Abstract This paper surveys the contributions and applications of queuing theory in the field of healthcare. The paper summarizes a range of queuing theory results in the following areas: waiting time and utilization analysis, system design, and appointment systems. The paper also considers results for systems at different scales, including individual departments (or units), healthcare facilities, and regional healthcare systems. The goal is to provide sufficient information to analysts who are interested in using queuing theory to model a healthcare process and want to locate the details of relevant models.

Applying queueing theory to the study of emergency department operations: a survey and a discussion of comparable simulation studies

Emergency Department (ED) overcrowding represents a common characteristic that may affect the quality and access to health care. Analyzing the ED presentations during the last few years we have observed a constant increasing of presentation numbers. For each ED it is a challenge to decrease the patients’ waiting time, to provide timely care and to improve the patient's satisfaction. Long waiting times is the most important complaint in patient 's satisfaction surveys. We have evaluated 2195 questionnaires for a period of three years (2010-2012). The general satisfaction rate is 84, 63% and the most frequent complaints are about the waiting time which is too long, the waiting room which is small and the personnel which is insufficient. To manage these situations in a proper way we proposed, for our analysis, to use queuing models which can provide reasonably accurate evaluations of our system's performance. The data used in the case study includes detailed information's over the period January 1 - December 31, 2012, when totally 51.458 patients were registered. The results of this study can help us to understand the magnitude of the broader problem, the relationship between resources and waiting times, and to provide a method for understanding and monitoring performance, to find solutions for a better understanding and for alleviating the daily crisis.

Application of Queuing Model to Patient Flow in Emergency Department. Case Study

Queuing is a major challenge for healthcare services all over the world, but particularly so in developing countries. Application of queuing theory to enhance decision making to improve this problem is not commonly used by managers in developing countries in contrast to their counterparts in the developed world. This study investigates the application of queuing theory and modelling to the queuing problem at the out-patient department at AngloGold Ashanti hospital in Obuasi, Ghana. Using a descriptive, observational and ex-post facto case study approach, data was generated, analysed and used to model five capacity scenarios for the outpatient department. It is established that the optimum system performance can be achieved with eight doctors effectively at post from contrast to the prevailing five doctors that were effectively at post. In sum, the study establishes that applying queuing theory and modelling to queuing and capacity challenges can enhance decision making with regards to what will provide optimal performance.

Queuing theory and the management of Waiting-time in Hospitals: The case of Anglo Gold Ashanti Hospital in Ghana

This paper shows how a queueing network model helped to uncover the causes of delay in a health center appointment clinic. Patients, clerks, technicians, doctors and nurses agreed that the clerical registration area was the major bottleneck in the system. Our first reaction was to simulate the system with special attention on the complex registration procedure. Time constraints on data collection and program development led us to a queueing network model and QNA, a software tool for analyzing queueing networks developed by Whitt. The queueing analysis showed the registration area was not the bottleneck and we conjectured that delays were due to scheduling problems. A preliminary trial in the clinic of a modified appointment system showed promise with a 20 minute reduction in average time in the system (based on a small sample). Although there were significant differences between features of the real system and assumptions in the queueing network model, the queueing network model yielded insight into the operation of the appointment clinic.
