This article provides a snapshot of muscle near-infrared spectroscopy (NIRS) at the end of 2010 summarizing the recent literature, offering the present status and perspectives of the NIRS instrumentation and methods, describing the main NIRS studies on skeletal muscle physiology, posing open questions and outlining future directions. So far, different NIRS techniques (e.g. continuous-wave (CW) and spatially, time- and frequency-resolved spectroscopy) have been used for measuring muscle oxygenation during exercise. In the last four years, approximately 160 muscle NIRS articles have been published on different physiological aspects (primarily muscle oxygenation and haemodynamics) of several upper- and lower-limb muscle groups investigated by using mainly two-channel CW and spatially resolved spectroscopy commercial instruments. Unfortunately, in only 15 of these studies were the advantages of using multi-channel instruments exploited. There are still several open questions in the application of NIRS in muscle studies: (i) whether NIRS can be used in subjects with a large fat layer; (ii) the contribution of myoglobin desaturation to the NIRS signal during exercise; (iii) the effect of scattering changes during exercise; and (iv) the effect of changes in skin perfusion, particularly during prolonged exercise. Recommendations for instrumentation advancements and future muscle NIRS studies are provided.

The use of near-infrared spectroscopy in understanding skeletal muscle physiology: recent developments

Continuous measurement of pressure is vital in many fields of industry, medicine, and science. Of particular interest is the ability to measure pressure in a noninvasive and contact-free manner. This work presents the potential of oblique incident reflectometry (OIR) to monitor variation in pressure via the reduced scattering parameter (μs′). Pressure deforms the geometry of the medium and causes distortion of its internal structure and the spatial distribution of optical properties. Light scattering is related to the morphology (size, density, distribution, etc.) and refractive index distributions of the medium, and applied pressure will influence directly these parameters. Therefore, we assume that pressure can be quantitatively assessed through monitoring the reduced scattering coefficient. For this purpose, the technique of OIR to evaluate the scattering parameter during pressure variations was utilized. OIR is a simple noninvasive and contact-free imaging technique able to quantify both absorption and scattering properties of a sample. In our setup, the medium is illuminated obliquely by a narrow laser beam, and the diffuse reflectance light is captured by a CCD camera. In offline processing, the shift (δ) of the diffuse light center from the incident point is mathematically analyzed and μs′ coefficient (μs′∼δ−1) is extracted. We present here confirmation of the validity of this assumption through results of a series of experiments performed on turbid liquid and artery occlusion of a human subject under different pressure levels. Thus, μs′ has the potential to serve as a good indicator for the monitoring of pressure. 

Pressure estimation via measurement of reduced light scattering coefficient by oblique laser incident reflectometry

Rapid screening for ischemic strokes in prehospital settings may improve patient outcomes by allowing early deployment of vascular recanalization therapies. However, there are no low-cost and convenient methods that can assess ischemic strokes in such a setting. Diffuse correlation spectroscopy (DCS) is a promising method for continuous, noninvasive transcranial monitoring of cerebral blood flow. In this study, we used a DCS system to detect cerebral hemodynamics before and after acute ischemic stroke in pigs. Seven adult porcines were chosen to establish ischemic stroke models via bilateral common carotid artery ligation (n = 5) or air emboli (n = 2). The results showed a significant difference in blood flow index (BFI) between the normal and ischemic groups. Relative blood flow index (rBFI) exhibited excellent results. Therefore, the diffuse optical method can assess the hemodynamic changes in acute cerebral ischemic stroke onset in pigs, and rBFI may be a promising biomarker for identifying cerebral ischemic stroke. 

Diffuse optical detection of global cerebral ischemia in an adult porcine model

AbstractA simple prototype device that may become part of the standard divers’ safety equipment is designed, constructed, and tested in order to prove the practicability of the underlying idea. The device is based on near-infrared spectroscopy (NIRS) and can detect drops in oxyhemoglobin and rises in deoxyhemoglobin concentrations in the blood. The proposed device not only collects biometric data, but can also autonomously identify and promptly notify anomalies so as to warn and speed up appropriate rescue activities. Easiness of use and low costs are important requirements in order to make the device suitable for everyday use by most divers. A Prototype NIRS Device to Increase Safety of Diving

ABSTRACT One of the urgent tasks of modern medicine is to detect microcirculation disorder during surgery to avoid possible consequences like tissue hypoxia, ischemia, and necrosis. To address this issue, in this article, we propose a compact probe with sapphire tip and optical sensing based on the principle of spatially resolved diffuse reflectance analysis. It allows for intraoperative measurement of tissue effective attenuation coefficient and its alteration during the changes of tissue condition, caused by microcirculation disorder. The results of experimental studies using (1) a tissue‐mimicking phantom based on lipid emulsion and hemoglobin and (2) a model of hindlimb ischemia performed in a rat demonstrated the ability to detect rapid changes of tissue attenuation confirming the feasibility of the probe to sense the stressful exposure. Due to a compact design of the probe, it could be useful for rather wide surgical operations and diagnostic purposes as an auxiliary instrument.
