Several interviews are available for assessing PTSD. These interviews vary in merit when compared on stringent psychometric and utility standards. Of all the interviews, the Clinician-Administered PTSD Scale (CAPS-1) appears to satisfy these standards most uniformly. The CAPS-1 is a structured interview for assessing core and associated symptoms of PTSD. It assesses the frequency and intensity of each symptom using standard prompt questions and explicit, behaviorally-anchored rating scales. The CAPS-1 yields both continuous and dichotomous scores for current and lifetime PTSD symptoms. Intended for use by experienced clinicians, it also can be administered by appropriately trained paraprofessionals. Data from a large scale psychometric study of the CAPS-1 have provided impressive evidence of its reliability and validity as a PTSD interview.

Hopelessness depression: A theory-based subtype of depression.

Earlier efforts to localize the symptoms of schizophrenia in a single brain region have been replaced by models that postulate a disruption in parallel distributed or dynamic circuits. Based on empirical data derived from both magnetic resonance and positron emission tomography, we have developed a model that implicates connectivity among nodes located in prefrontal regions, the thalamic nuclei, and the cerebellum. A disruption in this circuitry produces "cognitive dysmetria," difficulty in prioritizing, processing, coordinating, and responding to information. This "poor mental coordination" is a fundamental cognitive deficit in schizophrenia and can account for its broad diversity of symptoms.

This paper traces the evolution of the self-psychological view of depression from its roots in classical psychoanalytic theory and technique. Self psychology differs significantly from classical psychoanalysis in its theoretical understanding and its technical handling of the transferences and aggressive tendencies of depressed patients in psychotherapy or psychoanalysis.

The evolution of the self-psychological approach to depression

Cases of significantly depressed and regressed patients, where functional impairment necessitated psychiatric hospitalization, are presented to show the effects of a style of psychotherapeutic intervention whose goal is rapid mobilization of preexisting self-representations linked to positive affects. After defining the relevant self-psychological terms, we discuss the factors influencing emergence into consciousness of these positive affects and effects on the patient's vegetative symptoms of depression during self-psychologically oriented psychotherapy.

The psychodynamics and psychotherapy of depression : contrasting the self-psychological and the classical psychoanalytic approaches

This essay presents the case of a young man with severe posttraumatic stress disorder (PTSD) in order to demonstrate how a self-psychological orientation and psychotherapeutic approach enables acute symptoms to be relieved, while concomitantly addressing vulnerability in character structure. The self-psychological conceptualization of the psychodynamics of PTSD is discussed, along with a recently proposed neuropsychological theory of PTSD. After viewing the pertinent neurobiological research, an integration of the self-psychological concepts of transmuting internalization (a purported curative factor in psychotherapy) with neurobiological data and behavioral observations is presented.
