A medical infusion pump (10) including an improved electric motor assembly (256). The medical infusion pump (10) includes a pumping mechanism (250) and a pump chamber (132) in an operative position with respect to the pumping mechanism (250), the pumping mechanism (250), manipulating the pump chamber (132) to pump liquid through the pump chamber. An electric motor (256) having a rotatable drive shaft (316) is operably linked to the pump driving mechanism (250), and the pump driving mechanism (250) pumps a select amount of liquid through the pump chamber (132) on each rotation of the electric motor drive shaft (316). The improved electric motor assembly (256) includes a motor having a portion of a drive shaft (388) extending therefrom. A magnet (392) is attached to the extending portion of the drive shaft for rotation with the drive shaft. A flux gatherer (396) is spaced from and magnetically coupled to the magnet (392) for biasing the poles of the magnet in a select orientation relative to the flux gatherer (396) so as to stop the pump driving mechanism (250) in a select position. A magnetic position detector (400) is spaced from and magnetically coupled to the magnet (392) for emitting an electrical signal each rotation of the drive shaft (388).

Background: Studies from the Spanish influenza era reported that transfusion of influenza-convalescent human blood products reduced mortality in patients with influenza complicated by pneumonia. Treatments for H5N1 influenza are unsatisfactory, and convalescent human plasma containing H5N1 antibodies could be an effective therapy during outbreaks and pandemics. Purpose: To determine whether transfusion with influenza-convalescent human blood products reduced the risk for death in patients with Spanish influenza pneumonia. Data Sources: Manual search of English-language journals from 1918 to 1925. Citations from retrieved studies were also searched. Study Selection: Published English-language studies that had at least 10 patients in the treatment group, used convalescent blood products to treat Spanish influenza pneumonia in a hospital setting, and reported on a control or comparison group. Data Extraction: Two investigators independently extracted data on study characteristics, outcomes, adverse events, and quality. Data Synthesis: Eight relevant studies involving 1703 patients were found. Treated patients, who were often selected because of more severe illness, were compared with untreated controls with influenza pneumonia in the same hospital or ward. The overall crude case-fatality rate was 16% (54 of 336) among treated patients and 37% (452 of 1219) among controls. The range of absolute risk differences in mortality between the treatment and control groups was 8% to 26% (pooled risk difference, 21% [95% Cl, 15% to 27%]). The overall crude case-fatality rate was 19% (28 of 148) among patients who received early treatment (after <4 days of pneumonia complications) and 59% (49 of 83) among patients who received late treatment (after ≥4 days of pneumonia complications). The range of absolute risk differences in mortality between the early treatment group and the late treatment group was 26% to 50% (pooled risk difference, 41 % [Cl, 29% to 54%]). Adverse effects included chill reactions and possible exacerbations of symptoms in a few patients. Limitations: Studies were few and had many methodologic limitations. No study was a blinded, randomized, or placebo-controlled trial. Some pertinent studies may have been missed. Conclusions: Patients with Spanish influenza pneumonia who received influenza-convalescent human blood products may have experienced a clinically important reduction in the risk for death. Convalescent human H5N1 plasma could be an effective, timely, and widely available treatment that should be studied in clinical trials.

An improved medication infusion pump is provided of the type for receiving and supporting a medication-containing syringe, wherein the pump and syringe include matingly interfitted components to ensure use of the pump with a compatible and correctly installed syringe. The infusion pump comprises a pump housing defining a syringe compartment for receiving and supporting the syringe, in combination with a drive member for engaging and programmably displacing a syringe plunger to administer medication to a patient. In one preferred form, the drive member and plunger include a matingly interfitting tab and notch which are engaged when the syringe is fully and properly positioned within the syringe compartment. In another form, the tab and notch are formed on the syringe plunger and a portion of the pump housing.

The disposable administration set is injection molded as a single integral piece. The disposable administration set includes adjustment spacers disposed between the plate assembly and the cam followers which adjust the distance between them to keep the device accurate. The present invention provides an apparatus for use in a peristaltic pump 20 which prevents an inadvertent free flow condition prior to loading or after removal of an I.V. tube 26 in the peristaltic pump 20 and which also prevents the inadvertent occlusion of a loaded I.V. tube 26 in the peristaltic pump 20. A standard I.V. tube 26 has a slide clamp 109 provided thereon. The apparatus includes an I.V. tube groove 29 into which the I.V. tube 26 is positioned, a slide clamp receiving area 62 into which the slide clamp 109 is inserted, and a biased sliding member 81 which the inserted slide clamp 109 engages such that the I.V. tube 26 cannot be loaded into the apparatus without the slide clamp 109 occluding the I.V. tube 26. The present invention also contemplates a slide clamp 109' compatible with the apparatus having a surface pad 125 for facilitating automated identification of the slide clamp 109' in cooperation with a non-obstructive sensor 85 in the safety apparatus. The present invention further contemplates a slide clamp 109' having an I.V. tube receiving aperture 110' with an inner diameter profile which minimizes friction between I.V. tube contacting surfaces 126 of the aperture 110' as an I.V. tube 26 is slid transverse to its length within the aperture 110' and which has a surface pad 125 for facilitating automated identification of the slide clamp.

Intravenous tube safety apparatus

Convalescent plasma and fractionated immunoglobulins have been suggested as prophylactic or therapeutic interventions during an influenza pandemic. Intravenous immunoglobulin (IVIG) preparations manufactured from human plasma collected before the 2009 H1N1 influenza pandemic, and post-pandemic hyperimmune (H)-IVIG preparations were characterized with respect to hemagglutination inhibition (HI), microneutralization (MN) and neuraminidase-inhibiting (NAi) antibody titers against pandemic H1N1 (pH1N1) and seasonal H1N1 (sH1N1) viruses. The protective efficacy of the IVIG and H-IVIG preparations was evaluated in a SCID mouse challenge model. Substantial levels of HI, MN and NAi antibodies against pH1N1 (GMTs 1:45, 1:204 and 1: 727, respectively) and sH1N1 (GMTs 1:688, 1:4,946 and 1:312, respectively) were present in pre-pandemic IVIG preparations. In post-pandemic H-IVIG preparations, HI, MN and NAi antibody GMTs against pH1N1 were 1:1,280, 1:11,404 and 1:2,488 (28-, 56- and 3.4-fold enriched), respectively, compared to pre-pandemic IVIG preparations (p < 0.001). Post-pandemic H-IVIG (HI titer 1:1,280) provided complete protection from lethality of SCID mice against pH1N1 challenge (100% of mice survived for 29 days post-challenge). Pre-pandemic IVIG (HI titer 1:70) did not provide significant protection against pH1N1 challenge (50% of mice survived 29 days post-challenge compared to 40% survival in the buffer control group). There was a highly significant correlation between circulating in vivo HI and MN antibody titers and survival (p < 0001). The substantial enrichment of HA- and NA-specific antibodies in H-IVIG and the efficacious protection of SCID mice against challenge with pH1N1 suggests H-IVIG as a promising intervention against pandemic influenza for immunocompromised patients and other risk groups.
