Aims Ultrasound-derived myocardial strain can render valuable diagnostic and prognostic information. However, acquisition settings can have an important impact on the measurements. Frame rate (i.e. temporal resolution) seems to be of particular importance. The aim of this study was to find the optimal range of frame rates needed for most accurate and reproducible 2D strain measurements using a 2D speckle-tracking software package.

Methods and results Synthetic two dimensional (2D) ultrasound grey-scale images of the left ventricle (LV) were generated in which the strain in longitudinal, circumferential, and radial direction were precisely known from the underlying kinematic LV model. Four different models were generated at frame rates between 20 and 110 Hz. The resulting images were repeatedly analysed. Results of the synthetic data were validated in 66 patients, where long- and short-axis recordings at different frame rates were analysed. In simulated data, accurate strain estimates could be achieved at >30 frames per cycle (FpC) for longitudinal and circumferential strains. Lower FpC underestimated strain systematically. Radial strain estimates were less accurate and less reproducible. Patient strain displayed the same plateaus as in the synthetic models. Higher noise and the presence of artefacts in patient data were followed by higher measurement variability.

Conclusion Standard machine settings with a FR of 50–60 Hz allow correct assessment of peak global longitudinal and circumferential strain. Correct definition of the region of interest within the myocardium as well as the reduction of noise and artefacts seem to be of highest importance for accurate 2D strain estimation.

The influence of frame rate on two-dimensional speckle-tracking strain measurements: a study on silico-simulated models and images recorded in patients

To date, established ultrasonic methods for myocardial regional deformation recovery are based on the Doppler effect, which has inherent limitations restricting its accuracy and use. The reported time domain methods show in vivo insufficient accuracy. A novel approach is elaborated mimicking the human observer who reaches robust diagnosis upon the B-mode data. In a region-of-interest (ROI), acoustic markers stable for tracking are selected. A weighting index presenting the quality of tracking of each marker is used for spatial polynomial fitting. For the feasibility study, a simple straight ROI was selected, which matches the septum. A thorough proof of concept is provided by comparing with a gold standard method and by applying the method to clinical datasets. The peak systolic longitudinal strains of 12 normals were -15% + -2.3% and, of 12 patients with a light-to-mild dysfunction of the apical-septal segment, they were -9% + -0.8% (p < 0.05). Enhancements of the method using spline fitting are introduced.

Neonatologists can use echocardiography for real-time assessment of the hemodynamic state of neonates to support clinical decision-making. There is a large body of evidence showing the shortcomings of conventional echocardiographic indices in neonates. Newer imaging modalities have evolved. Tissue Doppler imaging is a new technique that can provide measurements of myocardial movement and timing of myocardial events and may overcome some of the shortcomings of conventional techniques. The high time resolution and its ability to assess left and right cardiac function make tissue Doppler a favorable technique for assessing heart function in neonates. The aim of this review is to provide an up-to-date overview of tissue Doppler techniques for the assessment of cardiac function in the neonatal context, with focus on measurements from the atrioventricular (AV) plane. We discuss basic concepts, protocol for assessment, feasibility, and limitations, and we report reference values and give examples of its use in neonates.

Tissue Doppler velocity imaging and event timings in neonates: a guide to image acquisition, measurement, interpretation, and reference values

One of the major criticisms of ultrasound Doppler is its angle dependency, that is its ability to measure velocity components directly to or from the transducer only. The present article aims to investigate the impact of this angular error in a clinical setting. Apical two- and four-chamber views were recorded in 43 individuals, and the myocardium was marked by hand in each image. We assume that the main direction of the myocardial velocities is longitudinal and correct for the angular error by backprojecting measured velocities onto the longitudinal direction drawn. Strain was calculated from both corrected and uncorrected velocities in 12 segments for each individual. The results indicate that the difference between strain values calculated from corrected and uncorrected velocities is insignificant in 5 segments and within a decimal range in 11 segments. The biggest difference between measured and corrected strain values was found in the apical segments. Strain is also found to be more robust against angular error than velocities because the difference between corrected and uncorrected values is smaller for strain. Considering that there are multiple sources of noise in ultrasound Doppler measurements, the authors conclude that the angular error has so little impact on longitudinal strain that correction for this error can safely be omitted. Effect of angular error on tissue Doppler velocities and strain

Aims: Movements of myocardial walls include components of high velocity and short duration calling for a high sampling rate in the acquisition of tissue velocity imaging data. This study aims at establishing the optimal sampling requirements for tissue velocity imaging measurements. Methods and Results: In 16 healthy individuals, tissue velocity imaging data were acquired at a frame rate of 141–203 frames/s for a subsequent off-line analysis using software enabling a reduction of the sampling rate to 50%, 25% and 12.5% of the initial frame rate. Different components of the myocardial velocity profile were measured at each of these frame rates. The deviation of the results from the initial values increased markedly at decreasing frame rates, producing an underestimation of peak systolic and diastolic velocities, most other measured parameters being overestimated. A cut-off point for an acceptable 10% deviation of the results corresponded to at least 70 frames/s for peak systolic and early diastolic velocity, and to at least 100 frames/s for other systolic and diastolic parameters. Conclusion: A high sampling rate is essential for a proper rendering of tissue velocity imaging signals, too low frame rates resulting in inferior accuracy of the results. This should be kept in mind while viewing reported tissue velocity imaging data.
