A Comparative Study of the Purchase, Management and Use of Large-scale Research Equipment in the UK and US Universities (report for Evidence Ltd on behalf of the Higher Education Funding Council for England)

The “Bicycle Process” is a systematic approach that requires health care providers to establish their own minimal standards of patient care. Any performance below their minimally accepted level is called a performance deficit. Educational programs are instituted for each performance deficit after which their performance is remeasured. To be successful the process requires at least the cooperation of physicians, administrators, boards of trustees and medical record analysts. At one children's hospital attempting to institute the “Bicycle Process”, the staff created 54 standards of care for 3 commonly encountered conditions (meningitis, asthma and pneumonitis). Performance deficits were shown for 34 standards before and 29 standards after the educational programs. Of the 29 remaining deficits, 20 showed improvement, 6 worsened and 3 remained unchanged. Thus, the “Bicycle Process” can lead to significant improvements in patient care and warrants close inspection by all who render patient care.

Background and Purpose

Physiotherapy or Physical Therapy (PT) is the most commonly practised allied health discipline globally. International PT workforce reforms are underway to deal with increasing patient numbers, shrinking medical and nursing workforces and lengthy waiting lists. It is timely to consider international differences in PT, with the aims of identifying opportunities for shared learning and forming stronger international alliances to support consistent and evidence-based workforce reforms.



Methods

This paper synthesizes freely available information on PT training and service delivery across the UK, Australia and United States (California). The paper considers differences in roles, workplaces, training, legislation and registration, continuing professional development, and accountability.



Results

There are similarities between UK, Australia and United States (California) in many areas of PT roles, training, registration, legislation and professional practice. However, none has a standard national mechanism by which to demonstrate PT accountability, patient safety or quality care. Moreover, there are different approaches to workforce reforms. There is considerable duplication in physiotherapy governance.



Conclusion

There are opportunities for targeted international collaborations regarding workforce reforms such as extending scope of practice, and determining and implementing internationally agreed ways of demonstrating PT accountability. The findings of this review have significant policy implications, and identify areas for collaborative research. Copyright © 2016 John Wiley & Sons, Ltd.

Physiotherapy Practice: Opportunities for International Collaboration on Workforce Reforms, Policy and Research.

Background: Excellence in Research for Australia (ERA) rankings are given to academic journals in which Australian academics publish. This provides a metric on which Australian institutions and disciplines are ranked for international competitiveness. This paper explores the issues surrounding the ERA rankings of allied health journals in Australia.Methods: We conducted a broad search to establish a representative list of general allied health and discipline-specific journals for common allied health disciplines. We identified the ERA rankings and impact factors for each journal and tested the congruence between these metrics within the disciplines.Results: Few allied health journals have high ERA rankings (A*/A), and there is variability in the impact factors assigned to journals within the same ERA rank. There is a small group of allied health researchers worldwide, and this group is even smaller when divided by discipline. Current publication metrics may not adequately assess the impact of research, which is largely aimed at clinicians to improve clinical practice. Moreover, many journals are produced by underfunded professional associations, and readership is often constrained by small numbers of clinicians in specific allied health disciplines who are association members.Conclusion: Allied health must have a stronger united voice in the next round of ERA rankings. The clinical impact of allied health journals also needs to be better understood and promoted as a research metric.
