Optimising self-care support for people with heart failure and their caregivers: development of the Rehabilitation Enablement in Chronic Heart Failure (REACH-HF) intervention using intervention mapping.
Colin J Greaves,Jennifer Wingham,Carolyn Deighan,Patrick Doherty,Jennifer Elliott,Wendy Armitage,Michelle Clark,Jackie Austin,Charles Abraham,Julia Frost,Sally Singh,Kate Jolly,Kevin Paul,Louise Taylor,Sarah Buckingham,Russell C. Davis,Hasnain M Dalal,Rod S Taylor,Reach-Hf investigators +18 more
TL;DR: The Rehabilitation Enablement in Chronic Heart Failure (REACH-HF) intervention is a comprehensive self-care support programme comprising the “Heart Failure Manual”, a choice of two exercise programmes for patients, a “Family and Friends Resource” for caregivers, a progress tracker tool and a facilitator training course.
47 Rehabilitation enablement in chronic heart failure (reach-HF) a multicentre randomised controlled trial of facilitated self-care rehabilitation intervention in heart failure with reduced ejection fraction
Hasnain M Dalal,Kate Jolly,Russell C. Davis,Patrick Doherty,Jackie Austin,Robin van Lingen,Fiona C Warren,Colin Green,Jenny Wingham,Nicky Britten,Colin J Greaves,Charles Abraham,Sally Singh,Kevin Paul,Chim C. Lang,Karen Smith,Vicky Eyre,Christopher Hayward,Rod S Taylor +18 more
TL;DR: The findings indicate that the addition of the REACH-HF home-based rehabilitation-intervention to usual care for the management of HFrEF results in clinically important improvements in disease-specific HRQoL and has the potential to reduce hospital admissions.

A randomised controlled trial of a facilitated home-based rehabilitation intervention in patients with heart failure with preserved ejection fraction and their caregivers: REACH-HFpEF pilot study
Hasnain M Dalal,Chim C. Lang,Karen Smith,Jenny Wingham,Victoria Eyre,Colin J Greaves,Fiona C Warren,Colin Green,Kate Jolly,Russell C. Davis,Patrick Doherty,Jackie Austin,Nicholas Britten,Charles Abraham,Robin van Lingen,Sally Singh,Kevin Paul,Melvyn Hillsdon,Susannah Sadler,Christopher Hayward,Rod S Taylor +20 more
TL;DR: The findings support the feasibility and rationale for the delivering the REACH-HF facilitated home-based rehabilitation intervention for patients with heart failure with preserved ejection fraction and their caregivers and progression to a full multicentre randomised clinical trial to test the clinical and cost-effectiveness of this novel intervention.
