MRI in five term children with congenital hemiplegia without clinically apparent prenatal or perinatal difficulties showed a focal or porencephalic enlargement of the lateral ventricle and periventricular T2 prolongation, which were identical to findings of periventricular venous infarction seen in preterm children. Some cases of congenital hemiplegia in term neonates may be due to clinically silent periventricular venous infarction that occurred in utero.

Widening spectrum of congenital hemiplegia: Periventricular venous infarction in term neonates

Background Congenital subependymal pseudocysts are incidental findings that are found in 0.5-5.2% of neonates during postmortem examination or head ultrasonography. In our institution we detected 10 neonates with CSEPC. Objective To investigate associated etiological factors, morphologic characteristics and outcome of CSEPC. Methods We performed a meta-analysis of the literature on CSEPC (1967-98), including our 10 cases. Results A total of 256 cases of CSEPC were analyzed. Ultrasound diagnosed 77.6% of CSEPC; 48.8% were bilateral and 53.4% were located in the caudothalamic groove or head of caudate nucleus. Altogether, 93.5% resolved during 1-12 months of ultrasonographic follow-up. Compared to the general neonatal population, the following features were more prevalent in the CSEPC population: prematurity, maternal vaginal bleeding, preeclamptic toxemia, intrauterine growth restriction, asphyxia, fetal cytomegalovirus and rubella infections, congenital malformations, chromosomal aberrations, infant mortality, and neurodevelopmental handicap. The risk for neurodevelopmental handicap was significantly higher when CSEPC were associated with fetal infections, IUGR, malformations and chromosomal aberrations, or persistence of CSEPC during follow-up. CSEPC infants without any of these four conditions had a low risk for neurodevelopmental handicap. Conclusions CSEPC are morphologic features of various underlying conditions encountered in the fetus. Association of CSEPC with IUGR, fetal infections, malformations and chromosomal aberrations or persistence of CSEPC indicates a higher risk for future neurodevelopmental handicaps, probably because of the deleterious effects on the fetal brain that are inherent in these conditions. A favorable outcome is expected in the absence of these risk factors.

Congenital subependymal pseudocysts: own data and meta-analysis of the literature.

Cranial sonography in neonates occasionally shows subependymal cysts. These cysts may be due to a variety of pathologic disorders, but they also occur as an "isolated" condition without an obvious cause in some patients. Assessment of the clinical significance of these lesions has been difficult because of the limited duration of follow-up, the informal nature of neurodevelopmental evaluation, and the heterogeneous cohort of patients previously reported. Accordingly, the purposes of this study were to provide more complete and longer neurodevelopmental followup and to describe the prevalence and sonographic characteristics of isolated subependymal cysts detected on cranial sonograms in neonates.In a 4 1/2-year period, more than 4000 cranial sonograms were obtained at our institution. Intracranial Hemorrhage: Subdural, Primary Subarachnoid, Cerebellar, Intraventricular (Term Infant), and Miscellaneous

Serial ultrasound examinations were performed through the anterior fontanelle to detect and follow subependymal pseudocysts (SP) in 19 neonates without severe malformations. A high-resolution real-time sector scanner was employed. In 8 cases the SP involved both lateral ventricles, in 11 cases they were unilateral and in 7 neonates they were associated with other cerebral abnormalities. Follow up showed that in 13 cases the SP had disappeared by 9 months. No neurodevelopmental abnormalities have so far been observed in the 11 children with isolated SP at birth, while in 5 of the 7 cases with associated cerebral abnormalities neurodevelopmental sequelae developed.

Subependymal pseudocysts in the neonate.

The authors report three cases of unexplained prenatal intraventricular haemorrhage (IVH) in three term infants. In the first two cases the suspected diagnosis of prenatal IVH was made a few hours after delivery, in accordance with the ultrasonographic feature of clots in the ventricles, whereas in the third case prenatal ultrasonography was suggestive of hydrocephalus with intraventricular clots.
