Background and Objective Sinusitis of odontogenic source is common disease accounts for about one tenth of cases of all maxillary sinusitis. Odontogenic sinusitis differs in its pathophysiology, clinical manifestation and the management from sinusitis of other causes. As the advance of dental procedure, the characteristics of odontogenic sinusitis are changing. The purpose of this study was to define the characteristics of odontogenic sinusitis with respect to the pathophysiology and management. Subjects and Method Sixty two patients who were diagnosed to odontogenic sinusitis between May 2000 and May 2008 were included. All the diagnosis was supported by both dentist and otolaryngologist with objective data such as endoscopic exam and radiologic finding. All the medical records were reviewed retrospectively. Results Dental caries was most common cause of odontogenic sinusitis. Iatrogenic injuries during invasive procedure such as tooth extraction and implants were considerable. Most commonly presented symptom is facial discomfort. The surgical managements including local treatment were needed over 80% of cases. Average follow up period was 15.7 months, no complication was observed. Conclusion The half of cases of odontogenic sinusitis was resulted from iatrogenic problem. The combination of a medical and surgical approach is generally required for the treatment.

The present case involved osteomyelitis related to a local infection of dental origin and was located in the rear area of the lingula of the mandible. We decided to use sagittal split ramus osteotomy to access the osteomyelitis area. Under general anesthesia, we successfully performed surgical sequestrectomy and curettage via sagittal split ramus osteotomy.

Treatment of osteomyelitis in the rear area of the lingula of the mandible using sagittal split ramus osteotomy: a case report.

Infantile osteomyelitis is a rare disease that is infective in nature and may rapidly turn fatal, as the disease is often misdiagnosed due to its varied presenting signs. Early diagnosis may help in avoiding systemic involvement and permanent deformity. The disease presents with signs of orbital involvement, nasal congestion, and emesis, as well as other standard hallmarks of infection. Furthermore, the maxilla is a highly vascular and porous bone and the occurrence of osteomyelitis in an infant maxilla is highly uncommon. In addition, routine blood work is not suggestive of the presence of this disease. Thus, prompt diagnosis of this condition poses a challenge to surgeons due to the confusing array of symptoms combined with the rarity of the disease. One such case of osteomyelitis of the maxilla in a young child is presented. The dilemma encountered by the surgeon during the diagnosis and treatment of the disease is discussed.
