A Concept Analysis on Patient-Centered Care in Hospitalized Older Adults with Multimorbidity

Health personnel's experience with resident-centered care in nursing homes in Korea: A qualitative study

ABSTRACT Background The miserable work conditions faced by many nurses around the world make them particularly prone to work–family conflict. Purpose This study was designed to explain from a resource perspective how and when nurses experiencing work–family conflict are more likely to exhibit decreased levels of organizational identification and professional identification. Methods A time-separation research design was used, and data were collected from 322 nurses in five hospitals in China. All of the hypotheses were tested using conduct structural equation modeling. Results The results showed that work–family conflict had negative effects on organizational identification and professional identification via emotional exhaustion. In addition, job meaningfulness may have a buffering effect on the negative impact of work–family conflict on emotional exhaustion, organizational identification, and professional identification. Conclusions/Implications for Practice The results advance knowledge on how and when work–family conflict damages organizational identification and professional identification. Providing family support, relieving emotional exhaustion, and enhancing job meaningfulness are necessary to support the organizational and professional identification of nurses.

Work–Family Conflict, Organizational Identification, and Professional Identification Among Chinese Nurses From a Resource Perspective

Purpose: Person-Centered Care (PCC) is a key indicator of health care quality. The purpose of this scoping review is to identify the characteristics of nursing literatures on PCC and examine the theoretical and operational definition of PCC. Methods: The scoping review was conducted following the Joanna Briggs Institute’s scoping review guidance. The search terms were “patient-centered”, “person-centered”, “participant-centered”, and “nursing”; and only publications on PCC were selected. Literatures published in Korean since 2001 were searched using electronic databases, and the last search date was June 2020. Results: Nineteen publications were included. Approximately 80% of the researches were published since 2018, and the most common methodology was survey (53%). Most researches were conducted in nursing homes (63%), and 74% of the participants were health-care providers or facility staff. The conceptual framework was used in only one study. The conceptual and operational definitions of PCC both comprised multilevels, including intrapersonal, interpersonal, and organizational levels. The conceptual definitions of PCC were mainly characterized by intrapersonal elements, including individualized care, dignity, and autonomy followed by organizational attributes such as comfortable environment. However, there were some inconsistencies in the attributes survey instruments shared. Conclusion: PCC needs to be recognized as a multilevel concept. More studies such as concept analysis and scale development are warranted for conceptualizing PCC systematically.

Conceptualization of Person-Centered Care in Korean Nursing Literature: A Scoping Review

This study was conducted to understand and describe Korean nurses’ perception of person-centered care for elderly patients in long-term care hospitals. Qualitative data were collected through four focus group interviews consisting of 21 nurses working in four long-term care hospitals in Jeolla and Gyeongsang provinces. Participants completed interviews from July 19 to 30, 2018. All interviews were recorded, transcribed, and analyzed by employing the thematic analysis method. Six main themes for the attributes of person-centered care for elderly patients in the long-term care hospitals were conceptualized: respecting individual needs, walking to the end, supporting hidden dreams, becoming a family partner, helping patients live like they are at home, and changing culture. Person-centered care perceived by nurses was conducted to provide individualized nursing according to elderly’s preferences and help them discover the value and meaning of life through various activity programs. Nurses also recognized person-centered care to maintain cooperative relationships with their family members and share their decision-making process, as well as to form a physical environment and organizational culture that respects the rights and autonomy of the elderly. Based on the results of this study, it is necessary to identify the diverse needs of the elderly and develop nursing intervention programs based on person-centered care.

Nurses' Perceptions of Person-Centered Care in Long-term Care Hospitals: Focus Group Study

Background: Although it has been established that person-centered care is an essential aspect in long-term care facilities, the relationship between person-centered care and job stress among nursing staff and the effect of this relationship on their quality of life are not clear. Purpose: This study aims to investigate correlations among job stress, quality of life, and person-centered care of nurses as well as factors affecting the person-centered care abilities of nursing staff working at long-term care hospitals. Methods: Nursing staff (N= 183) in three long-term care hospitals in South Korea completed a questionnaire on job stress, quality of life, and person-centered care. Data were analyzed using the SPSS 22.0 program for descriptive statistics, using an independent t test, a one-way analysis of variance, Pearson's correlation coefficient, and multiple stepwise regression analyses. Results: Significant factors found to be associated with person-centered care included the quality of life (r = .411, p ＜ .001) and job stress (r = -.305, p ＜ .001) of the participant. The regression model with job stress and quality of life as predictor variables accounted for 29.2% of the variance in person-centered care. Conclusions/Implications for Practice: Higher quality of life and lower job stress were found to increase the person-centered care abilities of nurses in long-term care facilities.

Person-Centered Care, Job Stress, and Quality of Life Among Long-Term Care Nursing Staff.

Objectives: The purpose of this study was to identify the relationship of self-leadership, emotional intelligence, social support, and problem-solving ability of nursing college students, and to understand the factors that affect problem-solving ability.Methods: Participants were in 2nd and 3rd grade of nursing students. The collected data were analyzed by SPSS 22.0 program using descriptive statistics, t-test, ANOVA, correlation, and multiple regression analysis.Results: The significant influencing factors of problem-solving ability were self-leadership. β= 0.36 (p= 0.001), emotional intelligence β= 0.15 (p< 0.04), social support β= 0.29 (p< 0.001) and club activity β= 0.14 (p< 0.02). These factors explained 45.5% of the variance (R2= 0.46, p< 0.001).Conclusions: This study showed that problem-solving ability were influencing factors on self-leadership and it is necessary to develop various strategies to improve self leadership in nursing college students.

Effect of Self-leadership, Emotional Intelligence and Social Support on Problem Solving Ability of Nursing Students

본 연구의 목적은 간호대학생의 학업적 자기효능감, 그릿, 임상추론역량의 상관관계를 파악하는 것을 목적으로 하였다. 그리고 간호대학생의 그릿이 학업적 자기효능감과 임상추론역량의 관계에 미치는 매개효과를 탐색하였다. 본 연구는 D시에 재학 중인 4학년 간호대학생 152명을 대상으로 설문조사하였다. 자료수집 기간은 2021년 5월 18일부터 7월 2일까지 수행되었다. 수집된 자료는 IBM SPSS 22.0 프로그램을 이용하여 기술통계, 독립표본 t-test와 일원분산분석, Pearson’s 상관계수분석, 다중회귀분석 및 매개효과에 대한 통계적 유의성 검정은 Sobel test를 이용하였다. 연구결과 간호대학생의 임상추론역량은 학업적 자기효능감(r=.35, p <.001), 그릿(r=.33, p <.001)과 유의미한 상관관계가 있는 것으로 나타났다. 자기효능감은 그릿과 유의미한 양의 상관관계를 보였다(r=.56, p <.001). 분석결과, 대상자의 일반적 특성에 따른 임상추론역량은 전공만족도(F=4.18, p =.017)와 임상실습 만족도(F=6.65, p =.002)에서 유의한 차이가 나타났다. 간호대학생의 학업적 자기효능감과 임상추론역량의 연관성에서 그릿의 매개효과를 분석한 결과, 그릿은 부분매개효과를 나타내었고(Z=2.06, p =.020) 설명력은 14%이었다. 본 연구결과를 바탕으로 향후 간호대학생을 대상으로 학업적 자기효능감과 임상추론역량을 증가시키기 위해 그릿을 강화하기 위한 중재프로그램을 개발할 필요가 있다. 

Mediating effect of grit on academic self-efficacy and clinical reasoning competence of college students

Background: The purpose of this study was to examine the mediating effects of mindfulness on the relationships between self-esteem and burnout among nurses. Methods: A descriptive cross-sectional survey design was conducted using purposive sampling of 132 nurses at two university hospitals in J city and C city. It was analyzed with descriptive statistics, Pearson correlation and multiple regression analysis using SPSS 20.0 program. Results: There were significant correlations among self-esteem, mindfulness and burnout. Self-esteem and mindfulness were negatively correlated with burnout among nurses. Mindfulness acts as a mediator in explaining relationship between self-esteem and burnout. Conclusions: Our results provide evidence for the role of mindfulness in the relationship between self-esteem and burnout in professional nurses. Mindfulness appears to be a protective factor against nurses’ burnout. The results of this study suggest that mindfulness training intervention suitable for nurses would be useful to reduce nurses' high levels of job burnout and risk of turnover and increase patients' satisfaction with their care.
