Peer support group programs are often recommended for burn survivors as a way to facilitate their psychosocial recovery and reintegration into the community. Such programs provide opportunities for burn patients and their caretakers to access emotional and informational support from healthcare providers and other survivors in inpatient or outpatient settings. Despite their popularity, however, there is little information currently available on the efficacy of these groups. In response, we reviewed the existing literature on peer support group programs and their impacts on psychosocial outcomes for burn survivors and their caregivers. A systematic review of the literature utilizing PubMed, PsycINFO, and Medline databases was conducted for articles published between 1990 and 2018. Twenty-five articles including inpatient, outpatient, integrative peer support groups, and burn camps met our inclusion criteria. All inpatient peer support group program articles (n = 4) reported associations with psychosocial improvements. Integrative peer support group program articles (n = 2) reported associations with social integration and reduction in post-traumatic stress and anxiety. All outpatient peer support group program articles (n = 8) demonstrated associations with psychosocial outcomes involving life satisfaction, acceptance of self, and reduced levels of isolation. Findings were less consistent for burn camps: eight articles suggested improvements in psychological outcomes while three articles reported no significant psychosocial effects. Although these results are encouraging, further study is indicated both to replicate these findings, and to determine the optimal implementation of inpatient and outpatient peer support programs.

The Impact of Peer Support Group Programs on Psychosocial Outcomes for Burn Survivors and Caregivers: A Review of the Literature.

Introduction: Life expectancy is increasing, this leads to a major number of hip fracture and its subsequent complications and costs. Proximal femur fracture is not only a medical problem but also a social handicap. Our objective is to see, by means of a prospective observational study, how the function varies after hip fracture and which are the most influencing factors in order to get maximum benefits of resources for this patology. Materials and Methods: 262 patients had been hospitalized suffering hip fracture in our centre between june 2006 and December 2007.107 of them were included in the study, the others were excluded for different reasons: cognitive impairment (based on Folstein test), previous hip fracture or pathologic fractures. 86 female and 21 male. Mean age 74 years (65–93).44 suffering subcapital fracture and 63 pertrochanteric fracture. 76 patients underwent gamma nail or canulated screws and 31 hemi or total hip replacement. Mean surgery delay 3 days (1–14) At the moment of hospital registration we also determined marital status, familiar support, living at home or institution, morbidity pre- fracture (according to Charlson index), level of dependence (Barthel index), mental status (Folstein Mini Mental State Examination)quality of life perceived (short form 36 (SF-36)) and depression symptoms (Geriatric Depression Scale 15 (GDS 15)). At three months post discharge we determined again all the test mentioned before and also recorded the functional level (Harris test), which haven’t been passed preoperatively because it includes a part of physical evaluation, not possible to do in a fractured leg. Statistics: Analysis of Variance for repeated measures, Spearman correlation coefficient, level significance 0.05 Results: Most part of patients came from their own houses (87,6%), where only returned 23% of them at hospital discharge. In the control visit al 3 months most of them had returned at home (79%) There were no significant differences between functional outcome al three months and delay of the surgery (p 0.76) or the type of surgery (artroplasty vs osteosintesis) (p0.308). There was a negative correlation (rho-spearman −0.656)between depression and functional outcome, the more depressed a patient is the worse functional outcome he has (p 0.002) There was a positive correlation (rho-spearman 0.605) between function (Harris test) and dependency (Barthel index).Patients less independent pre fracture are the ones with low function at three months. (p 0.000) Discussion: There are a lot of patients needing nursing or social facilities at hospital discharge with the high costs that this supposes. We have seen that depressed people at hospital income are going functionally worse than non depressed and consequently are more dependent persons. Maybe early diagnosis ant treatment of depression will help to get maximum results of rehabilitation procedure.

Functional outcome after hip fracture

Abstract Purpose Peers are uniquely able to draw on their lived experiences to support trauma survivors’ recovery. By understanding the functions and outcomes of peer support and the factors that impact implementation, evidence can be mobilized to enhance its application and uptake into standard practice. As such, we aimed to review the literature on peer support for trauma survivors to: examine the role of peer support in recovery; describe the nature and extent of peer support; Examine the influence of peer support on health and well-being; and identify the barriers and facilitators to developing and implementing peer support. Methods Scoping review methodology as outlined by Arksey and O’Malley. Results Ninety-three articles were reviewed. Peer support was highlighted as an important component of care for trauma survivors and provided hope and guidance for the future post-injury. Most peer support programs were offered in the community and provided one-on-one support from peer mentors using various modalities. Interventions were successful when they involved knowledgeable peer mentors and maintained participant engagement. Prior negative experiences and stigma/privacy concerns deterred trauma survivors from participating. Conclusions Peer support fulfills several functions throughout trauma survivors’ recovery that may not otherwise be met within existing health care systems. Implications for rehabilitation By understanding the functions and outcomes and the factors that impact implementation of peer support, evidence can be mobilized to enhance its application and uptake into standard practice. Peers provide trauma survivors with socioemotional support as well as assistance in daily management and life navigation post-injury. Peer support provided hope and guidance for the future after injury and improved self-efficacy amongst trauma survivors. Peer support programs are most likely to be successful when they involve knowledgeable peer mentors, are flexibly delivered, align with organizations’ values and priorities, and have adequate resources and funding to support their implementation.

Peer support for traumatic injury survivors: a scoping review

Disparities in psychosocial outcomes after burn injury exist in patients from racial or ethnic minority groups in the United States. Peer support groups can help patients with many psychosocial aspects of recovery from burns; however, access to such support among patients of racial and ethnic minority or low socioeconomic groups are unknown. The present study examined participation rates in outpatient peer support within this patient population. Patients attending outpatient clinic at an urban safety-net hospital and regional burn center with a majority minority patient population were asked about participation in burn survivor group, interest in joining a group, and given validated survey questions about managing emotions and social interactions since injury. Current or past participation in peer support was low (4.2%), and 30.3% of patients not already in support group were interested in joining. Interest in future participation in peer support was highest among Hispanic patients (37.0%) and lowest among Black patients (0%). Logistic regression models demonstrated that increased total body surface area burned, hospital length of stay, and need for surgical intervention were associated with interest in joining or having joined a peer support group. Effectiveness of management of emotions and social interactions were not associated with interest in joining peer support in the future. These findings demonstrate a considerable difference between levels of interest and participation in peer support within this population. Improving access to and education about benefits of peer support in underresourced communities may help to address the variation in psychosocial outcomes of patients across racial or ethnic minority groups recovering from burns.

Peer Support Groups: Identifying Disparities to Improve Participation.

Nursing the burn-injured patient and supporting his/her family throughout recovery is a complex, demanding, but ultimately, rewarding professional role. The repertoire of nursing skills is varied and includes comprehensive critical care, complex wound care, pain and anxiety management, psychosocial support and community re-integration. Nurses are a consistent presence throughout all phases of a patient’s care, including arrival in the emergency department, time in the critical care unit, transfer to a burn unit/center, repatriation to a community hospital, transfer to a rehabilitation facility, and discharge to an outpatient burn clinic. Burn nursing care continues to be driven by evidence-based practices in burn care and improved upon by both quantitative and qualitative nursing research. This book chapter on “Nursing Management of the Burn Patient” is intended to assist the nurse in providing comprehensive care to the burn patient and his/her family.

Research and anecdotal evidence has shown that burn survivors benefit from peer support participation, but also that relatively few attend such groups. This study investigated Dutch adult burn survivors' views on the reasons that promote or prevent participation in peer support activities. In this study, a distinction was made between those with and without an overt need for peer support and between those who do and those who do not already experience peer contact. Questionnaires were sent online and/or per post to adult Dutch burn survivors. Data were analyzed using descriptive analyses, t-tests and χ tests. The results indicate that the main perceived benefits of participation are related to receiving and giving support rather than to personal developmental reasons. The main perceived barriers to participation are related to positive emotional states. Overall, the perceived benefits and barriers do not differ substantially between those who do and those who do not report a need for peer support or those who have and have not participated in peer support. The results of the present research may be indicative of an existing misconception among burn survivors that peer support is meant for people in great need and distress. Promoting the possibly unknown personal developmental function of peer contact, but also the importance of its reciprocal benefits might help in having more patients profit from peer support.
