronary heart disease, and impaired autophagy homeostasis. In this review, we discuss the metabolic features in PCOS focusing on obesity and type 2 diabetes, and their correlation with cardiometabolic disorders.
