Background IL-22 is expressed at barrier surfaces, which suggests its critical role in the maintenance of normal barrier homeostasis and tissue repair. IL-22 can both promote pathological inflammation and prevent the destruction of tissues. The functional outcomes of IL-22 on vascular smooth muscle cells, which are shown to regulate immune processes within the vascular wall and which are involved in certain pathologies, have not been analyzed. Objectives The effect of IL-22 on the expression of novel antiand pro-inflammatory and barrier disrupting cytokines, apoptosis and the expression of adhesive molecules in human primary aortic smooth muscle cells (AoSMC) was investigated. Material and methods Human AoSMC were induced with IL-22 for 24 h in vitro. The influence of IL-22 on IL-35 subunits EBI3 and p35, IL-33, IFN-γ and VEGF mRNA expression in Ao-SMC were assessed using real-time PCR. Additionally, the surface expression of ICAM-1 and apoptosis of AoSMC were analyzed in the flow cytometer. Results IL-22 caused a 2- and 3-fold increase of mRNA expression of the EBI3 and p35 IL-35 subunits, and a 40% decrease of IL-33 mRNA expression in AoSMC. Additionally, IL-22 decreased ICAM-1 expression on the surface of AoSMC by 30%. However, IL-22 affected neither IFN-γ and VEGF mRNA expression in AoSMC nor their apoptosis and viability. Conclusions Our data suggest that IL-22, which is released by Th22 and NK cells, may be an agent affecting the inflammatory response of AoSMC, and thus it may regulate immune homeostasis of the vascular wall.

Rebellious headache revealing an extensive rhinoscleroma: A case report and review of the literature.

Rhinoscleroma is a rare chronic granulomatous disease caused by Klebsiella rhinoscleromatis, which is a Gram-negative bacilli. Scleroma primarily affects the upper respiratory airway, preferably the nasal cavity, but the pharynx and larynx may also be involved. The term scleroma is preferred over the term rhinoscleroma because this disease affects not only the nose. Scleroma is endemic in countries of East Europe, Africa, South-East Asia and in sporadic areas worldwide. Scleroma usually begins at the nose and may progress to involve the larynx, pharynx or other regions of the neck. Scleroma generally progresses in three stages: the catarrhal or exudative phase, the proliferative or granulomatous phase, the sclerotic stage. Diagnosis of Rhinoscleroma depends on identification of the pathognomonic Mickulicz cells which is most prominent during granulomatous phase but spares or absent during catarrhal or sclerotic phases of the disease. We report a case of 11-year-old girl with the sclerotic stage of scleroma. This diagnose was proved by Mikulich cells histological detecting. We review the history, epidemiology, pathology, diagnosis, and treatment of scleroma. We report the case of a 11-year-old girl, hospitalize in our institute, diagnosed with rhinoscleroma in the context of recurrent breathlessness, wheezes and cough. The diagnosis difficulty caused by changes of nasal laryngeal mucous membrane after septoplasty 3 months ago and bronchial asthma diagnosed 2 years ago. The patient was given a six-month regimen of ciprofloxacin, and a dramatic improvement was observed.

Background: Viscosity is a common focus in the diet of patients with dysphagia. However, adhesiveness is an equally significant property that can affect swallowing function, even in semi-solid foods with similar levels of viscosity. The purpose of this study was to classify the adhesiveness of food into stages and determine whether these classifications are relevant to swallowing function. Methods: This study included 30 healthy elderly adults aged 65 years. After categorizing adhesiveness into three levels based on the results of the sensory test, 30 participants were asked to swallow representative foods at each level. A videofluoroscopic swallowing study (VFSS) was performed to determine the presence or absence of pharyngeal residues based on the level of adhesiveness. The chi-square test was used to verify whether there was a difference in remnants according to the level of adhesiveness, and significance was judged with a p-value of less than 0.05. Results: Adhesiveness was categorized into three distinct levels: level 1 (0–4 mJ), level 2 (4–18 mJ), and level 3 (>18 mJ). Upon examining the subjects presenting with residual material, we observed three cases of level 1 adhesiveness, 16 cases of level 2, and 25 cases of level 3. The chi-square test was used to assess the statistical significance between the levels, revealing a significant difference with a p-value < 0.0001. Conclusions: We presented the level of adhesiveness by dividing it into three stages and proved that it is meaningful in relation to the swallowing function. Selecting and recommending foods with an appropriate level of adhesiveness will help enhance swallowing safety in older adults.

Current Approach to Dysphagia: A Review Focusing on Esophageal Motility Disorders and Their Treatment

INTRODUCTION An 84yearold woman was hospitalised with a 1year history of aqueous diarrhoea, with 10–15 episodes a day, particularly during daytime and after meals. She also mentioned anorexia, weight loss (20% of body weight) and epigastric discomfort. There was no history of fever, vomiting or blood or mucus in the stools. She had a history of multiple surgical interventions, including a Nissen fundoplication, a left hemicolectomy due to diverticular disease, a gastrojejunostomy with adhesiolysis due to intestinal subocclusion and a cholecystectomy, the last one 6 years before. She had an extensive outpatient investigation with negative microbiological stool tests, including Clostridium difficile, normal faecal calprotectin and elastase, normal thyroid function, negative coeliac antibodies and no vitamin or nutrient deficiency. Radiological assessment, including CT and magnetic resonance enterography, showed small bowel and left colon distension, without signs of complications. A colonoscopy, performed 7 months before hospital admission, revealed erythema of the distal colon and rectum, with biopsies showing a subtle chronic inflammatory infiltrate, and a colocolonic anastomosis 45 cm from the anal verge. During hospitalisation, she had an upper gastrointestinal endoscopy (figure 1A,B, submitted in colour as online supplemental material) and a barium study (figure 2) performed.
