Pediatric spinal epidural abscess is a major suppurative infection of the central nervous system. It is an extremely rare pathology carrying serious risk of permanent neurological sequelae if is not properly treated. All the pertinent literature was analyzed, focused on pediatric cases of spinal epidural abscess and its peculiar features. Two illustrative cases are also presented. The first case is that of a 9-year old girl who took medical attention, when she was already paraplegic. Despite prompt surgical evacuation and good neuroradiological outcome and intensive rehabilitation, motor deficits did not recover after surgery. The second case was that of a 14-year old girl who presented with fever, neck pain, and torticollis. Prompt diagnosis, decompressive surgery, and 6 weeks of antibiotics allowed good neurological outcome. The management of spinal epidural abscess includes evacuation of the abscess with decompression of the spinal cord and prolonged antibiotic treatment. The presence of neurological deficit and the delay in the initiation of proper treatment are the two factors that more worsen prognosis.

Spinal epidural abscess in children: a case-based review.

Non-Hodgkin's lymphoma (NHL) compromises the vast majority of lymphomas and predominantly takes on the form of B-cell lymphomas. More specifically, 30% of all newly diagnosed cases of NHL in the United States (US) are of diffuse large B-cell lymphoma (DLBCL) type, making it the most prevalent form of NHL in the US. Arising from either nodal or extra-nodal lymphatic tissue origin, DLBCL is an aggressive tumor which is fatal if left untreated. Primary central nervous system lymphoma is rare; however, when diagnosed, it presents as a DLBCL in 90% of patients. Herein, we present an elderly male complaining initially of acute epigastric pain but soon afterward developed acute spinal cord compressive symptoms; subsequently, it was found to be caused by a primary DLBCL diagnosed in the thoracic spinal cord. This case report presents a rare condition with unexpected initial presentation, and we attempt to illustrate the importance of early detection and treatment of DLBCL in attaining more favorable prognostic and survival rates among patients. Written consent was obtained from the patient after reading a written summary of the case report. This consent was checked and approved from the Scientific Board of the University of Aleppo.

ABSTRACT Spinal epidural abscess (SEA) is a life‐threatening condition that often presents with non‐specific symptoms, resulting in a delay in diagnosis and treatment. Rarely, it can co‐exist with acute abdominal conditions, which can further complicate the diagnosis and management. Delay in diagnosis and the presence of comorbidities may result in unfavorable outcomes. We present a case of a 55‐year‐old male diagnosed with a spinal epidural abscess presenting as low back pain, fever, and paraparesis. This case report aims to investigate the clinical presentation, diagnostic challenges, and treatment approaches for spinal epidural abscesses in low and middle‐income countries. 

Long Segment Spinal Epidural Abscess With Acute Intestinal Obstruction: A Case Report and Literature Review

Abdominal presentations in patients with spinal-epidural abscess (SEA) are not uncommon. However, it needs to be continually emphasized that the atypical presentations of these diseases could delay the timely diagnosis and consequently prompt management, which threatens spinal cord, leading to potential to paralysis or even death. We report a case of a young girl who presented with abdominal pain and fever. The diagnosis of SEA was unfortunately so delayed that the patient did not show any functional recovery postoperatively. This report aims to highlight the importance of high level of clinical suspicion to recognize an atypical presentation of the SEA. Moreover, the fully detailed history-taking and physical examination are of paramount importance. Spinal underlying pathologies should always be considered as a cause of abdominal pain, especially in children.
