Enthesitis: A hallmark of psoriatic arthritis.

Peyronie’s disease (PD) is a connective tissue disorder which can result in penile deformity. The prevalence of diagnosed PD in the United States (US) has been estimated to be 0.5% in adult males, but there is limited additional information comparing definitive and probable PD cases. We conducted a population-based survey to assess PD prevalence using a convenience-sample of adult men participating in the ResearchNow general population panel. Respondents were categorized according to PD status (definitive, probable, no PD) and segmented by US geographic region, education, and income levels. Of the 7,711 respondents, 57 (0.7%) had definitive PD while 850 (11.0%) had probable PD. Using univariate logistic regression modeling, older age (18–24 vs 24+) (OR = 0.721; 95% CI = 0.570,0.913), Midwest/Northeast/West geographic region (South vs Midwest/Northeast/West) (OR = 0.747; 95% CI = 0.646,0.864), and higher income level (<25K vs 25K+) (OR = 0.820; 95% CI = 0.673,0.997) were each significantly associated with reduced odds of having a definitive/probable PD diagnosis compared with no PD diagnosis. When all three variables were entered in a stepwise multivariable logistic regression, only age (OR = 0.642; 95% CI = 0.497, 0.828) and region (OR = 0.752; 95% CI = 0.647, 0.872) remained significant. This study is the first to report PD prevalence by geographic region and income, and it advocates that the prevalence of PD in the US may be higher than previously cited. Further, given the large discrepancy between definitive PD cases diagnosed by a physician and probable cases not diagnosed by a physician, much more needs to be done to raise awareness of this disease.

Ultrasound assessment of elbow enthesitis in patients with seronegative arthropathies

Recently, it was reported that phentolamine redosing during penile duplex can abolish a false diagnosis of venous leakage in patients with impotence. The aim of this study is to identify any useful role of phentolamine redosing in diagnosis of venogenic impotence. Sixty-seven consecutive patients complaining of weak erection for at least 6 months were included in this study. Penile color Doppler ultrasound (CDU) was performed using a 7.5 MHz linear array transducer with a color flow mapping capability. Following intracavernous injection of 20 microg prostaglandin E1 (PGE1), all patients with persistent end diastolic velocity (EDV) >5 cm/sec with an erectile response of E3 or lower, 20 min after intracavernosal injection of PGE1, were asked to revisit our clinic for a second CDU, 2 weeks later. During initial CDU examination, all 67 patients experienced poor response to 20 microg PGE1 with their average peak systolic velocity (PSV) and EDV being 42.8 and 6.6 cm/sec, respectively. The second CDU examination had similar results to the first one. Addition of 2 mg phentolamine did not significantly change the PSV and EDV of cavernosal arteries in any of the 67 patients. In conclusion, addition of intracavernous phentolamine during PGE1 CDU examination carries no advantage over the use of PGE1 alone regarding cavernosal artery response in patients with suspected venogenic EDV.

Significance of phentolamine redosing during prostaglandin E1 penile color Doppler ultrasonography in diagnosis of vascular erectile dysfunction.

Ejaculatory duct obstruction (EDO) is diagnosed in approximately 5% of azoospermic men. EDO is considered in patients with low semen volume, azoospermia or oligospermia and evidence of obstruction on transrectal ultrasonography. Theoretically, prostatic cysts may cause EDO either by compression or only displacement of ejaculatory ducts. The aim of the present study is to establish a protocol for management of cases of cystic ejaculatory duct obstruction presenting with azoospermia or oligozoospermia. Patients and Methods: Nineteen patients with diagnosed EDO due to prostatic cyst were included in the present study. TRUS guided aspiration of prostatic cyst with analysis of cyst contents together with TRUS guided seminal vesiculography was done to all patients. Follow up semen analysis was done after 1 month. If no improvement in semen, TURED was performed. If sperm appear in semen post-aspiration, follow up semen was done every month for 3 months. If deterioration in semen parameters was found, TURED was performed. Follow up semen was done one month after TURED. Results: The seminal vesicle was found to be communicating with ED in 7 patients (Group I) and was not communicating with the cyst in 12 patients (Group II). In group I, post-aspiration semen analysis did not show any improvement in all patients so TURED was done for all of them. Post-TURED semen analysis showed improvement in semen in 6/7 patients while one remained azoospermic. In Group II, post-aspiration semen analysis showed improvement in 9/12 cases but semen deteriorated in 6/9 patients with initial improvement. TURED was done for the 9 patients (3+6). Post-TURED semen showed improvement in semen in 6/9 cases. Conclusion: We concluded that the best management in case of cystic EDO is to do seminal vesiculography. If the seminal vesicles are communicating with the cyst, go for TURED directly. If there is no communication, TRUS guided aspiration may be offered to the patient.

A New Algorithm For Management Of Ejaculatory Duct Obstruction Due To Prostatic Cyst In Infertile Males

BACKGROUND: Spontaneous spinal epidural hematoma (SSEH) is a rare pathology, which carries a significant morbidity. OBJECTIVE: To review our institutional experience of surgically managed patients with SSEH, seeking to better understand clinical prognostic factors related to postoperative outcomes and thereby improve counseling of patients before treatment. METHODS: All patients who underwent surgical management of SSEH between September 2011 and 2021. Baseline and postoperative clinical and radiological characteristics are presented, including the American Spinal Injury Association grade (ASIA). Statistical analyses were performed using Stata 13.1. RESULTS: Eighteen patients were identified in total (11 male patients and 7 female patients) with a median age of 59.5 (range 3-83) years. The most common spinal region affected was cervicothoracic (33.3%). Limb weakness (94.4%) and urinary dysfunction (83.3%) represented the most common presenting symptoms. Preoperatively, the presence of spinal cord edema on imaging was associated with worse preoperative Medical Research Council (MRC) grade (P = .033), female sex was associated with preserved saddle sensation (P = .04), and patients receiving antiplatelet medication were associated with a higher risk of preoperative axial back pain (P = .005). Higher postoperative MRC grade was associated with higher preoperative ASIA (P = .012) and MRC grade (P = .005), and preservation of saddle sensation (P = .018). Postoperative improvements in axial back pain were associated with higher preoperative ASIA grade (P = .035) and anticoagulation treatment (P = .029). CONCLUSION: Neurosurgical intervention for SSEH yields positive outcomes and benefits patients. Patients with higher preoperative ASIA, MRC grade, and those presenting with preserved saddle sensation may experience further improved clinical outcomes after intervention.
