Introduction Alveolar osteitis (AO) is the most common complication faced by exodontia patients and is usually seen 24–74 hours after tooth extraction, heralded by severe throbbing pain. Nigella sativa is commonly known as black seed known to have anti-inflammatory and antibacterial properties along with other reparative properties that enhance bone formation. This study aimed to evaluate and compare the effects of Alvogyl and a mixture of Nigella sativa powder and oil in the treatment of dry sockets. Materials and Methods Sixty patients above the age of 18 and below 70 years, from both genders, who underwent extraction of teeth and were clinically diagnosed with a dry socket at the clinic of the College of Dentistry, Jouf University, Saudi Arabia, were included in this study. Pain scores were assessed after placement of the dressing at the following intervals: 5 minutes, 30 minutes, 60 minutes, 2nd day, 4th day, and 7th day. Patients were randomly allocated to three groups, namely, Group 1 (Alvogyl), Group 2 (mixture of Nigella sativa's powder and oil), and Group 3 (control). Pain relief and healing of the socket were compared between the three groups. The collected data were subjected to statistical analysis through Spearman's correlation test, independent t-test, ANOVA, and post hoc test. Results A mixture of Nigella sativa powder and oil showed a statistically significant difference in relieving pain compared to the Alvogyl group. A mixture of Nigella sativa's powder and oil required fewer dressings when compared to the Alvogyl group. Conclusion A mixture of Nigella sativa powder and oil is the more efficacious dressing material for the management of dry sockets compared to Alvogyl. It provides immediate and complete pain relief and fewer numbers of repeated visits.

A Comparative Study on Alvogyl and a Mixture of Black Seed Oil and Powder for Alveolar Osteitis: A Randomized Double-Blind Controlled Clinical Trial

Aim: The purpose of this study was to evaluate the clinical pattern of post-extraction wound healing and determination the types, incidence, and pattern of healing complications following non-surgical tooth extraction.Materials and Methods: this study includes 156 patients, who were come for extractions of teeth. This study was carried out at specialized dental centre of AL-Thawra during the period from the beginning of May to the end of September 2007). For each patient collected information pre-operatively included age and gender of the patient, indications of extraction, and tooth/teeth removed. Extractions were performed under local anesthesia with dental forceps, elevators, or both. Patients were evaluated on the third and seventh postoperative days for alveolus healing assessmentResults: one hundred forty-one patients (141) with 159 extraction teeth were evaluated for alveolus healing. Uncomplicated healing was in 142 (89.3 %), while 17 alveoli (10.7 %) developed healing complications. These complications were: localized osteitis 13 (8.2 %); acutely infected alveolus 3 (1.9 %); and an acutely inflamed alveolus1 (0.6%). Females developed more complications than males (p=0.003). Most complications were found in molars (82.5 %) and premolars (17.5 %). Localized osteitis caused severe pain in all cases, while infected and inflamed alveolus caused mild or no pain.Conclusions: Most of the post-extraction alveoli healed uneventfully. Apart from alveolar osteitis (AO), post-extraction alveolus healing was also complicated by acutely infected alveoli and acutely inflamed alveoli. This study also demonstrated a painful alveolus is not necessarily a disturbance of post-extraction site wound healing; a thorough clinical examination must, therefore, be made to exclude any of the complications.

Post operative complications associated with non- surgical tooth extraction

Aim 

The aim of the present study was to compare bacterial load using preoperative rinsing and swabbing techniques for oral surgery with 0.2% chlorhexidine (CHX).

 Methods 

Participants were healthy volunteers undergoing a general anesthetic for the removal of teeth. Participants were randomly allocated to receive 15 mL of 0.2% CHX for 60 s as either a rinse or have their mouths swabbed. Plaque samples were aseptically collected pre- and post-rinsing from the same sites in all patients (the distal surface of all second molar teeth).

 Results 

Patients in the swab group had similar bacteria counts before and after the application of CHX (143.4 vs 138.5 colonies, P = 1.000). After rinsing with CHX, there was an eightfold reduction of bacterial load (71 vs 8.8 colonies, P < 0.001).

 Conclusion 

The present study demonstrates that the use of CHX as an antimicrobial agent is effective in reducing the overall number of bacterial colonies in the oral cavity. Rinsing is a more effective method of doing this.

Bacterial comparison of preoperative rinsing and swabbing for oral surgery using 0.2% chlorhexidine

Objective: To compare disinfecting effectiveness of 2% glutaraldehyde and 4.8% chloroxylenol on tooth extraction instruments at the Department of Oral Surgery, Faculty of Dentistry, University of North Sumatera. Material and Methods: This was an experimental study with posttest only control group design approach. Purposive technique is applied to collect samples which are lower molar extraction forceps. In this study, sample were divided into 2 groups and each consisting of 18 instruments. The treatment group was treated with 2% glutaraldehyde while the control group was treated with 4.8% chloroxylenol. Each instrument was pre-cleaned using a brush, water and soap for both groups underwent the disinfection process. Results: The results were statistically analyzed using Mann-Whitney Test. The comparison between glutaraldehyde and chloroxylenol showed a significant difference to the total bacteria count on instrument after disinfection (p=0.014 < 0.05). Conclusion: 2% glutaraldehyde was more effective than 4.8% chloroxylenol at disinfecting lower molar extraction forceps. Latar Belakang. Penelitian seputar ozonazed water akhir-akhir ini cukup banyak diminati karena bahan ini mempunyai potensi antibakteria yang menjanjikan. Tujuan Penelitian ini adalah untuk mengetahui pengaruh ozonated water pada pertumbuhan bakteri Staphylococcus Aureus. Metode Penelitian . Penelitian ini dilakukan dengan cara mencampurkan 1 ml larutan bakteri 10 6 CFU/ml dengan 10 ml ozonated water konsentrasi 4 ppm selama 10 detik (kelompok I), 20 detik (kelompok II), 30 detik  (kelompok III), dan 40 detik (kelompok IV), akuades (kontrol negatif) kemudian semua diencerkan dengan akuabides 10ml. selanjutnya dari masing-masing kelompok diambil 0,01 ml untuk ditanam pada MHA, inkubasi 24 jam pada suhu 37 0 C dalam anaerobic jar kemudian dilakikan perhitungan koloni. Data dianalisis dengan Avana satu jalur dilanjutkan dengan uji LSD. Hasil. Avana satu jalur memperlihatkan adanya perbedaan yang bermakna antar kelompok perlakuan. Uji LSD menunjukkan perbedaan rerarta yang bermakna (p<0,05) antar kelompok I, II, III, IV dengan kelompok V (akuedes) terdapat perbedaan rerata yang bermakna antara kelompok I dan kelompok VI (iod). Tidak terdapat perbedaan bermakna antara kelompok II, III, IV terdapat kelompok VI (oid). Uji korelasi Produk Momen Pearson menunjukkan adanya korelasi negatif yang sangat kuat. Kesimpulan . Ozonated Water 4 ppm memiliki daya antibakreri dalam menghambat pertumbuhan Staphylococcus Aureus. Semakin lama waktu kontak, daya anti bakterinya semakain besar. Background. Some previous research show that the antibacterial effect of ozonated water is very promi-sing. The aim of this study was to know the antibacterial effect of ozonated water on staphylococcus aureus. Method. One ml of 10 6 CFU/ ml S. aureus suspension was mixed with 10 ml ozonated water for 10 second (group I), 20 second (group II), 30 second (group III), 40 second (group IV)As negative control S. aureus was mixed with aquadest (group V), and as positive control S. aureus was mixed with iod (groupVI). Furthermore, 0,01 ml from each group was cultivated on MHA, incubated for 24 hours-37C followed by colony caunting. Data was analyzed using one way anova followed by LSD. Result . One way anova show significant difference among the group and LSD test show significant mean different between group I, II, III, IV, and V. Significant difference can also be seen between group I and V. No significant difference between group II, III, IV and VI. Product momen test show strong negative correlation. Conclusion . Ozonated water 4 ppm has antibacterial effect on S. aureus. The longer the contact time, the stronger the antibacterial effect.

Pengaruh Ozonated Water sebagai Antiseptik dalam Menghambat Pertumbuhan Staphilococcus Aureus (in vitro)

Streptococcus mutans is a bacterium that plays an essential role in forming dental caries. Odontogenic infection is an oral cavity infection that can develop from dental caries if not appropriately treated. Caries treatment can be done with the use of antimicrobial agents. The increasing resistance to antimicrobial drugs has made herbal medicines the choice because they are more effective and less harmful. The community uses turmeric leaves as a cooking spice and treatment for stomach aches in children. Turmeric leaf extract contains flavonoids, tannin, and phenolic antimicrobial active compounds. This research aims to find out if turmeric leaf extract inhibits the growth of the Streptococcus mutans. This study uses a post-test-only control condition approach in a laboratory setting. The concentration of turmeric leaf extract tested was 10%, 15%, and 20% with the positive control (Chlorhexidine 0,2% mouthwash) and negative control (DMSO) with five repetitions. Antibacterial effectiveness test using Kirby-Bauer disc diffusion method. The data analysis using ANOVA showed a p-value of 0.00 on inhibition, which indicates that turmeric leaf extract can suppress the development of Streptococcus mutans. The average inhibition zone obtained was 8.7 mm at a concentration of 10%, at a concentration of 15% at 9.5 mm, and at a concentration of 20% at 8.54 mm. Inhibitory test of turmeric leaves extract (Curcuma Longa. L) against the growth of Streptococcus mutans bacterial growth in vitro

Objective: This study aims to investigate the effects of black cumin extract (Nigella sativa) on the process of wound healing in tooth extraction sockets. Material and Methods: This study involved 24 patients from the Department of Oral and Maxillofacial Surgery grouped into the control and treatment groups. After extracting the first or second molar left or right in adult patients, the black cumin extractwas applied to the socket in the treatment group as soon as possible while it was not applied in the control group. The sockets were divided into 1,2,3 groups, which were treated as black cumin extracts of 10%, 12%, and 14% respectively. On day 1,3,7, patients were recalled and sockets size were calculated to analyze wound healing. Results: Black cumin was found to be able to make the wound healing faster in the treatment group when compared to the control group. Although the effect was not immediately apparent on the second day, on the seventh day the group treated with black cumin extract had a smaller sizing average compared to the control group. The concentration of 12% black cumin extract was found to have the best effect on wound healing. Significance of findings: Black cumin extract can make wound healing in tooth extraction sockets in patients.

Comparison of the effectivity of handrubbing and handwashing on the number of bacterial colonization on clinical students at the Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, Universitas Sumatera Utara March-May 2018

Cleft lip and palate (CLP) are one of the birth defects commonly found in Medan and surrounding areas with approximately 150 cases handled annually at Mitra Sejati and Grand Med Hospitals. Generally, labioplasty surgery is the only given treatment, but fabrication of the feeding plate before or after surgery is important to cover the palatal defect. From the prosthodontics aspect, fabrication after surgery cannot be performed because most parents do not know the importance of the feeding plate. To overcome this problem, community service was conducted by fabricating feeding plates that will be given 1 week after surgery. This was achieved by getting information about patients with CLP, general examination by paediatricians, the impression of the oral cavity in the operating room before surgery, outline design, and feeding plate fabrication at the dental laboratory. Subsequently, the insertion was carried out to evaluate whether infants can drink normally using a bottle. A total of 18 infants with CLP recognized the benefits of the feeding plate in Mitra Sejati and Grand Med Hospitals. The education given had a positive effect on knowledge about the importance of feeding plate and motivation for its use to make infants drink normally and gain weight according to age.

Feeding Plate Fabrication for Infants with Cleft Palate Congenital Disorder at Mitra Sejati Hospital Medan and Grandmed Hospital Lubuk Pakam

Objective: Tooth extraction in oral surgery can lead to bleeding. Ozone water is a material that is able to accelerate healing process and prevent socket inflammation by stimulating and speeding up the work of cells like fibroblasts. Moreover, ozone water quickens the release of oxygen to the tissue to kill microorganism in tooth socket and accelerate healing process. Material and Methods: This is experimental research using post-test only with control group design. Ozone water is made using DSM Aero High Voltage. The ozonation process takes 30 minutes and followed by soaking a tampon in the ozone water. Patients in the treatment group bite the tampon for 30 minutes while patients in the control group bite a dry tampon. After 30 minutes, the tooth socket is checked to observe the inflammation using the Gingival Score Index. Patients are instructed to come back 7 days after the tooth extraction to check on the inflammation. There is a total of 38 samples in the study and they are divided into 2 groups (treatment and control). This study is conducted in the Department of Oral Surgery and Maxillofacial Faculty of Dentistry USU. Results: The average of Gingival Index after 30 minutes is 0.42±0.507 and 0.95±0.911 for treatment and control groups, respectively. On day 7 the average is 0.00±0.00 for treatment group and 0.37±0.496 for control group. Conclusion: Ozone water is able to stimulate and quicken the healing process for wounds and reduce the inflammation of the tooth socket.
