Long-term Outcomes of Globe-Preserving Surgery With Proton Beam Radiation for Adenoid Cystic Carcinoma of the Lacrimal Gland

Lacrimal gland tumors are rare, with an estimated incidence of 1 per 1 million people per year in Denmark.1 The proportion of orbital spaceoccupying lesions represented by lacrimal gland tumors varies widely among studies. In a study from Wills Eye Hospital (Philadelphia, PA), of the 1018 orbital biopsies performed, 142 (14%) were from the lacrimal fossa (36% tumors and 64% inflammation); thus, lacrimal gland tumors accounted for only 5% of the orbital masses biopsied at this referral center.2 Others report that lacrimal gland tumors account for 22% to 28% of space-occupying lesions of the orbit with approximately one-half benign and the other half malignant.1 Approximately 50% of lacrimal gland masses in Denmark are tumors, with inflammatory lesions and dacryops being the most frequent non-neoplastic causes of lacrimal gland enlargement.3 In this review, we provide a brief overview of lacrimal gland anatomy and histology and discuss primary lacrimal gland tumor classification, clinical characteristics, imaging modalities, management, prognosis, and histopathology of the major types of lacrimal gland neoplasms. Genetic and molecular characteristics are examined for tumors that have been characterized. Tumors metastatic to the lacrimal gland (breast,4,5 carcinoid tumor,6 choriocarcinoma,7 cutaneous melanoma,8 esophagus,9 hepatocellular carcinoma,10 osteosarcoma,11 renal cell carcinoma12) and tumors secondarily invading the gland3 are not discussed in this review.

Lacrimal Gland Tumors.

To report a case of acinic cell carcinoma occurred in the lacrimal gland. A 59-year-old man was admitted because of sudden blurring of vision, progressive proptosis of the left eye, and mild double vision in left and down directions of the gaze (Hess-Lancaster test). His medical history detailed controlled bilateral keratoconus and open angle glaucoma. On examination, the best corrected visual acuity decreased from 8/20 till 1/50 in one week. There was a swelling of the left upper eyelid. A hard and tender mass was palpated in the superior temporal left orbit. Ultrasound scan showed an extraconal solid mass, situated in the superior lateral corner of the orbit. Computed tomography and magnetic resonance imaging (MRI) revealed a mass of two centimeters in diameter, with round well-defined outline, within the lacrimal gland. We performed an enucleoresection of the mass, via a coronal approach and a lateral orbitotomy by a piezosurgical device. The lesion appeared nodular, brownish, measuring about 2 × 1.5 cm. Histopathological findings were consistent with acinic cell carcinoma with a microcystic, focally papillary-cystic growth of pattern. Follow-up MRI outcomes led to removal of the residual lacrimal gland for suspicion of recurrence. No tumor recurrences where detected at 7-year follow-up.

ель исследования. Определить эффективность лечения и оценить витальный прогноз больных раком слезной железы (РСЖ) в отдаленном периоде наблюдения. Материалы и методы. Проведено лечение 26 больных (17 женщин, 9 мужчин) с РСЖ в возрасте от 10 до 69 лет (средний возраст − 38,9±14,9 лет). Всем больным проводилось комплексное клинико-лучевое обследование, диагноз РСЖ подтвержден морфологически после хирургического лечения. Сроки наблюдения составили от 13 до 204 месяцев (медиана − 22,5 мес.) с момента хирургического лечения. Результаты. У 3-х больных отмечена стабилизация опухолевого процесса, у остальных пациентов выявлены различные локальные/региональные рецидивы. Показано, что прогностически наиболее неблагоприятным вариантом является базалоидный вариант аденокистозного РСЖ. Отдаленные метастазы выявлены у 9 больных, 4 из которых погибли (медиана наблюдения – 22,5 мес.). Заключение. Эффективность лечения РСЖ на сегодняшний день остается низкой с высоким риском локального и регионального рецидивирования. Прогноз заболевания зависит от ряда факторов, в том числе гистологического типа опухоли, наличия периневральной инвазии и, возможно, от возраста пациентов.

To review current approaches to the diagnosis and management of the most common primary and secondary malignant tumors of the orbit in children and adults. Given the rarity of most malignant orbital tumors, randomized clinical trials are lacking, but small retrospective studies investigating multimodal treatment strategies show early promise in preserving the eye and improving survival outcomes. Malignant orbital masses may be due to a broad range of pathologies, including primary tumors of the lacrimal apparatus, tumors of mesenchymal origin involving the orbit, tumors of the optic nerve, lymphoproliferative disorders, and secondary tumors from adjacent structures or distant metastases. A good clinical knowledge of the most common characteristics and imaging findings is needed to make the correct diagnosis and provide appropriate treatment. Typically, a biopsy is required to confirm the diagnosis. Staging is also critical to determine the extent of disease. Prognosis generally depends on clinical stage and histological grade of the tumor. All patients with history of a malignant orbital tumor should undergo long-term surveillance for disease recurrence and progression.
