Previous research has suggested that frequent vaginal douching modifies vaginal flora and increases the risk of cervical infections -- a probable risk factor for human immunodeficiency virus (HIV). To assess the association between HIV infection and vaginal douching 397 women attending a sexually transmitted disease clinic in Bangui Central African Republic during 1994-95 were interviewed regarding their sexual behavior and vaginal douching practices in the preceding 3 years. The overall HIV prevalence rate was 34%. Douching primarily for hygiene or treatment of a vaginal discharge was reported by 195 women (49%); its mean weekly frequency was 7 times. A commercial antiseptic was used by 140 women (72%); 48 (25%) used a noncommercial preparation (largely herbs) and 7 (3%) used water. The analysis was restricted to women who used commercial or noncommercial preparations at least once a week (104 and 32 women respectively). 25 (21.7%) of the 115 HIV-positive women reported use of commercial antiseptics compared with 79 (35.4%) of seronegative women (odds ratio 0.6; 95% confidence interval 0.4-0.9 after adjustment for condom use lifetime number of sexual partners and marital status). Use of noncommercial preparations was reported by 17 (14.8%) seropositive and 15 (6.7%) seronegative women (adjusted odds ratio 1.7; 95% confidence interval l.0-3.0). Although these findings should be considered preliminary it is speculated that the antifungal and bacteriostatic properties of commercial antiseptics used after sexual intercourse may reduce the inoculum of HIV through a flushing mechanism or reduce the frequency of sexually transmitted diseases that serve as co-factors for HIV infection. Moreover the agents used in noncommercial preparations may irritate vaginal mucosa enhancing HIV transmission.

HIV infection and vaginal douching in Central Africa.

Reproduction is a dual commitment, but so often in much of the world, it is seen as wholly the woman's responsibility. She bears the burden not only of pregnancy and childbirth but also the threats from excessive child bearing, some responsibility for contraception, infertility investigation and often undiagnosed sexually transmitted diseases (STDs) including AIDS. Failure to target men in reproductive health interventions has weakened the impact of reproductive health care programmes. The paper proposes that sophisticated and dynamic strategies in Africa and elsewhere which target women's reproductive health and research (such as control of STDs including AIDS, family planning, infertility investigation) require complementary linkage to the study and education of men. Men's perceptions, as well as determinants of sexual behavioural change and the socioeconomic context in which STDs, including AIDS, become rife, should be reviewed. There is a need to study and foster change to reduce or prevent poor reproductive health outcomes; to identify behaviours which could be adversely affecting women's reproductive health. Issues of gender, identity and tolerance as expressed through sexuality and procreation need to be amplified in the context of present risks in reproductive health. Researchers and providers often ignore the social significance of men. This paper reviews the impact of male dominance, as manifested through reproductive health and sexual decisions, against the background of present reproductive health problems. A research agenda should define factors at both macro and micro levels that interact to adversely impinge on reproductive health outcomes. This should be followed up by well-developed causal models of the determinants of positive reproductive health-promoting behaviours. Behaviour specific influences in sexual partnership include the degree of interpersonal support towards prevention, for example, of STDs, unwanted pregnancy or maternal deaths. Perceived efficacy and situational variables influencing male compliance in, say, condom use, form part of the wider study that addresses men. Thus preventive reproductive health initiatives and information should move from the female alone to both sexes. Women need men as partners in reproductive health who understand the risks they might be exposed to and strategies for their prevention.

Reproductive health and AIDS prevention in sub-Saharan Africa: the case for increased male participation

Networks of concurrent sexual partnerships may be the primary cause of epidemic spread of HIV-1 in parts of sub-Saharan Africa. This pattern of sexual behaviour increases the likelihood that individuals experiencing primary HIV-1 infection transmit the virus to other persons. Networks of concurrent partnerships are likely to be important in both the early ('epidemic') and late ('endemic') phases of HIV-1 transmission. Interventions should aim to break the sexual networks, whatever the stage of the epidemic. However, prevention of transmission in the endemic phase also requires a greater awareness of early clinical manifestations of HIV-1 infection in the general population. Such awareness, coupled with the availability of condoms and access to HIV-1 testing facilities, may reduce transmission in discordant couples.

AIDS in rural Africa: a paradigm for HIV-1 prevention:

Background Opportunities for HIV testing could be enhanced by offering HIV self-testing (HIVST) in populations that fear stigma and discrimination when accessing conventional HIV counselling and testing. Field experience with HIVST was poorly reported in French-speaking African countries. Objective To investigate the usability of HIVST in Bangui, Central African Republic. Methods The prototype self-test Exacto® Test HIV (Biosynex, Strasbourg, France) was used to assess the usability of HIVST in 300 adults living in Bangui, according to WHO technical recommendations. Simplified and easy-to-read leaflet was translated in French and Sango. Results Preliminary survey in 3,484 adult volunteers including students, men who have sex with men and female sex workers living in Bangui showed that previous HIV testing in conventional centres for HIV counselling and testing was relatively infrequent and that acceptability of HIVST was elevated, although high heterogeneity could be observed between groups. The notice in French and Sango of Exacto® Test HIV were chosen in 242/300 (80.6%) and 58/300 (19.4%), respectively. It was correctly understood in 273/300 (91.0%). The majority (275/300; 91.6%) correctly performed the HIV self-test; however, 71/300 (23.0%) asked for oral assistance. Most of the participants (273/300; 91.0%) found that performing of the self-test was very easy or easy, and less than Most of participants (273/300; 91.0%) found that performing of the self-test was very easy or easy and less than 1.0% (2/300) found it difficult. Overall the result were correctly interpreted in 96.9% (3,782/3,900), the reading/interpretion errors concerned the positive (96/1,800;5.3%), invalid (17/600;2.8%) and negative (5/1,500; 0.3%) self-test. The Cohen's coefficient κwas 0.94. The main obstacle for HIVST was the educational level, with interpretation difficulties in poorly educated people. Conclusions Our observations on profane adults living in Central African Republic, demonstrate: (i) the need to adapt the notice of instruction to African public, including educational pictograms as well as notice in vernacular language(s); (ii) the frequent difficulties in understanding the notice with frequent misinterpretation of test results; (iii) and the generally good usability of the HIV self-test despite these latter pitfalls. More research on exploring the best strategy (i.e. supervised versus unsupervised strategies) for different high- and low- risk populations in resource-constrained settings remains needed.

Acceptability and Usability Evaluation of Finger-Stick Whole Blood HIV Self-Test as An HIV Screening Tool Adapted to The General Public in The Central African Republic.

Objective 
The aims of the study were i) to categorize female sex workers (FSW) according to socio-anthropologic criteria in Bangui; ii) to examine the association between a selection of demographic and risk variables with the different categories of female sex work as outcome, and iii) to investigate factors associated with HIV status.


Methods 
A cross-sectional questionnaire survey was conducted to describe the spectrum of commercial sex work in Bangui among 345 sexually active women. After collection of social and behavioral characteristics, each woman received a physical examination and a blood sample was taken for biological analyses, including HIV testing. The relationships between sociodemographic characteristics, behavioral variables involved in high risk for HIV as well as biological results were investigated by bivariate analysis in relationship with FSW categories as main outcomes, and by bivariate analysis followed by multivariate logistic regression analysis in relationship with HIV as the main outcome. The strength of statistical associations was measured by crude and adjusted Odds ratios (OR) and their 95% confidence intervals.


Results 
The typology of FSW comprised six different categories. Two groups were the “official” professional FSW primarily classified according to their locations of work [i) “kata“(18.55%) representing women working in poor neighborhoods of Bangui; ii) “pupulenge” (13.91%) working in hotels and night clubs to seek white men]. Four groups were “clandestine” nonprofessional FSW classified according to their reported main activity [i) “market and street vendors” (20.86%); ii) “schoolgirls or students” (19.13%) involved in occasional transactional sex (during holidays); iii) “housewives or unemployed women” (15.65%); iv) “civil servants” (11.88%) working as soldiers or in the public sector]. The overall prevalence of HIV-1 was 19.12% (66/345). HIV varied according to FSW categories. Thus, among professional FSW, the HIV prevalence was 6-fold higher in "kata" than "pupulenge" (39.13% versus 6.30%; P = 0.001). Among nonprofessional FSW, the "vendors" showed the highest HIV prevalence (31.91%), which was higher than in "students" (6.10%; P = 0.001), "civil servants" (9.83%; P = 0.005), and "housewives" (13.00%; P = 0.01). In bivariate analysis, the following variables showed statistically significant association with risk for HIV infection: nationality; age of first sexual intercourse; self-assessment of HIV risk; knowledge of HIV status; anal sex practice with last clients; irregular condom use in last week; consumption of alcohol; other psycho-active substances; past history of STIs; HBs Ag; HSV-2 and bacterial vaginosis. However, the variable “sex workers categories” dichotomized into professional versus nonprofessional FSW was no longer associated with HIV. In multivariate logistical regression analysis, HIV infection was strongly associated with nationality (15.65% versus 3.77%) [adjusted OR (aOR) 3.39: 95% CI:1.25–9.16, P<0.05]; age of first sexual intercourse (21.10% versus 14.00%) (aOR 2.13: 95% CI: 1.03–4.39, P<0.05); anal sex practice with last clients (43.40% versus 11.50%) (aOR 4.31: 95% CI:2.28–8.33, P<0.001); irregular condom use in past week (33.50% versus 3.00%) (aOR 5.49: 95% CI:1.89–15.98, P<0.001); alcohol consumption before sex (34.70% versus 7.80%) (aOR 2.69: 95% CI:1.22–4.96, P<0.05); past history of STIs (41.00% versus 10.80%) (aOR 2.46: 95% CI:1.22–4.97, P<0.05) and bacterial vaginosis (29.80% versus 4.29%) (aOR 6.36: 95% CI: 2.30–17.72, P<0.001).


Conclusion 
Our observations highlight the high level of vulnerability for HIV acquisition of both poor professional “kata” and nonprofessional “street vendor” FSW categories. These categories should be particularly taken into account when designing specific prevention programs for STIs/HIV control purposes.

Association between HIV1 infection and sexually transmitted disease among men in Central Africa.

A 1988 study compared the sexual histories of 160 patients at a sexually transmitted disease (STD) clinic (98 with acute predominantly gonococcal urethritis and 62 with a genital ulcer) and 95 controls recruited from a traumatology clinic in Bangui. There were significant differences between STD patients and controls in the following demographic factors: mean age 25.4 and 28.1 years respectively); unemployed status (21.9 and 15.8%); education beyond the middle level 67.9 and 49.4%); agnosticism (26.2 and 9.6%); and body scarification (30 and 64.9%). The age at 1st intercourse was 15.9 years among STD patients and 15.0 years among controls and involved a girlfriend in 90% of men in both groups. Men in the STD group were more likely than controls to be unmarried (67.6% and 46.3% respectively) and to have fewer regular sexual partners (means of 1.0 and 1.46) and more casual partners (means of 1.59 and 0.77). 29.5% of controls and 15.6% of STD patients claimed to be in a sexually monogamous relationship at the time of interview. Mean frequency of intercourse was 7.12 times/week among STD patients and 7.58 times/week among controls. 60.6% of STD patients and 47.3% of controls claimed they had never engaged in sexual contact with a prostitute; frequent contact with prostitutes was reported by 10.6% of men in the former group and 2.2% of controls. Despite the low incidence of contact with prostitutes reported most respondents were knowledgeable about the prices charged by various classes of sex workers. In terms of sexual behavior the main difference between STD clients and controls was the greater diversity not greater activity in the former group. Perhaps the most useful finding of this study is its identification of risk factors for STDs in Africa--specifically young age unmarried status low religiosity unemployment and high intellectual level.
