TL;DR: A patient's decision to undergo surgery is influenced more by low expectations regarding the effectiveness of physical therapy than by patient symptoms or anatomic features of the chronic rotator cuff tear, which may not be the best features to use when deciding on surgical intervention.

Nonoperative Treatment of Rotator Cuff Tears.
TL;DR: Physical therapy should include scapular and glenohumeral motion and strengthening exercises as well as joint and soft-tissue mobilization to maximize a patient’s chance of successful treatment.
The duration of symptoms does not correlate with rotator cuff tear severity or other patient-related features: a cross-sectional study of patients with atraumatic, full-thickness rotator cuff tears.
