Post traumatic stress disorder (PTSD) is the most frequently reported psychiatric morbidity among the survivors of natural disasters. It is the main hindrance to rehabilitate their life. However its prevalence particularly in Nepal is largely unknown. To investigate the prevalence of post traumatic stress disorder and use of coping strategies among the adult survivors of earthquake. A cross- sectional descriptive study was carried out on a sample of 291 adult survivors after 10 months of Nepal Earthquake 2015. Study setting was Nuwakot district with multistage sampling (cluster sampling and systematic random sampling) method. PTSD checklist-5 was used to measure PTSD, and adapted and modified brief cope scale was used to assess coping strategies. Data were analyzed using descriptive and inferential statistics (independent t-test and one-way ANOVA) at 5% level of significance. Study findings revealed that PTSD was prevalent among 24.10% of adult survivors with highest intrusion symptoms (3.24 ± 0.71). It was significantly associated with age (p = 0.017), sex (p = 0.013), education (p < 0.0001) and injury to self (p = 0.003). Elderly, females, illiterates and those who were injured during earthquake are at more risk for PTSD. Highest used coping strategy was active coping (2.92 ± 0.51). Survivors not having PTSD scored more on active coping (p < 0.0001) and self distraction coping (p = 0.006) while those with PTSD mostly used passive coping (p < 0.0001), religious coping (p < 0.0001) and substance use coping (p < 0.0001). Earthquake poses significant impact on mental health of the survivors. After 10 months of devastating earthquake, prevalence of PTSD among the survivors is high. Maladaptive coping strategies further increase possibility of PTSD. Effective screening and awareness program regarding promotion of positive coping strategies among the vulnerable groups should be reinforced for prevention of psychiatric morbidity among the survivors of earthquake.

Introduction The Sustainable Development Goals (SDGs) 2015 aim to ‘…promote well-being for all’, but this has raised questions about how its targets will be evaluated. A cross-cultural measure of subjective perspectives is needed to complement objective indicators in showing whether SDGs improve well-being. The WHOQOL-BREF offers a short, generic, subjective quality of life (QoL) measure, developed with lay people in 15 cultures worldwide; 25 important dimensions are scored in environmental, social, physical and psychological domains. Although validity and reliability are demonstrated, clarity is needed on whether scores respond sensitively to changes induced by treatments, interventions and major life events. We address this aim. Methods The WHOQOL-BREF responsiveness literature was systematically searched (Web of Science, PubMed, EMBASE and Medline). From 117 papers, 15 (24 studies) (n=2084) were included in a meta-analysis. Effect sizes (Cohen’s d) assessed whether domain scores changed significantly during interventions/events, and whether such changes are relevant and meaningful to managing clinical and social change. Results Scores changed significantly over time on all domains: small to moderate for physical (d=0.37; CI 0.25 to 0.49) and psychological QoL (d=0.22; CI 0.14 to 0.30), and small for social (d=0.10; CI 0.05 to 0.15) and environmental QoL (d=0.12; CI 0.06 to 0.18). More importantly, effect size was significant for every domain (p Conclusion International evidence from 11 cultures shows that all WHOQOL-BREF domains detect relevant, meaningful change, indicating its suitability to assess SDG well-being targets.

How will the sustainable development goals deliver changes in well-being? A systematic review and meta-analysis to investigate whether WHOQOL-BREF scores respond to change.

Post-traumatic stress disorder (PTSD) widely occurs among victims or witness of disasters. With flashbacks, hyperarousal, and avoidance being the typical symptoms, PTSD became a focus of psychological research. The earthquake in Wenchuan, China, on May 12, 2008, was without precedent in magnitude and aftermath and caused huge damage, which drew scientists' attention to mental health of the survivors. We conducted a systematic overview by collecting published articles from the PubMed database and classifying them into five points: epidemiology, neuropathology, biochemistry, genetics and epigenetics, and treatment. The large body of research during the past 6 years showed that adolescents and adults were among the most studied populations with high prevalence rates for PTSD. Genomic and transcriptomic studies focusing on gene × environment studies as well as epigenetics are still rare, although a few available data showed great potential to better understand the pathophysiology of PTSD as multifactorial disease. Phytotherapy with Chinese herbs and acupuncture are rarely reported as of yet, although the first published data indicated promising therapy effects. Future studies should focus on the following points: (1) The affected populations under observation should be better defined concerning individual risk factor, time of observation, spatial movement, and individual disease courses of patients. (2) The role of social support for prevalence rates of PTSD should be observed in more detail. (3) Efficacy and safety of Chinese medicine should be studied to find potential interventions and effective treatments of PTSD.

Systematic Review on Post-Traumatic Stress Disorder Among Survivors of the Wenchuan Earthquake:

Association between functional disability and long-term trajectories of depressive symptoms: Evidence from the China Health and Retirement Longitudinal Study.

Previous studies have showed that personality traits are associated with posttraumatic stress disorder (PTSD). The present study investigated the mediating effect of perceived social support between conscientiousness and PTSD in young Chinese firefighters. A total of four hundred and nine firefighters were recruited from a firefighter school in this study. The Chinese big five personality inventory, the PTSD checklist for the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), and perceived social support scale were used to test personality traits, PTSD and perceived social support from family and from others. The results showed that conscientiousness has a significant positive correlation with perceived social support and has a significant negative correlation with PTSD. Moreover, support from others mediated the relationship between conscientiousness and PTSD, while support from family didn't. The results of this study suggest that support from others plays a key role in the relationship between conscientiousness and PTSD of young Chinese firefighters. Clinical implications for trauma interventions and limitations were discussed.

The relationship between conscientiousness and posttraumatic stress disorder among young Chinese firefighters: The mediating effect of perceived social support.

In this study, we explored the predictors of quality of life and depressive features in older people living in temporary housing 13 months after the Wenchuan earthquake in western China. Anonymous data were collected via questionnaires in a cross-sectional survey of 189 older people living in temporary housing 13 months after the earthquake. To explore the predictors of the outcomes of interest, Pearson correlation and multiple linear regression analysis were used. The results indicated that interests/hobbies, subjective support, and family function were positive predictors of quality of life, whereas instrumental activities of daily living and depressive symptoms were its negative predictors. In addition, we found that a higher level of instrumental activities of daily living predicted a greater likelihood of depression. These results suggested that developing strategies to decrease the instrumental activities of daily living score of these people helps improve their quality of life and depression. To enhance the quality of life of these individuals, healthcare providers should also focus on developing their interests/hobbies and provide them with adequate social support, especially subjective support.

Predictors of quality of life and depression in older people living in temporary houses 13 months after the Wenchuan earthquake in western China: A cross-sectional study.

Background: The 2008 Sichuan Earthquake caused great damage to the environment and property. In the aftermath, many citizens were relocated to live in newly constructed prefabricated (prefab) communities. Purpose: This paper explored the current quality of life (QOL) of elderly residents living in prefabricated communities in areas damaged by the Sichuan earthquake and identified factors of influence on QOL values. The ultimate objective was to provide evidence-based guidance for heath improvement measures. Methods: The authors used the short form WHOQOL-BREF to assess the quality of life of 191 elderly residents of prefabricated communities in the Sichuan Province 2008 earthquake zone. A Student's t-test, variance analysis, and stepwise multivariate regression methods were used to test the impact of various factors on QOL. Results: Results indicate the self-assessed QOL of participants as good, although scores in the physical (average 56.2) and psychological (average 45.7) domains were significantly lower than the norm in China. Marital status, capital loss in the earthquake, number of children, level of perceived stress, income, interest, and family harmony each correlated with at least one of the short form WHOQOL-BREF domains in t-test and one-way analyses. After excluding for factor interaction effects using multivariate regression, we found interest, family harmony, monthly income and stress to be significant predictors of physical domain QOL, explaining 13.8% of total variance. Family harmony and interest explained 15.3% of total variance for psychological domain QOL; stress, marital status, family harmony, capital loss in the earthquake, number of children and interest explained 19.5% of total variance for social domain QOL; and stress, family harmony and interest explained 16.5% of total variance for environmental domain QOL. Family harmony and interest were significant factors across all domains, while others influenced a smaller proportion. Conclusions: Quality of life for elderly living in prefab communities should be improved. The authors hope study findings will increase awareness among healthcare providers regarding the quality of life of this vulnerable population. Study results suggest that key steps to promoting QOL in this population include improving family harmony, helping to cultivate well-rounded interests, alleviating economic stresses, providing necessary medical and psychological counseling services, and affording more social support. [Factors influencing the quality of life of elderly living in a pre-fabricated housing complex in the Sichuan earthquake area].

BACKGROUND The 8.0-magnitude earthquake that struck Wenchuan County in Sichuan was the most destructive earthquake to hit China since the People's Republic of China was founded in 1949. It caused great environmental harm and property damage, and brought considerable psychological stress and emotional problems to many in the population. PURPOSE This study was designed to investigate the psychological state of nursing students studying at a university located within the Wenchuan earthquake zone. METHODS Nurses were recruited using a striated cluster sampling approach. The study was conducted using a self-designed questionnaire, Self-Rating Depression Scale (SDS), Self-Rating Anxiety Scale (SAS), Social Support Rating Scale (SSRS) and Coping Style Questionnaire. RESULTS Most (68.8%) participants lacked sufficient knowledge regarding the earthquake, and the social support condition of college nursing students was lower than the population average (p < .001). However, degree of anxiety and depression was higher than the population average (p < .001). SSRS scores and its three dimensions were positively correlated with SAS scores and negatively correlated with SDS scores (p < .001). Participants most often employed basic problem solving approaches to deal with psychological conflicts arising from their earthquake experience. No significant difference between nursing students in different class years in terms of study variables was identified. CONCLUSION Nursing students in areas affected by the Wenchuan earthquake suffer from anxiety and depression disorders. Adequate social support represents an important intervention needed to maintain and enhance their mental health.

[A study on the psychological state of nurse-students of a university in affected areas of Wenchuan earthquake].

The psychological impact of a mudslide on survivors of the Wenchuan earthquake in China and the risk factors for development of disaster-related post-traumatic stress disorder (PTSD) were investigated. The study design was cross-sectional and included 1321 survivors who had endured both an earthquake and a mudslide. Participants filled out a self-report questionnaire. One month after the mudslide, the rate of PTSD symptoms was 18.7%. Females, the elderly, those with lower educational levels, those that lacked social support, those who did not take precautionary measures, those living with children below 6 years of age, and those who had higher exposure to traumatic events experienced a higher level of PTSD symptoms. Results indicated that timely rescue, abundant material help, and mental rehabilitation after a disaster play important roles in recovery, and that there are still some high-risk groups that need attention, care, and effective intervention from healthcare professionals and society.
