An association between cancer and the coagulation system was suggested by Trousseau more than a century ago and initial reports of fibrin deposition in the stroma of solid tumors date back some 25 years. However, the validity and generality of these observations have only quite recently been established, and their implications for an understanding of tumor biology, metastasis, and therapy are only now coming to be appreciated by investigators in the mainstream of cancer research. This article reviews the current status of fibrin's role in the biology of tumor growth, considering in turn: (1) the evidence that fibrin is present in tumors, the nature of such fibrin, and its relation to plasma fibronectin; (2) the mechanisms by which fibrin may come to be deposited in tumors; and (3) the potential biological and medical significance of tumorassociated fibrin deposition and degradation. Among the last are such important possiblities as a barrier function to the immune response and possible roles in angiogenesis, desmoplasia, and metastasis.

Fibrin as a component of the tumor stroma: origins and biological significance

An association between malignancy and abnormal hemostasis has been appreciated for more than a hundred years.1.2 A broad spectrum of clinically significant hemostatic abnormalities (eg., migratory thrombophlebitis, hemorrhage, disseminated intravascular coagulation) afflicts as many as 15% of cancer patients, and hemostatic complications are the second most common cause of mortality in cancer. Only recently, however, has it been recognized that, in addition to a proclivity to systemic coagulation, cancer patients activate the clotting system locally and extravasdarly, depositing fibrin gels in solid tumors that serve as a provisional stroma and that have a role in the induction of mature tumor stroma. The Irish pathologist, R.A.Q. O'Meara, was the first to propose in the late 1950s that fibrin was deposited in solid tumors; however, his work was inconclusive because the techniques then available fbr identifylng fibrin in tissues were primitive and relatively nonspecific. The first solid evidence that fibrinogen-related proteins (FRP) were localized in solid tumors came fiom studies which showed that systemically administered fibrinogen, and also antifibrinogen antibodies, were selectively concentrated in animal and human tumors.2 Thereafter, seved groups demonstrated FRP in rat and human neoplasms by immunofluorescence. For some time there was considerable reluctance to accept even these findings. Valid questions were raised as to the specificity of the antibodies used, the generality of the observations, and the possibility ofartifict, arising from accumulation of FRP either postmortem or as tissues were being removed surg~cally. Fibrin deposits were observed in the involved lymph nodes and/or spleens of 15 patients with Hodgkin's disease by specific immunofluorescence and by electron microscopy. Two basic patterns of fibrin deposition were observed: 1) intercellular deposits, chiefly associated with nonneoplastic-appearing lymphoid cells and 2) deposits associated with the collagen fibers of young connective tissue. In addition, coarse fibrin deposits were observed in areas of necrosis, presumably a non-specific finding. Fibronectin was also observed in intercellular areas, but staining was less intense than for fibrin. Fibrin deposits were also observed in 3 of 6 cases of non-Hodgkin's lymphoma, indicating that the finding is not an exclusive feature of Hodgkin's disease. The pathogenesis and possible significance of fibrin deposition in Hodgkin's disease are related to earlier observations of activation of the coagulation system on neoplasia and cell-mediated immunity and to the possible role of fibrin, fibronectin, and their breakdown products in angiogenesis and fibroplasia.
