We study the health trajectories of the population aged over 60, comparing between one European and two Latin American countries (Spain, Chile and Costa Rica) which have similar longevity patterns. Our focus is on functional limitation and mortality risks, considering differences by gender, education and social participation. Data come from national panel surveys (EPS, CRELES, SHARE). Multistate modelling is used to estimate transition probabilities between two health states: healthy to unhealthy, unhealthy to healthy as well as the transition to death from healthy or unhealthy states, to estimate the duration of stay in a specific state (computing healthy and unhealthy life expectancies) and the effect of the selected covariates. Results show that older Costa Ricans have the smallest gender gap in life expectancy but women have a lower healthy life expectancy compared to those in Chile and Spain. Participation in social activities leads to higher healthy life expectancy among the elderly in Costa Rica and Spain, whilst there were no relevant educational differences observed in longevity in the analysed countries. To conclude: despite the different patterns observed in health transitions and survival across the three countries, social participation is associated with greater health and longevity among people of old age, with little effect coming from educational attainment. Public policies should therefore be aimed at reducing unhealthy life years and dependency at advanced ages by promoting more engagement in social activities, especially among vulnerable groups who are more likely to experience impairment from a younger age.

The contribution of social participation to differences in life expectancy and healthy years among the older population: A comparison between Chile, Costa Rica and Spain.

Background This study aimed to explore the impact of social activity frequency on mid- and long-term overall survival in older Chinese people. Methods The association between social activity frequency and overall survival was analysed in 28 563 subjects from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) cohorts. Results A total of 21 161 (74.1%) subjects died during the follow-up of 132 558.6 person-years. Overall, more frequent social activity was associated with longer overall survival. From baseline to 5 years of follow-up, adjusted time ratios (TRs) for overall survival were 1.42 (95% CI 1.21 to 1.66, p<0.001) in the not monthly but sometimes group, 1.48 (95% CI 1.18 to 1.84, p=0.001) in the not weekly but at least once/month group, 2.10 (95% CI 1.63 to 2.69, p<0.001) in the not daily but at least once/week group, and 1.87 (95% CI 1.44 to 2.42, p<0.001) in the almost everyday group versus never group. From 5 years to the end of follow-up, adjusted TRs for overall survival were 1.05 (95% CI 0.74 to 1.50, p=0.766) in the not monthly but sometimes group, 1.64 (95% CI 1.01 to 2.65, p=0.046) in the not weekly but at least once/month group, 1.23 (95% CI 0.73 to 2.07, p=0.434) in the not daily but at least once/week group, and 3.04 (95% CI 1.69 to 5.47, p<0.001) in the almost everyday group versus the never group. Stratified and sensitivity analysis revealed similar results. Conclusion Frequent participation in social activity was significantly associated with prolonged overall survival in older people. However, only participating in social activity almost every day could significantly prolong long-term survival.

Association between social activity frequency and overall survival in older people: results from the Chinese Longitudinal Healthy Longevity Survey (CLHLS)

Background: Life participation is an outcome of critical importance to patients receiving peritoneal dialysis (PD). However, there is no widely accepted or validated tool for measuring life participation in patients receiving PD. Methods: Online consensus workshop to identify the essential characteristics of life participation as a core outcome, with the goal of establishing a patient-reported outcome measure for use in all trials in patients receiving PD. Thematic analysis of transcripts was performed. Results: Fifty-six participants, including 17 patients and caregivers, from 15 countries convened via online videoconference. Four themes were identified: reconfiguring expectations of daily living (accepting day-to-day fluctuation as the norm, shifting thresholds of acceptability, preserving gains in flexibility and freedom), ensuring broad applicability and interpretability (establishing cross-cultural relevance, incorporating valued activities, distinguishing unmodifiable barriers to life participation), capturing transitions between modalities and how they affect life participation (responsive to trajectory towards stable, reflecting changes with dialysis transitions) and maximising feasibility of implementation (reducing completion burden, administrable with ease and flexibility). Conclusions: There is a need for a validated, generalisable outcome measure for life participation in patients receiving PD. Feasibility, including length of time to complete and flexible mode of delivery, are important to allow implementation in all trials that include patients receiving PD. Establishing a core outcome measure for life participation in patients receiving peritoneal dialysis: A Standardised Outcomes in Nephrology–Peritoneal Dialysis consensus workshop report

Background: This exploratory survey study examined the relationship between older adults’ five socio-demographic determinants (urban/rural residence, gender, age, marital status, and education) and their self-reported perception of importance, desire to perform, and ability to perform nine self-care behaviors related to promoting health and getting preventive health care. Methods: We reported a secondary analysis of a dataset from an exploratory survey project; we analyzed 2015–2016 retrospective data collected from a cross-sectional survey study, including 123 adults aged 65 years and older living in southern United States. Data were collected from the Patient Action Inventory for Self-Care and a demographic questionnaire and analyzed using binary and multiple logistic regression analyses. Results: Advancing age, marital separation, and holding less than a high school education were significantly associated with at least one of the unfavorable perceptions of the importance, the desire to perform, and the ability to perform three self-care behaviors. These three behaviors were: (1) creating habits that will improve health and prevent disease, (2) discussing the use of health screening tests with healthcare providers, and (3) joining in local health screening or wellness events. Gender and urban/rural residence were not significant. Conclusions: Comprehensive health care should include an individual’s socio-demographic context and self-care perception of importance, desire, and ability.

Older Adults' Socio-Demographic Determinants of Health Related to Promoting Health and Getting Preventive Health Care in Southern United States: A Secondary Analysis of a Survey Project Dataset.

The growing population of middle-aged and older empty nesters is characterized by poorer health, and social participation (SP) has been shown to improve this situation. However, few studies have investigated specific performance and gender differences between SP and health. The present study aims to address these issues. A total of 1207 middle-aged and older empty nesters over 45 years old were selected from the China Health and Retirement Longitudinal Study (CHARLS, 2011–2018). Random-effects analyses were used to explore the association between changes in SP (diversity, frequency, type) and changes in health status. Health status include physical health, mental health, self-reported health (SRH). Female middle-aged and olderly empty nesters have significantly poorer health and participate in SP more frequently. The higher the diversity of SP, the better the health of middle-aged and olderly empty nesters, while higher frequency is beneficial to SRH. Female's participation in sports and Internet had better mental health and SRH, and mahjong helped female's mental health. Clubs are helpful for male's SRH. This study reveals the specifics of the association between SP and health status of middle-aged and older empty nesters. Therefore, all aspects of SP and gender differences should be taken into account when predicting and improving the health status. Help the government to better formulate policies to better cope with the increasing empty nest phenomenon and build a harmonious and stable society. 

Social participation and health in middle-aged and older empty nesters: A study on gender differences

This study uses nested Cox proportional hazard models to analyze data from five waves (2002, 2005, 2008, 2011, 2014) of the Chinese Longitudinal Healthy Longevity Survey. We found that secular social participation and religious participation both contributed to statistically lower death risk among older respondents. These associations still held after controlling for a wide range of covariates. In particular, secular social participation predicted a greater decrease in death risk and had interactive effects with gender and age-group. Female and young–old adults enjoyed more health benefits from participating in secular social activities than their male or old–old counterparts, respectively. This interactive effect was not observed regarding the impact of religious participation on death hazard.

 Recent research has shown that jellyfish blooms occur more frequently near nuclear power plants (NPPs), and a relationship between thermal discharges from the NPPs and the blooms has been hypothesized. There is, however, no direct evidence that links the thermal discharges with jellyfish blooms. In this study, we conducted 3 cruises (pre-bloom, bloom, and post-bloom) during July to September 2017 in the area surrounding the largest NPP in northern China. The water temperature near the cooling water outfall rose by 2 °C. Although we found no significant impact on phytoplankton abundance or community composition near the outfall, a broader analysis of monitoring data revealed significant correlations between temperature and the abundance of
 Nemopilema nomurai
 , cyanobacteria, and haptophytes. Temperature increases may have a stimulatory effect on key stages in the life history of jellyfish, and the abundance of jellyfish prey may be the factor that triggers jellyfish blooms. Blooms of jellyfish were accompanied by a decline of diatom dominance and an increase of the concentrations of small phytoplankton such as cyanobacteria and haptophytes. This study revealed for the first time a significant positive correlation (

 0.001) between jellyfish abundance and concentrations of dissolved organic nutrients. This correlation suggested that the blooms of jellyfish resulted in the release of large amounts of organic nutrients. The results provided a comprehensive perspective and suggested a need for further study of the dynamics of planktonic ecosystems to link thermal discharges and jellyfish blooms.
