There is a broad spectrum of neurogenic tumors that involve the abdomen. These tumors can be classified as those of (a) ganglion cell origin (ganglioneuromas, ganglioneuroblastomas, neuroblastomas), (b) paraganglionic system origin (pheochromocytomas, paragangliomas), and (c) nerve sheath origin (neurilemmomas, neurofibromas, neurofibromatosis, malignant nerve sheath tumors). Abdominal neurogenic tumors are most commonly located in the retroperitoneum, especially in the paraspinal areas and adrenal glands. All of these tumors except neuroblastomas and ganglioneuroblastomas are seen in adult patients. Abdominal neurogenic tumor commonly manifests radiologically as a well-defined, smooth or lobulated mass. Calcification may be seen in all types of neurogenic tumors. The diagnosis of abdominal neurogenic tumor is suggested by the imaging appearance of the lesion, including its location, shape, and internal architecture. Benign and malignant neurogenic tumors are difficult to differentiate unless distant metastatic foci are seen. For malignant tumors, imaging modalities other than computed tomography (CT) and magnetic resonance (MR) imaging may be necessary for staging. However, because most neurogenic tumors in adults are benign, CT and MR imaging can be used to develop a differential diagnosis and help determine the immediate local extent of tumor.

Neurogenic tumors in the abdomen: tumor types and imaging characteristics.

Purpose: The purpose of the current study was to define the imaging findings of adult neuroblastoma (ANB) and correlate them with clinicopathologic features. Materials and Methods: The CT scans and MR images of six patients with histologically confirmed ANB (mean age, 49 years) were retrospectively analyzed. Results: The arising sites of tumors included the retroperitoneum (n = 2), pelvis (n = 2), anterior mediastinum (n = 1), and adrenal gland (n = 1). The common CT finding was poorly marginated and heterogeneous mass without calcification. Cystic components were often found. On contrast-enhanced CT or MR images, tumors showed heterogeneous enhancement. On T2-weighted images, all masses demonstrated predominantly hyper signal intensity relative to skeletal muscle and the images showed heterogeneous appearance with focal areas of high intensity interspersed with septations of low signal intensity. Soft tissue masses with bone involvement were identified in one case on MR images. The CT findings in one tumor originating from mediastinum were similar to those of tumors arising from other sites. Conclusion: ANB is an uncommon malignancy that often pursues an aggressive clinical course, involves multiple sites, and has a poor prognosis. Tumors usually manifest on CT or MR images as a poorly demarcated mass mainly in the retroperitoneum and pelvis. Imaging studies can depict aggressive characteristics and disease extent of ANB.

Adult Neuroblastoma: Radiologic and Clinicopathologic Features

Obstructive jaundice is a rare symptom in the neuroblastic tumor. Seven cases of obstructive jaundice caused by neuroblastoma have been reported, and only three of these patients had the symptom at the onset of the disease. The authors report a case of ganglioneuroblastoma presenting with obstructive jaundice as a rare initial feature. After the histologic diagnosis with open biopsy, chemotherapy consisting of cisplatin, pirarubicin hydrochloride/doxorubicin, cyclophosphamide, and vincristine was given. The treatment resulted in reduction in the tumor size and relief of the obstructive jaundice. Complete resection of the tumor was possible after five courses of chemotherapy.

Obstructive jaundice as a presentation of ganglioneuroblastoma

Neuroblastoma is an embryonal tumor from the neural crest cell, which generally occurs in young children with a median age of about 17 months. The incidence of adult was only 0.12 cases per million per year. Because metastatic disease was present in approximately one-third of all adult patients at diagnosis, accurate staging was necessary before the initial therapy. We report F-18 FDG PET/CT findings a case of adult neuroblastoma in the anterior mediastinum with skeletal and pulmonary metastasis, which demonstrates the utility of F-18 FDG PET/CT in the staging of adult neuroblastoma.

Obstructive jaundice as a presentation of abdominal tumors in childhood is extremely rare. To date, only 4 cases of neuroblastoma causing obstructive jaundice at diagnosis have been reported in children. We report a 4-year-old boy who presented to the emergency department with abdominal pain, jaundice, choluria and acholia. A diagnosis of unresectable, nonmetastatic neuroblastoma was made. Chemotherapy reduced the size of the tumor and relieved the symptoms of obstructive jaundice without the need for decompressive surgery. Abdominal tumors should be included in the differential diagnoses of obstructive jaundice in childhood.
