Recent trends in abortion and contraception in 12 countries

Some 35 million abortions occur in developing countries each year. Approximately 20 million of these are unsafe abortions which claim the lives of 67000 women as a result of related complications. These deaths represent 13% of all pregnancy-related mortality and in some countries as much as 25% of maternal deaths. In developing countries one of every 75 women die of pregnancy- or childbirth-related causes compared to one of every 7300 women in developed countries.

Postabortion family planning: addressing the cycle of repeat unintended pregnancy and abortion.

Does contraception contribute to better maternal health beyond its potential to reduce the proportion of births that are unwanted? The answer depends on the extent to which births averted by contraception would otherwise pose a greater risk to the mother than wanted or intended births. The risks of childbirth are known to vary with the mother's age and may also be linked to her parity and to the interval since the previous birth. Contraception is likely to change the age pattern of childbearing, particularly by reducing fertility at older ages, and will certainly affect parity-specific fertility. The first section of this paper reviews the evidence for the assertion that contraception benefits maternal health by reducing the number of high-risk births. The second section examines a related possibility - that unwanted births represent a greater threat to the mother's health than wanted births because less time and money are invested in antenatal and natal care. Lack of antenatal care and, to a greater extent, lack of skilled medical supervision during delivery are established risk factors for poor obstetric outcomes. This section presents new evidence on the link between unwantedness and obstetric care.
The second major contribution of contraception to reducing obstetric mortality and morbidity is related to its potential to diminish recourse to unsafe abortion. Globally, it is estimated that 22% of pregnancies are aborted (Alan Guttmacher Institute, 1999). By definition these pregnancies are unwelcome, in the sense of being either absolutely unwanted or mistimed (i.e. the mother may want to have a child at some future date but not at the time of conception). In most industrialized countries, abortion is legally permitted for a wide variety of reasons, is performed by properly trained staff and carries very little risk to the physical well-being of the mother. Conversely, in most African and Latin American countries and in many Asian countries, abortion is legally permitted only in extreme circumstances and the vast majority of abortions performed are illegal. It has been estimated that in 1995 20 million illegal abortions were carried out, of which 19 million were in developing countries (Henshaw et al., 1999). Abortion legality and safety are strongly correlated. When a pregnancy is terminated by someone lacking the necessary skills or in an environment that does not conform to minimum standards of hygiene, the risk of serious complication to the woman is high. Globally, it is estimated that about 13% of pregnancy-related and birth-related deaths in women are caused by unsafe abortion. While it might seem self-evident that greater use of contraception will lead to a reduction in abortions - both safe and unsafe - this link has been challenged and, indeed, it is true that rising levels of contraceptive use can be accompanied by a rising incidence of abortion. The third section of this paper uses published data to re-examine the relationship between changes in contraceptive practice and abortion rates.

The Effects of Contraception on Obstetric Outcomes

The contraceptive method chosen is an important determinant of contraceptive discontinuation. However, method choice is endogenous to contraceptive discontinuation. Using data from the 1997 Indonesia Demographic and Health Survey, we apply a multilevel multiprocess model to examine the impact of method choice on three types of contraceptive discontinuation. We confirm that method choice is endogenous to the processes of contraceptive abandonment and method switching, but not failure. Ignoring the endogeneity of contraceptive choice leads to various biases in the magnitude of estimated effects of method choice on abandonment and method switching, but the general conclusions are robust to these biases.

Appropriate Methods for Analyzing the Effect of Method Choice on Contraceptive Discontinuation

In Turkey the inadequacy of family planning services leads many women to have repeat abortions which constitutes a major public health issue. In response the postabortion family planning program established linkages between abortion and family planning services. This paper reviews a decade of experience in postabortion family planning services which were delivered through three related phases: 1) a pilot program in a single public maternity hospital 2) an expansion of the program into 10 public facilities and 3) further expansion into 12 private-sector and two public-sector hospitals in Istanbul. Overall the experience in Turkey shows that women turn to abortion because of lack of contraception options. In an analysis of abortion trends it is noted that the postabortion family planning program efforts implemented throughout the 1990s contributed to the decline in abortion both by increasing the use of contraceptives among women who had had an abortion and by tilting the method mix toward more effective methods. The commitment of decision-makers was a particularly important criterion in the success of postabortion interventions. At the national level the program required support from management at the Ministry of Health and at the Social Insurance Organization. The most important challenge for the postabortion family planning program in Turkey was reaching the private sector.

Bridging the gap: integrating family planning with abortion services in Turkey.

Tlie induced abortion rate in Turkey declined from a peak of 4.5 abortions per 100 women in 1988 to 2.4 in 1998. This study examines the extent to which the decline in abortion in Turkey can be attributed to increased use of modern contraceptives. Trends in induced abortion rates and in contraceptive use are examined among Turkish women together with fertility preferences, changes in the contraceptive behavior associated with abortion, and changes in the propensity to abort unwanted pregnancies. The analysis includes a number of simulations that examine what abortion levels might be in different contraceptive-use scenarios. Results indicate that the decline in abortion is due to a decrease in the number of abortions associated with traditional method failure. This decrease is related to three factors: a shift from traditional method use to modern method use, a decline in the traditional method failure rate, and a decline in the proportion of pregnancies resulting from traditional method failures that are aborted.
