Infant birth weight, which is classified into low birth weight, normal birth weight and macrosomia, is associated with short and long-term health consequences, such as neonatal mortality and chronic disease in life. Macrosomia and low birth weight are double burden problems in developing counties, such as Ethiopia, but the paucity of evidence has made it difficult to assess the extent of this situation. As a result there has been inconsistency in the reported prevalence of low birth weight and macrosomia in Ethiopia. This study aimed to determine the incidence and predictors of low birth weight and macrosomia in Tigray, Northern Ethiopia. We conducted a cross-sectional survey among a cohort of 1152 neonates delivered in Tigray Region at randomly selected hospitals between April and July 2014. We used the birth weight category described previously as an outcome variable. Data were collected using structured questionnaire by midwives. We entered and analyzed data using STATA™ Version 11.0. Data were described using a frequency, percentage, relative risk ratio, and 95% confidence interval. Multinomial logistic regression was conducted to identify independent predictors of low birth weight and macrosomia. In this study, we found a 10.5% and 6.68% incidence of low birth weight and macrosomia, respectively. Seventy (57.8%) of all low birth weight neonates were term births. The predictors for low birth weight were: early marriage (<18 year) (RRR: 0.59, CI: 0.35–0.97); rural residence (RRR: 0.53, CI: 0.32–0.9); prematurity (RRR: 15.4, CI: 9.18–25.9); no antenatal follow-up (RRR: 6.78, CI: 2.39–19.25); and female sex (RRR: 1.77, CI: 1.13–2.77). Predictors for macrosomia were: female gender (RRR: 0.58, CI: 0.35–0.9); high body mass index (RRR: 5.0, CI: 1.56–16); post-maturity (RRR: 2.23, CI: 1.06–4.6); and no maternal complication (RRR: 0.46, CI: 0.27–0.8). In this study, we found gestational age and gender of the neonate to be common risk factors for both low birth weight and macrosomia. Strengthening antenatal follow up, prevention of pre and post maturity, controlling body mass index, and improving socioeconomic status of mothers are recommendations to prevent the double burden (low birth weight and macrosomia) and associated short and long-term consequences.

Low birth weight and macrosomia in Tigray, Northern Ethiopia: who are the mothers at risk?

To identify risk factors for macrosomic babies. This cross-sectional analytical study was carried out in the University Teaching Hospital and the Central Hospital of Yaounde (Cameroon) from October 1st, 2012 to March 31st, 2013. Women who gave birth to ≥4,000 or 3,000–3,499 g babies were recruited. Variables recorded were fetal sex and birth weight, gestational age, maternal age, parity, mother’s body mass index (BMI), weight gain during pregnancy, previous macrosomia, and father’s BMI. Fisher exact test and student t-test were used for comparison. Level of significance was P < 0.05. Main risk factors for macrosomia are delivery of a previous macrosomic baby (OR 13.1), maternal weight gain ≥16 kg (OR 10.2), parity ≥3 (OR 4.8), father’s BMI ≥30 (OR 3.7), male sex (OR 2.2), and post-term (OR 1.9). Father’s obesity should be added among the known risk factors for macrosomia.

Risk Factors for Macrosomia

To investigate the possible risk factors related to macrosomia. Pregnant women and their newborns (n = 1041) were recruited from a cohort study in Maternal and Child Care Center of Hefei from January 2011 to July 2012. Questionnaires were applied to collect the demographic data besides the medical records. Detailed health records of the entire pregnancy were obtained using retrospective study. Meanwhile the data of neonatal outcomes was prospectively tracked. Associations between exposure risk factors and macrosomia were analyzed using Pearson's chi squared test. Logistic regression models were used to assess the independent association between these potential predictors and macrosomia. The incidence of macrosomia of this cohort was 11.24% of which male: female = 2.55:1. Male incidence (8.07%) of macrosomia was higher than female (3.17%), p < 0.001. Body mass index (BMI) before pregnancy (pre-BMI), maternal height, parity were not independently associated with macrosomia; multiple logistic regression analysis indicated that macrosomia was mainly independently associated with weight gain in pregnancy (OR=1.14, 95% CI [1.10-1.19]), maternal age (OR = 1.09, 95% CI [1.03-1.15]) and gestational age (OR = 1.62, 95% CI [1.31-1.99]), respectively. Our findings indicate that weight gain in pregnancy, maternal age and gestational age should be considered as independent risk factors for macrosomia.

Weight Gain in Pregnancy, Maternal Age and Gestational Age in Relation to Fetal Macrosomia

The increasing complexity of the Nigerian society as a result of social changes has affected the educational institution, most especially, the tertiary level of education. The Federal Government has introduced some novel practices to contend these pressures one of which is the privatization of higher education for standards and quality; increased access and better funding to meet the needs of globalization and the deregulation of hitherto publicly managed organizations. This article explores the relationship between public and private university education in Nigeria. The paper is divided into three sections. The first section analyses the historical development of the private university education in Nigeria. The second section analyses the justification for the establishment of private university institutions. The third section examines the challenges facing private university education in Nigeria and the fourth section discusses the various recommendations for quality assurance in the private university education.

Time and Change: Development of Private Universities in Nigeria

Chlamydia spp. are obligate intracellular bacteria distributed all over the world, known to cause various forms of diseases in animals and humans. In the present study, a serological survey was conducted to detect the seroprevalence and risk factors associated with rabbit chlamydiosis in northeast China, including Liaoning province, Jilin province, Heilongjiang province, and Inner Mongolia Autonomous Region. Antibodies to Chlamydia were determined by indirect hemagglutination assay (IHA). The overall seroprevalence was estimated at 17.88% in total of 800 blood samples. The Chlamydia seroprevalence varied in domestic rabbits from different factors, and genders of domestic rabbits were considered as major risk factors associated with Chlamydia infection. Our study revealed a widespread and high prevalence of Chlamydia infection in domestic rabbits in northeast China, with higher exposure risk in female domestic rabbits. These findings suggested the potential importance of domestic rabbits in the transmission of zoonotic Chlamydia infection, and thus Chlamydia should be taken into consideration in diagnosing rabbit diseases. To our knowledge, there is no report of Chlamydia infection in domestic rabbits in China and the results extend the host range for Chlamydia, which has important implications for public health and the local economy.

Seroprevalence and Risk Factors of Chlamydia Infection in Domestic Rabbits (Oryctolagus cuniculus) in China.

Background: Chlamydia infections in women cause pelvic inflammatory disease, which often results in devastating consequences of infertility, ectopic pregnancy, or chronic pelvic pain. The infection is largely asymptomatic. Objective: To determine the seroprevalence of Chlamydia trachomatis in Enugu, South Eastern Nigeria. Materials and Methods: A population-based prospective study comprising female residents of Enugu, South Eastern Nigeria. Indirect solid phase enzyme immunoassay of Chlamydia antibodies was done using ImmunoComb C0. Trachomatis IgG Kit (Orgenics). Results: The population comprised 136 female undergraduate students and 150 non-student women. The overall prevalence of C. trachomatis in the population studied was 29.4%. The percentage of subjects who admitted to be having multiple sexual partners was higher among the student population (71.2%) compared to those from the non-student population (28.8%). The highest percentage of seroprevalence was 28 (33.3%) in the age group of 20-24 years for the student population and 18 (21.4%) in the age group of 25-29 years for the non-student population. The highest seroprevalence of C. trachomatis antibodies (69.0%) in both populations was observed in females without any history of infection. Females that had pelvic inflammatory disease, sexually transmitted infection, and secondary infertility assayed for C. trachomatis had seroprevalence levels of 19%, 9.5%, and 2.4%, respectively. There was a positive correlation between positive Chlamydia assay and the type of subject population (student or non-student) with r2 value of 1.55 at P  Conclusions: C. trachomatis infection is largely underdiagnosed and remains a silent disease in the apparently healthy population of Enugu, South eastern Nigeria.
